McGuire has been employed by the Boyceville district for 17 years, teaching fifth and sixth grades at Tiffany Creek Elementary School. He began performing assistant principal duties there in 2005, primarily in the areas of transportation and discipline. The school has an enrollment of about 400 students. North Bay Shopping Center, 662 N. 4th Street, Tomahawk B2S07 NEL240 NATIONAL SCHOOL LUNCH AND BREAKFAST PROGRAMS RELEASE STATEMENT – 2007-08 SCHOOL YEAR The Tomahawk School District has announced its policy for children unable to pay the full price of meals served under the National School Lunch Program and School Breakfast Program. Each school office and the central office has a copy of the policy, which may be reviewed by any interested party. The following household size and income criteria will be used for determining eligibility for the 2007-08 school year. Children from families whose annual income is at or below the levels shown are eligible for free and reduced price meals. FAMILY SIZE INCOME SCALE For Determining Eligibility for Free and Reduced Price Meals ANNUAL INCOME LEVEL MONTHLY INCOME LEVEL Family Free Reduced Price Free Reduced Price (Household) Must be at or below Must be at or between Must be at or below Must be at or between Size figure listed figures listed figure listed figures listed 1 $13,273 $13,273.01 and $18,889 $1,107 $1,107.01 and $1,575 2 $17,797 $17,797.01 and $25,327 $1,484 $1,484.01 and $2,111 3 $22,321 $22,321.01 and $31,765 $1,861 $1,861.01 and $2,648 4 $26,845 $26,845.01 and $38,203 $2,238 $2,238.01 and $3,184 5 $31,369 $31,369.01 and $44,641 $2,615 $2,615.01 and $3,721 6 $35,893 $35,893.01 and $51,079 $2,992 $2,992.01 and $4,257 7 $40,417 $40,417.01 and $57,517 $3,369 $3,369.01 and $4,794 8 $44,941 $44,941.01 and $63,955 $3,746 $3,746.01 and $5,330 For each additional household member, add +$4,524 + $4,524 and + $6,438 + $377 + $377 and + $537 Application forms will be sent home on the first day of school with a notice to parents or guardians. To apply for free or reduced price meals, households must fill out the application and return it to the school as soon as possible. Additional copies are available at the office in each school. The information provided on the application will be used for the purpose of determining eligibility and may be verified at any time during the school year by school or other program officials. Applications may be submitted at any time during the year. To obtain free or reduced price meals for children for whom households receive FoodShare or Wisconsin Works (W-2) benefits, an adult member needs only to list the names of the school children, give FoodShare or W-2 case number for each child, sign his/her name and return the application to the school office. For the school officials to determine eligibility for free or reduced price meals of households not receiving FoodShare or W-2, the household must provide the following information requested on the application: names of all household members, and the social security number of the adult household member who signs the application. In lieu of a social security number, the household may indicate that the signer does not possess a social security number. Also, the income received by each household member must be provided by amount and source (wages, welfare, child support, etc.). Under the provisions of the free and reduced price meal policy, the Food Service Director will review applications and determine eligibility. If a parent or guardian is dissatisfied with the ruling of the official, he/she may wish to discuss the decision with the determining official on an informal basis. If the parent/guardian wishes to make a formal appeal, he/she may make a request either orally or in writing to: District Administrator, Allan Prosser at 453-5551. If a hearing is needed to appeal the decision, the policy contains an outline of the hearing procedure. If a household member becomes unemployed or if the household size changes, the family should contact the school. Such changes may make the household eligible for reduced price meals or free meals if the household income falls at or below the levels shown above, and they may re-apply at that time. In certain cases foster children are also eligible for these benefits. If a household has foster children living with them and wishes to apply for free or reduced price meals, the household should complete an application for a family of one or contact the school for more information. The information provided by the household on the application is confidential. Public Law 103-448 does authorize the release of student free and reduced price school meal eligibility status to persons directly connected with the administration and enforcement of federal or state educational programs. In the operation of the child nutrition programs, no child will be discriminated against because of race, color, national origin, sex, age, or disability. If any member of a household believes they have been discriminated against, they should write immediately to USDA, Director, Office of Civil Rights, Room 326-W, Whitten Building, 1400 Independence Avenue, SW, Washington, D.C. 20250-9410. Any questions regarding the application should be directed to the determining official, Darlene Machel, who may be reached at the Food Service Office at 453-2106, Ext. 310. Children from the same household share money from the same account, however, each child uses a different pin number to identify purchases made. It also allows younger siblings and parents the opportunity to make meal purchases from the family account when visiting at school. The 2007-08 meal prices for the Tomahawk School District have not changed and are as follows: Breakfast Lunch Elementary $1.10 $1.65 Middle and High School $1.10 $1.95 Milk $ .30 $ .30 Reduced meals $ .30 $ .40 Adult $1.65 $2.70 The meal system is a pre-pay debit system, making it necessary to have money in the account before purchases can be made. Parents are urged to maintain an adequate amount of money in their family meal account to cover the cost of purchases made by their children. For your convenience the following prepayment schedule is listed.
