Centre – South Building.
The Winter Clinic is the largest one-day comprehensive dental meeting in North America. You and your team will have the opportunity to partake in a world

The clinical program covers a broad spectrum of topics that include: periodontal disease and the immune system, oral cancer screening, maximizing your

WHO WE ARE
As early as 1890 Toronto dentists had formed as a group called the Toronto
Dental Society. This organization would reinvent themselves in 1906 to
“form a society on a basis more suited to the demands of a profession
of busy men in the most progressive city in a progressive country”. This
group continued to thrive over the years and would subsequently become
the Toronto Academy of Dentistry on May 8, 1922. They shared office
space and staff with the RCDS and the annual fee for membership was
$5.00. What was once called Clinic Night was formally changed to Winter
Clinic for the meeting on December 8, 1937. The Academy had in fact
been holding spring and winter clinics for many years before this, attracting several hundred dentists to each meeting.
As an ODA member who practices in Toronto and belongs to: North Toronto Dental Society, Toronto East Dental Society, Toronto Central Dental
Society, or the West Toronto Dental Society you are automatically a member of the Toronto Academy of Dentistry and part of this long heritage.
The Toronto Academy of Dentistry draws from the four Toronto component societies for its Board members and Committee members. The Toronto
Academy of Dentistry is the four Toronto component societies and all that
it does is on behalf of these societies.

The Job Fair will be located close to the Toronto Academy of Dentistry
Booth. Dental professionals can post staffing notices on the Job Fair Board.
Silent Auction
All registrants will receive Winter Clinic Bucks with their registration package. Additional Winter Clinic Bucks may be obtained from exhibitors and
at games throughout the exhibit floor. Registrants will bid by signing the
Bid Sheet in each participating exhibitor’s booth. At the end of the day, the
highest bidder will win the Silent Auction item, pick it up from the exhibitor’s booth and pay with the appropriate amount of Winter Clinic Bucks.
Photography Contest
See page 18 for the Entry Form to submit your photograph(s) in one of
five categories….Ontario, Nature, Action, People or Cell Phone Pics. The
contest is being sponsored by Henry Schein and prizes will be awarded in
each of the five categories. All photographs submitted will be displayed and
winners announced at the Winter Clinic.
Children’s Policy
Babies/Infants in arms (non-ambulatory are permitted in meeting rooms
and on the exhibit floor provided they are not disruptive. Strollers may be
used on the exhibit floor but are not permitted in the meeting rooms. If you
do use a stroller on the exhibit floor, you do so at your own risk. Neither the
Toronto Academy of Dentistry nor the Metro Toronto Convention Centre
will be responsible for accidents that occur due to the use of strollers.
Room 802A will be made available for the convenience of
nursing mothers and as a change area.
Complimentary Box Lunch
This year, we are offering a Complimentary Box Lunch to all registered attendees. Box lunch tickets will be attached to badges (one per registration).
These box lunches may be picked up between 11:30am and 2:00pm from
one of 10 stations located throughout the Exhibit Floor.

Primary Contact
The person named as Primary Contact on the Registration Form will be the
payer of the registration(s). Confirmation, badges, badge-holders, workshop
tickets and box lunch tickets will be mailed to the Primary Contact.
Register before midnight on Friday, October 15, 2010, after
which date registration fees increase.
Registration Categories
• Everyone must register in their professional category. Practitioners cannot
register under the “Guest” category
• All Ontario dentists and dental students registering must be a current
ODA member
• All other dentists and dental students who are from outside Ontario must
register in the Out-of-Province/Country category. Dentists registering in
this category must have a practice address outside of Ontario
• Complementary registration is extended to: 50-Year/Honorary ODA
Member Dentists, Active Members of the Canadian Armed Forces Dental
Unit (irrespective of local affiliation), and ODA Member Dental Students
Guest Category
Guests must be registered by a paid registrant in a professional category
and listed as a guest in the space provided on their registration form. The
registrant must verify that the guest is not a dentist, dental hygienist, dental
assistant, dental technologist, or dental personnel.

