2014 Summer Camp Field Trip Permission Forms

Thank you for choosing Camp Wonder for your child’s 2015 Summer care. We have a fun and exciting
Summer planned with lots of activities and field trips. Below you will find all of the permission forms
needed for Camp. Please print and complete ALL forms and turn in to Wonder Years by April 15th, 2015.
Wonder Years
Field Trip Rule
1.
All children must wear camp T-shirts on all field trips, including swim days. This makes it easier
to spot children that are with us in public locations. Children not wearing their camp shirt will be
required to return home to get it before busses leave for the day. Children also need to be
dressed in comfortable clothes, socks, and tennis shoes.
2. On swim days each child needs to wear his/her swimwear along with our summer camp shirt.
We also require each child to bring a canvas bag with their name on it, a labeled towel,
underwear, and slip on shoes. We have all the children change out of their bathing suits prior to
leaving the park. The slip on shoes helps us exit the pool area quicker.
3. Busses are scheduled to leave at 8:45 each morning. Children attending the field trip must be
here by 8:30 to allow for time to use the restroom, fill water bottles, and go over the day’s
schedule prior to leaving the building.
4. Sunscreen needs to be applied before arriving at school for the morning. On swim days our
teachers will reapply sunscreen at lunch time. Any other days, our teachers will help children
reapply sunscreen prior to our afternoon playground time. NO AEROSOL SUNSCREENS WILL BE
ALLOWED IN THE BUILDING.
5. Children are expected to bring a labeled empty water bottle to school for their field trips. We fill
these bottles before leaving each morning and refill them throughout the day as necessary. We
want the children to stay well hydrated! Children are required to be responsible for their water
bottles at all times.
6. Children must be respectful all the time both in and out of our facility. We visit many crowded,
public places throughout our summer and expect the children to be on their best behavior at all
times. The children are expected to be polite to others they happen to meet, respectful of the
places and people we visit, and follow directions for their safety at all times. Any children that
are not on their very best behavior may be excluded from field trips.
7. In the event that an outdoor field trip is cancelled due to storms or heat advisories, we will do
our best to take the children on a comparable trip indoors. We have a few options included in
your field trip packet to help with days that bring inclement weather.

To ensure the safety of your child it is important for Wonder Years Staff to know any pertinent
medical information past and present. Please list any medications, both prescription and OTC, and
medical conditions. In the event of an emergency it is important to have access to this information
to know how to best care for your child. Examples include but are not limited: ADD/ADHD (even if
medication is not being administered during summer months), Nursemaid Elbow, Asthma,
Allergies, Seizures, ect.

Swimming Permission
On Wednesdays and Fridays our teachers take the children swimming at Alligator
Creek in Civic Park O’Fallon. Our teachers assess each child’s swimming skills over
the course of the first few swim days. If the children prove they are comfortable,
confident swimmers, they will be allowed to use the lazy river, vortex, deep pool,
and slides under constant supervision. Children are required to report to the
teacher before and after using the diving boards. Our teachers stay near the slides
and ladders of the deep pool, not near the diving boards. If you feel your child is
capable of jumping off the diving board, please fill out permission below.
Children are not allowed to bring swim aids to the pool. If your child would prefer
to swim with a life jacket, we will borrow a Coast Guard Certified one from the pool.
Children may bring a pair of goggles to the pool but are required to be responsible
for them. If your child’s goggles are lost or stolen, the teachers will not be able to
help locate them.
