Registration Policies
Method of Payment
Registration must be accompanied by a deposit of at
least 50% of each student’s registration fee. Payment
balance is due no later than May 1, 2012.
Selects Hockey Training will automatically debit your
credit card if payment is not made by May 1, 2012.
Registration forms received after May 1, 2012 must be
accompanied by payment of the full registration fee.
NON-NEGOTIABLE PAYMENTS WILL BE SUBJECT TO A
$35.00 SERVICE CHARGE.
Enrollment Confirmation
Selects Hockey Training will contact you to confirm the
enrollment of each registration form submitted. All of
our programs are offered on a first come, first serve
basis. There is no reduction in registration fees for late
enrollment or early departure from a program. It is
strongly suggested that you submit your registration
form and deposit well before the May 1, 2012 deadline.
Cancellation Policy
Payments are non-refundable in the event of student
cancellation or termination of program unless
cancellation insurance has been purchased.
SELECTS HOCKEY
Conditions of Enrollment
Selects Hockey Training reserves the right to terminate
the contract of any student if determined that it is in the
best interest of the other participants or the program.
Selects Hockey Training also reserves the right to
reassign students if we feel their program choice does
not fit within evaluation guidelines. Program adjustments
are made with the player’s best interest in mind.
Grouping is subject to final enrollment numbers, and
students may be combined if enrollment does not support
separate programs. Selects Hockey Training reserves
the right to cancel camps if enrollment does not support
its operations. In the case that Selects Hockey Training
cancels a camp, payments will be refunded in full.
Late Payments/Walk-in Registrations
Players will not be allowed to attend a camp until
registration fee is paid in full. Players that register
on-site or who choose to make final payment on-site
MUST pay with VISA, MasterCard, American Express,
or money order – cash and personal checks will not
be accepted.

Liability Waiver (This release must be signed prior to the start date of any camp)
I hereby agree that I shall provide health insurance to cover any personal injury or property
damage incurred by the player listed above while he/she is participating in any activity
conducted by Selects Hockey Training, or while on any premise leased or otherwise
controlled by Selects Hockey Training. The undersigned assumes the responsibility for any
and all risk of damage or injury that may occur to the above listed player as a participant
in Selects Hockey Training, both on and off the ice. Additionally, the undersigned hereby
releases demands, rights, or causes of action present or future, whether known or
anticipated and resulting from or arising out of any incident involving the above listed
player. I also give my permission to have the listed player admitted and attended to for
medical or dental treatment in the event of illness or injury. I understand that Selects
Hockey Training will not tolerate any uncontrollable behavior, or the possession or use
of alcohol, tobacco, drugs, or any other controlled substance or weapons of any kind. If
the above listed player is found to be in violation of this policy, he/she will be immediately
terminated from the program, thereby forfeiting all moneys paid. By signing this waiver,
I agree to the enforcement of these policies and hereby grant Selects Hockey Training
permission to inspect all personal items of the player at any time on or off the premise in
relation to the program. I understand that dates, times, and prices are subject to change.