• Each form must include its own means of payment (credit card or cheque)

• Registration forms received after Friday, October 15, 2010 will be
processed as late registrations and the higher fee will apply

Tickets and Box Lunch Tickets
Badges, badge-holders, workshop tickets and Box Lunch Tickets will be
mailed between October 1, 2010 and October 21, 2010 for registrations
received by October 15, 2010. Badges, etc. for registrations received after
October 15 must be picked up on-site. Out of Canada documents will not be
mailed – they must be picked up on-site.
Cancellation Policy

• All cancellations will be subject to a $35 administration fee per registrant
• A $45 administration charge will apply if the badge has been mailed out
and subsequently returned
• No refunds will be issued for cancellations received after midnight on
Friday, October 29, 2010

AM – Periodontal Disease Pathogenesis… the Role of the
Immune System and Inflammatory Suppression
This session will address the current views on the etiology and pathogenesis
of periodontal diseases, the development of the critical inflammatory burden as mediated by the immune mechanism and contemporary paradigms
for management, including DNA-PC laboratory testing for pathogenic distribution and load.
PM – A Review of Contemporary Periodontal Management and
Adjunctive Oral Cancer Screening
The advances in our understanding of the nature of periodontal disease and
its intimate relationship to systemic changes have altered the way we look
at, recognize and treat these disease entities. The shifting paradigm of diagnosis and inflammatory suppression provides the clinician with a better
understanding of targeted therapy and disease control. New approaches to
management include reconstructive techniques, laser therapy and more specific antimicrobials based on the causative organisms. New tests for pathogen distribution, HPV presence, etc. will be discussed as well.
This session will review the treatment of the exposed pulp on immature
teeth. Pulp capping and pulpotomy will be discussed for the treatment of
vital teeth and apexification and revascularization will be reviewed for
non-vital teeth with apical periodontitis. The use of intracanal antibiotics
will also be presented. Additionally, for conventional endodontic treatment,
contemporary access preparations with the use of ultrasonics will be presented for the purpose of locating hidden anatomy on molar teeth.
Dr. Frederic Barnett received his DMD degree in 1978 and his Certificate
in Endodontics in 1981, both from the University of Pennsylvania, School
of Dental Medicine. He received his Board Certification in Endodontics in
1986, has served as the Director of Postdoctoral Endodontics at the University of Pennsylvania, and is currently the Vice-Chairman of Dental Medicine and Chairman and Program Director of the IB Bender Postdoctoral
Endodontic Program at Albert Einstein Medical Center in Philadelphia.
Learning Objectives:
• Familiarization with our current understanding of the pathogenesis of
periodontal disease
• Use of clinical laboratory testing
• Adjunctive oral cancer screening
• Gain an overview of current surgical approaches
Dr. Herbert Bader is a graduate of New York University Dental School, and
Harvard School of Dental Medicine postdoctoral program in periodontology. He has lectured throughout the United States and Canada for a number
of years on the subjects of inflammatory suppression, diagnosis and adjunctive oral cancer screening. He holds the position of Lecturer in Periodontology at the Harvard School of Dental Medicine and is widely published
in the literature. As an actively practicing periodontist he maintains busy
practices in Plymouth and Cape Cod, Massachusetts.

David Rosenthal is a senior lawyer whose practice is devoted to corporate,
commercial and business law, with special emphasis on advising and consulting for the dental profession. Mr. Rosenthal publishes articles regularly
and lectures frequently regarding legal matters affecting dentists.

dental professional. We will present the benefits of regular exercise and how
they relate specifically to dentistry. We will discuss the unique musculoskeletal needs of the dental professional and how fitness can impact function
in the operatory. Can you, the busy dental professional, best match your
musculoskeletal needs with a regular exercise routine? Is time a problem
for you? Is commitment to a regular routine difficult for you? Are you a
busy dental professional with no time? Are you someone living with pain

routines as well as some alternative approaches to getting a little regular

Timothy J. Caruso is a practicing Physical Therapist who has focused
his professional expertise in the area of manual therapy and orthopedics,
specifically neuromusculoskeletal disorders. He is qualified in mechanical
diagnosis and therapy from the McKenzie Institute USA and continues in
private practice in treatment of spinal disorders. He continues with direct
patient care at Shriner’s Hospital for Children in Chicago and Community
Physical Therapy, a private physical therapy practice. He is a Certified
Ergonomics Assessment Specialist and chairs the Ergonomics Committee
at Shriner’s Hospital for Children in Chicago.

