safety measures recognized by the MSQH and the Malaysian Council on Chronic Dialysis Treatment
Standards.

The MSQH may accredit any healthcare facilit� defined by
The Private Healthcare Facilities and Ser�ices Act 1998,
which defines healthcare facilit� as any premises in which
one or more member of the public receive healthcare
ser�ices. Similar healthcare facilit� in the public sector is
also eligible to be sur�eyed and accredited.
To be eligible for an accreditation sur�ey by MSQH, a
healthcare organization shall:
Be a member of MSQH;
Be a healthcare organization operating in Malaysia,
either in the public or private sector;
Be a provider of ser�ices for which the MSQH have
applicable Accreditation Standards;
Have a cur�ent and valid "license" as required by the
Minist�� of Health, or any other relevant healthcare
reg�lating body;
Have been in f�ll operations for a minimum of t�elve
(12) months prior to the sur�ey date;
Ensure the availabilit� of all ser�ices necessar� to f�lfill
the organisation's mission and objectives. (These
ser�ices may be provided on site, or may be provided
off-site by par��erships with acceptable communit� or
regional resources);
Complete and ret�r� a "Sur�ey Application For�" to
MSQH. The application must be sig�ed by the
Person-in-charge of the dialysis cent�e or an equivalent
person with overall authorit� and responsibilit� for the
healthcare facilit�.
CHRONIC DIALYSIS SURVEY
ELIGIBLITY CRITERIA
The Chronic Dialysis Treat�ent Standards is applicable to
all haemodialysis facilities and ser�ices in Public sectors,
Private sectors and NGO’s. These facilities and ser�ices are
either hospital-based or ‘free standing’ and provide only
chronic haemodialysis t�eat�ent.
The Chronic Dialysis Treat�ent Standards were developed
in collaboration bet�een the members of the Malaysian
Societ� of Nephrolog�, National Kidney Foundation,
Minist�� of Health (MOH) and MSQH. These standards
were developed based on the ISQua Accreditation
Federation Council principles and philosophy on standards
development.
The pur�ose of these standards was to ensure safe medical
practice, patient safet� and qualit� ser�ice at the
haemodialysis facilities and ser�ices.
The standards cover the following areas:

At the accreditation sur�ey, the sur�eyors identif� and commend on areas of excellence as well
as highlight any oppor��nities for improvement, using cur�ently accepted standards as the
benchmark. The obser�ations of the sur�eyors, together with individual recommendations on
the accreditation stat�s by each member of the sur�ey team are collated by the Chief Sur�eyor,
who will for�ard this composite result with a final Chronic Dialysis Treat�ent Sur�ey Repor�
to MSQH.
The final accreditation stat�s decision is made based on a review of the Chronic Dialysis
Treat�ent Sur�ey Repor� and recommendations of the sur�ey team by a panel of t�o (2)
Councillors, selected from the members of the Malaysian Council of Healthcare Standards
(Chronic Dialysis), a body chaired by the National Kidney Foundation Malaysia.
Names of organisation shall be ex�unged before the sur�ey repor� are sent to the Councillor
for voting. Each Councillor vote on the repor� individually and they are not infor�ed who the
other councillors are. The results from the three (3) Councillors are then agg�egated by the
MSQH Secretariat. This will deter�ine the Accreditation Stat�s of the organisation.
Three categories of Accreditation awards may be awarded, depending on the level of
compliance to standards attained by the healthcare facilit� which is undergoing the
accreditation sur�ey process

A four-year Accreditation is awarded to those facilities, which in the opinion of MSQH,
substantially comply with the MSQH healthcare standards. It should be noted that the
healthcare facilit� must achieve substantial compliance in all Core Standards and other Safet�
Standards, to achieve four (4) - year Accreditation.
Accreditation cannot be awarded to a facilit� in which the sur�eyors have obser�ed and
repor�ed that a sig�ificant number of standards are not complied with. Facilities who are not
accredited are encouraged to implement the recommendations made in the Sur�ey Repor�
and to re-apply for sur�ey. It is recommended that a minimum of t�elve (12) months should
elapse, to allow time for remedial actions and rectification works, before the nex� sur�ey is
under�aken. The final award decision and the recommendations are for�arded by MSQH to
the Chief Executive Officer (CEO), the owner, or the chair of the gover�ing body of the sur�eyed
facilit�. Release of any confidential infor�ation per�aining to the facilit� will be subject to
written consent from the CEO, the owner or the chair of the gover�ing body.
A Delayed Accreditation is awarded to those facilities which have met the requirements of most
of the standards. A facilit� awarded a delayed accreditation is offered the oppor��nit� to
undergo a Re-Assessment within the six (6) months period. During the re-assessment, only
those areas of deficiency noted in the initial sur�ey are visited. However, this does not exclude
visits to other areas deemed relevant by the sur�eyors. The facilit� should have taken action on
the recommendations which were recorded by the sur�eyors at the initial sur�ey, and should
achieve substantial compliance to MSQH standards in order to qualif� for the four year award.

Yong Ting joined MSQH as Technical Officer in
December 2009. She is one year sabbatical away in
May 2013 and returned to MSQH in July 2014.
Her previous work experience include industrial
training as laboratory assistant at Bukit Mertajam
Specialist Hospital and over 2 years of working
experience in health care and retail in a private
pathology laboratory. Prior to joining MSQH, she
worked as a R&D executive in a food based company
until November 2009.

August 2014. She graduated with a Diploma in

joining MSQH, she worked in Ampang Puteri
Specialist Hospital as a State Registered Nurse for 5
years with experience in Medical-Surgical and
Emergency Services.
En Saifulhazmi Salihin was awarded the ISQua Fellowship in
November 2014. The ISQua Fellowship Programme is a
‘learner led’ programme that encourages participants to avail
themselves of a range of learning opportunities that already
exist, such as conferences; and to undertake opportunities,
created by ISQua, such as webinars, case studies, debates and
e-learning modules.
Each month ISQua introduces the Fellowship Programme
Participants who have most actively engaged with the ISQua