Cosmetic dentistry is a major component of dental practice today. This panel presentation will discuss the ethical issues related to the provision and
promotion of this dental service from the dentist’s perspective, the ethics /
patient perspective and the legal perspective. How ought dentists reconcile
the ethical tension that exists between one’s duty to heal and prevent disease
with interventions that have less to do with health promotion and more to do
with beauty? How do dentists manage the conflict of interest that can arise
between the obligation to treat disease/promote health and the profit orientation of private practice? Does the ethical principle of patient autonomy outweigh other ethical principles like beneficence and non-maleficence? Learn
why ethical consideration should be given to “what ought to be done, why
ought it be done and how ought this be done” (quote from Dr. B. Secker)
when providing cosmetically based treatments. What are possible adverse
consequences both professionally and legally when ethical consideration is
not given? Where cosmetic dentistry is being considered: should dentists
use veneers or crowns instead of orthodontic treatment? Should CAD/CAM
restorations be used instead of “conventional” crowns? Should posterior
amalgams be replaced with tooth coloured restorations? When should anterior teeth be extracted and replaced with implant supported prostheses
instead of receiving endodontic/periodontic/restorative therapy? What are
reasonable fees for cosmetic dentistry? When should specialists be providing the services rather than a general dentist? These and other “real life”
ethical challenges facing dentists everyday in the clinical practice of cosmetic dentistry will be discussed.
Karen Faith is a Bioethicist, Consultant and Educator as well as a former
Director of the Ethics Centre at Sunnybrook Health Science Centre. She is
a member of the Joint Centre for Bioethics and an Adjunct Lecturer for the
Factor-Inwentash Faculty of Social Work at the University of Toronto. She
addresses inter-professional groups on topics pertaining to moral distress
and the ethical challenges we face in healthcare. Karen continues her work
in Clinical ethics education as an Organizing Board Member for the Clinical Ethics Summer Institute.

Dr. Cary Letkemann has been a Fellow of the American College of Dentists
since 1998 and currently serves as Vice President of the Ontario Section.
He is the Past President of the Hamilton Academy of Dentistry, Canadian
Academy of Restorative Dentistry and Prosthodontics as well as the International Dental Study Club. Dr. Letkemann also has Fellowships in the
International College of Dentists, the Academy of Dentistry International
and the Pierre Fauchad Academy and maintains a private general practice
in Ancaster, Ontario.
Dr. Rollin Matsui received his DDS dental degree from the Faculty of Dentistry, University of Toronto in 1979 and his LL.B. law degree from Osgoode
Hall Law School, York University in 1991. He is a fellow of the American
College of Legal Medicine, a member of Omicron Kappa Upsilon, Honour Dental Society and holds the following fellowships: F.A.C.D., F.I.C.D.,
F.P.F.A. and F.A.D.I. He was an elected RCDSO. Councillor from 19831986 and served on the College’s Complaints Committee and Discipline
Committee. He provides dental-legal lectures for dentists as well as dental
students at the Faculty of Dentistry, University of Toronto. He is the Oral
Health Journal Editorial Board Member for Ethics and Jurisprudence. He
maintains a full-time law practice in Richmond Hill and a part-time dental
practice in Toronto, Ontario. In his law practice, he primarily acts for dentists in complaint and professional misconduct related matters and provides
legal advice regarding business agreements involving dentists in various
working relationships.
Dr. Emmanuel Rajczak is primarily a full-time prosthodontist whose principal practice emphasis is in the field of crown and bridge prosthodontics,
occlusion, temporomandibular joint therapy and implant prosthodontics,
and his lectures are concerned mainly with the how to-do-it aspect of complex restorative dentistry. He is also mentor to a Hamilton based study club
where he instructs primarily in the diagnosis and treatment planning of
complex restorative cases. Dr. Rajczak has lectured to graduate students
at the University of Toronto, Temple University and to continuing dental
education students at the University of Buffalo, the University of Nebraska,
Marquette University and Baylor University as well as to dental groups in
Canada, the United States and Europe.

Panoramic radiographs are complicated images depicting the anatomy of
the maxillofacial region as complex superimposed shadows. Dental practitioners may frequently feel frustrated determining which shadows should
be a concern and which ones are normal. Errors in image acquisition often
exacerbate this challenge. This short review will help participants become
more comfortable interpreting panoramic radiographs. Panoramic anatomy
will be demystified and through a case-based approach, panoramic artifacts,
common variations of normal and pathology will be presented.
The digital world has arrived in dentistry. New technologies are being developed and aggressively promoted for daily application in clinical dentistry from diagnosis to treatment. Some of these technologies are designed for
use in prosthodontics. This presentation will give an overview of digital impressioning and CAD/CAM manufacturing as it applies to prosthodontics.
Dr. Susanne Perschbacher received her DDS at the University of Western
Ontario and completed her specialty training and MSc in Oral and Maxillofacial Radiology at the University of Toronto. She is a Fellow of the Royal
College of Dentists of Canada and a Diplomate of the American Board of
Oral and Maxillofacial Radiology. Dr. Perschbacher is an Assistant Professor in the Department of Radiology, University of Toronto, where she
teaches in the undergraduate and postgraduate program. She also works
in a private radiology practice in Toronto and is a member of the editorial
board of Oral Health magazine.
Dr. Dmitri Svirsky is a Diplomate of the American Board of Prosthodontics
and Fellow of the Royal College of Dentists of Canada. He completed his
training and has taught at the Boston University School of Dental Medicine, Post-Graduate Program in Prosthodontics. Dr. Svirsky’s private practice in Toronto is limited to conventional and implant Prosthodontics.

The January 2010 earthquake in Haiti devastated the country resulting in
numerous casualties. Canada deployed Disaster Victim Identification (DVI)
teams, led by the RCMP, to identify Canadian decedents and allow the repatriation of their remains. This session will provide an overview of DVI in
Canada, the experiences of the military forensic odontologists attached to
the DVI teams including case studies and the challenges experienced with
obtaining ante-mortem records.

AM – All-Ceramic Restorations, Minus the Sales Hype
In restorative dentistry there are traditional materials with which most of us
are very familiar. At the same time there is a barrage of information to sort
through as we decide whether to incorporate unfamiliar new technologies
and materials into our practices.
In this presentation the various all-ceramic restorative options will be reviewed in detail and suggested indications and limitations for these materials will be presented. We will also discuss why these materials fail and how
we can treatment plan, accomplish preparations and design occlusion to
avoid failure.
Learning Objectives:
• To understand the physical properties for the various all-ceramic materials
• Which all-ceramic systems are most likely to succeed in given situations
Major Richard Groves enrolled in the Canadian Forces in 1975 and graduated from the University of Saskatchewan College of Dentistry in 1990. He
has served at numerous military bases in Canada, with operational tours to
Alert, Northwest Territories, the United Nations Forces in Cyprus, and the
military response to the crash of SwissAir 111 in Nova Scotia. For the past
three years he has been the forensic odontology advisor to the Director of
Dental Services.
Major Ellen Sim enrolled in the Canadian Forces in 1988. She graduated
from Dentistry at Dalhousie University in 1992 and has served at many locations within Canada. She has completed operational missions in Afghanistan and Haiti. She specialized in periodontics at the University of Manitoba
in 2009 and is currently the Canadian Forces Atlantic Regional Periodontist
located at Canadian Forces Base Halifax.
• Which all-ceramic restorations must be bonded and how critical it is to
their survival
PM - Bonded Ceramic Restorations-Procedures, Indications
and Limitations
With traditional fixed prosthodontic restorations we have a clear vision of
preparation design and cementation procedures. However, with the selection
of bonded ceramic materials and resin cements available today, it is difficult
for the practitioner to keep up with this basic information for every system.
In this presentation we will examine what ceramic materials are available
for bonded restorations as well as what advantages they may offer. We will
discuss the preparation details critical for anterior and posterior bonded
ceramic restorations, including CAD/CAM produced restorations. We will
also review veneer provisionalization techniques, as well as bonding techniques and resin cement materials available.

Dr. James Kessler is a 1977 graduate of the University of Oklahoma College of Dentistry. During his career he has spent fourteen years in dental
education at the University of Oklahoma and the Medical University of
South Carolina as well as 20 years in private practice in Tulsa Oklahoma.
Recently, Dr. Kessler was the Chair of the Division of Restorative Dentistry at the Oklahoma University College of Dentistry. In October 2010 Jim
moved to Key Biscayne, Florida to become the Chair of the Department of
Education at the Pankey Institute for Advanced Dental Education.

Most patients who have periodontitis respond to routine management,
which can include scaling and root planning and periodontal flap surgery.
In general, 80% of patients will respond to routine therapy, but the problem
remains: what do we do about those who do not respond? Are there any new
developments available on the horizon where more patients will respond
to treatment and that treatment might be less invasive, or more predictable, or both? This session will review the administration and side effects of
both systemic and locally administered antimicrobial (LAAs) medications,
identify the need for adjunctive topical therapies and identify new trends in
non-surgical periodontal therapy.
Dr. Jim Yuan Lai is Head and Graduate Director of Periodontology at the
University of Toronto, Faculty of Dentistry. He is a Fellow of the Royal

that are perpetuated through incorrect or poor positioning. Dental professionals are constantly working under stressful conditions. Correct breathing patterns to decrease stress will also be taught to participants.

Smile dynamics plays a major role in diagnosis and treatment of various
orthodontic problems. This presentation will discuss the application of
smart wires to correct gummy smiles, negative smiles, open bites and canted occlusal planes. Step by step appliance design will be presented.
Dr. Ravindra Nanda is UConn Alumni Endowed Chair and Professor and
Head of the Department of Craniofacial Sciences and Chair of division of
Orthodontics, University of Connecticut, Farmington, Connecticut. He has
done extensive research during the last forty years in the areas of cleft lip
and palate, orthopedic forces and on long-term growth with orthognathic
surgery in adolescents. In recent years, his major thrust has been in development of orthodontic wires, clinical orthodontic trials and application of
biomechanics in a busy orthodontic practice.
The number one cause of lab remakes is shade. Shade is also the only area
in dentistry that will immediately alert the patient’s untrained eye that something is very wrong with their new restoration. We all need to understand
the four elements in shade selection and colour communication – you will
learn what these are and how to implement them immediately in your practice. This well organized, evidence based approach to shade selection and

Dr. Sara Werb received her DDS from the University of Toronto, and her
Masters in Education from Canisius College in Buffalo, NY. Dr. Werb then
received her Diploma in Paediatric Dentistry from Jacobi Medical Centre in Bronx, NY. She has her own private practice in Toronto, is a Dental
Consultant for Bloorview Macmillan Rehab Centre, a Pre-Clinical Instructor in undergraduate Paediatric Dentistry at the University of Toronto and
is Dental Director for the non-profit organization Pediatrician for Central
America’s Children.
Char Sweeney began her career in dentistry in 1979 as a chair side Dental
Assistant. She attended Ferris State University and the University of Alaska
and received her degree in Allied Health Science Dental Technology. She
returned to private practice as a business assistant, and eventually, a practice administrator. She was an independent practice management consultant prior to joining Linda L. Miles & Associates in 1992. Char’s experience and enthusiasm for dentistry, as well as her love of people, enable
her to offer concrete solutions to the daily stumbling blocks all practices
encounter. She has worked in both the business and clinical aspect of dentistry and understands the unique challenges that both areas face. Her goal
is to help clients create a sound business plan and assist everyone in reaching their true potential.

This one day course is designed specifically for the general practitioner and team members that wish to incorporate the Invisalign® system into their practice. This case
based training provides participants with the clinical and
operational confidence to successfully treat a range of
highly predictable cases.
A lecture and workshop designed for GPs who currently
offer Invisalign in their practice. Advanced topics and
techniques using Invisalign are discussed to help dentists
effectively select and treat more complex malocclusions.
We recommend that participants have experience submitting and finishing Invisalign cases prior to attending
Invisalign: Clear Essentials II.
In addition to providing your entire team with essential
clinical, operational and marketing skills, the course
presents a full range of support resources specifically designed to meet the needs of the non-orthodontic practice.
Tuition covers the doctor and up to 4 team members for
the full-day course.

Bernie Teitelbaum, B.Comm
Bernie Teitelbaum holds a Bachelor of Commerce degree (1971) from the University of Toronto and
has more than 33 years of experience in the dental industry in a wide variety of executive and management functions ranging from finance and operations to sales and marketing.
For the past 12 years he has served as the Executive Director of the Dental Industry Association of
Canada (DIAC) and is responsible for moving the Association’s programs forward from conception
to completion. He is also the Association’s spokesperson and watchdog on Market Ethics.
Bernie is involved in all of the Association’s programs including membership, regulatory affairs, market research, business and employee development, industry relations and conventions.
Prior to joining DIAC, Bernie was Director of Sales and Marketing for the Dentsply division of Dentsply Canada Ltd., Sales Manager of the Laboratory Products division of Dentsply Canada Ltd.,
Treasurer and Controller of Dentsply Canada Ltd., and Assistant Controller of Denco, division of
McGaw Supply Limited.
Bernie had the honour of participating in a panel discussion on the Grey Market with Drs. Cal
Torneck, John O’Keefe, Paul Santerre and Arthur Conn at the 2008 Winter Clinic. He has also conducted Grey Market Clinics at the 2009 Pacific Dental Conference, the 2010 Les Journees Dentaires
Convention and several study groups.
Bernie has published articles on the Grey Market in both Oral health as well as the Journal of the
Canadian Dental Association.

Donald Garth Woodside
CM, DDS, MSC, FRCD(C), PHD, FRCS, FDS, FDSRCS(ED)(AD HOM)
Dr. Donald G. Woodside received his DDS from Dalhousie University, Halifax, Canada, where he was
awarded the Gold Medal in Dentistry; and he received his M.Sc. in Orthodontics from the University
of Toronto. From 1962-1993 he was Professor and Head of the Orthodontic Department, Faculty
of Dentistry, University of Toronto, where he is currently Professor Emeritus in the Discipline of Orthodontics.
His published work ranges through the areas of both human and experimental mandibular growth
and the clinical use of functional appliances. He was the 1987 co-recipient of the S.I.D.O. Award
for the most outstanding article in international orthodontic literature during the previous two years.
Dr. Woodside has been the recipient of many international awards in orthodontics and an invited
honorary lecturer throughout the world. He has been both Mershon and Salzmann honorary lecturer
for the American Association of Orthodontics. In 1989 he received an honorary doctorate from the
Karolinska Institute in Sweden, and in 1990 he received the Ketcham Award from the American Association of Orthodontists. In 1991 he was the Sheldon Friel Memorial Lecturer for the European
Orthodontic Congress. In 1994 he received a Fellowship in Dental Surgery-by-Election from the
Faculty of Dental Surgery, Royal College of Dental Surgeons of England, and he is a recipient of the
Canadian Dental Association Distinguished Service Award.
In 2005, Dr. Woodside was awarded a Fellowship in Dental Surgery ad hominem of the Royal College of Surgeons of Edinburgh, as part of the 500th Anniversary Celebrations of the founding of the
College. Dr. Woodside was also elected to Honorary Membership in the World Federation of Orthodontics during its meeting in Paris in September 2005.
Dr. Woodside is a Fellow in the Royal College of Dentists of Canada, a Diplomate of the American
Board of Orthodontics and a Fellow in the American College of Dentists. He is also an honorary
member of the Italian Orthodontic Society, the British Society for the Study of Orthodontics, the
Dutch Society of Orthodontists, the South African Society of Orthodontists, the Taiwan Association
of Orthodontists and the Ontario Association of Orthodontists. In 1996 he was named a Member of
the Order of Canada which is the highest civilian honour awarded by the Government of Canada. In
2003 he was awarded the Queen’s Jubilee Medal.
In addition to his research and teaching, Dr. Woodside maintained a private orthodontic practice in
Toronto until recently.
