o one likes being sick. But when it happens, it sure helps to have someone with the know-how who can quickly figure out what’s wrong,
devise a plan to make it go away, and answer all our questions without
reservation, no matter how much hand-holding might be needed. For that
reason we turned to Castle Connolly, the physician-led, patient-trusted medical
data research group whose job it is to evaluate doctors across the country,
for a comprehensive directory of the best medical talent in our area. And from
this list, we selected six physicians and asked them to debunk myths and clarify
questions specific to their chosen specialties.
To maintain its database, Castle Connolly’s experts routinely turn to leaders
in the medical profession to identify the MDs who are believed to be the best in their
respective specialties and fields. It is from this resource that they assembled
their final selections for us to share with you. This list, our most comprehensive
to date, includes 393 doctors in fourty-seven specialties. Hopefully you won’t
need to refer to it, but we suggest you keep it handy, just in case.

We caught up with Dr. Ghofrany just as she’d finished
delivering twins via cesarean section. This led us to start
with the following...
What’s with all the C-sections?
Are they just a matter of convenience,
and an easier way for the doctor
to make more money?
I’m glad you asked. Let me
dispel this myth: C-sections
are not ‘better for the doctor’
and we don’t get paid more
for one delivery method versus
another. After a C-section we
need to see the patient for all
four mornings they’re in the
hospital, versus two for a vaginal
delivery. We don’t get paid extra
for the extra visits, and we don’t
get paid more if someone is in
labor for ten hours versus if the
baby pops out. In most instances
we get paid a global fee from
the insurance company. This
includes thirty-two weeks of
pregnancy care plus delivery
and post-delivery visits. And
we don’t get paid until the end.
As for why the C-section rate
has been higher, when you
look at the patient population
there are a lot of older pregnant
women these days, many of
whom, unfortunately, are also
overweight. And more people
are having twins. Just the obesity
rate and twins alone is enough
to increase the C-section rate.
Neither delivery method is
‘better’ than the other. I had
both [methods] with my children.
Herpes and HPV seem to be in
the news a lot lately. Has this
affected your practice?
Sadly, I’m hearing from many of
my married patients that they

fear their husbands are
unfaithful. They’re worried
about these viruses and
infections and they want to
get tested. I’ve been hearing
that more and more.
It’s a common misconception
that herpes is always relegated
to people who are promiscuous.
Forty percent of us may carry
it and not know it.
Another common [myth] is
that if you have HPV (human
papillomavirus), you will
have cervical cancer. Carrying
[HPV] is extremely common.
About 90 percent of us carry
it at some point in our lifetime,
though many of us don’t ever
know we carried it, and far, far
less than 1 percent will actually
get [cervical cancer]; there are
only about 5,000 to 8,000
cases of cervical cancer in
America per year.
Any trends in birth control?
More of my young patients are
using IUDs. I have to remind
them to also use condoms.
What questions should a patient
concerned about infertility be
prepared to answer?
We send many of our patients
to fertility specialists, but we
do initiate workups when
appropriate. A patient should be
able to tell us about her general
health, her partner’s health, her
menstrual cycle—how often
she gets her period, how long it
lasts—and her sexual history.
Isn’t it hard to determine on a CT scan or X-ray what a spot on
a lung really is?
Yes, but as part of the process we ask where the patient was born,
where they grew up, whether they were born in parts of the
country where there are certain organisms in the soil. And we
have something called a ‘tumor board.’ There are twenty to thirty
doctors—radiologists, oncologists, thoracic surgeons—[who] review
all the patients being seen and collectively talk about the best
approach to come to a consensus. We also look at the metabolic
activity on a PET scan.
Aren’t more nonsmokers getting lung cancer?
It feels like a new trend. About 15 percent of women who have lung
cancer have never smoked. Most of the patients who have [genetic]
mutations have never been smokers, many Asians, but also lots of
non-Asians. We don’t quite understand what’s going on.

What’s new in pulmonology at Norwalk Hospital?
We’ve been designated as a Lung Cancer Center of Excellence, one
of only six in the United States. We conduct lung-cancer screening
for free for certain patients in a high-risk category—we do a
low-dose CT scan. This has been found to reduce lung cancer deaths
by 20 percent. Our ability to detect lung cancer earlier has changed
the face of things so we can diagnose it and we can cure it. We use
robot-assisted surgical procedures and the new chemotherapy is just
pills. That’s been a huge change.
The TV seems to be filled with ads about treating COPD.
What is this?
Chronic obstructive pulmonary disease is the way we think of the
obstruction of the lungs. The airways are diseased in some way,
whether from severe emphysema to chronic bronchitis to asthma.
Most COPD is really a smoking-related disease.
Should all smokers be checked for this?
People who have smoked more than a little should have a pulmonary
function test. Patients who begin to have early signs of disease are at
higher risk for lung cancer. Early detection is key for both alerting the
doctor and the patient. As people get older, they will compensate in
subtle ways. The changes are gradual so you don’t notice. Lung cancer
is such a bad disease. If it’s left untreated you begin to get symptoms
and it spreads. It’s not a subtle disease.

What is an interventional
cardiologist?
The majority of cardiologists are
noninvasive cardiologists. But
interventional cardiologists are
a subspecialty; we do procedures
designed to treat and hopefully
stabilize heart conditions.

is becoming more common.
Is this area growing?
Is heart failure inevitable?
Yes and no. As medication
advances, and smoking cessation

long-term, noninvasive

Any particular reasons why?
Number one: They’re born with
good luck. Number two: They
don’t smoke. The most common
cause of death in smokers is not
lung cancer; it is heart attack
and stroke. Number three:
They’re not obese. Obesity is
associated with many of the risk
factors that lead to heart disease.
How does a person know they need
this intervention?
There are lots of different
scenarios. There are [the] patients

who think they are well and [then]
they wake up at two in the morning
with severe discomfort in their chest,
like an elephant is sitting on their
chest, and their spouse convinces
them to call 911. They have an EKG in
the ambulance, which is transmitted to
the emergency room physician. We’re
seeing what they look like on the EKG
and we typically know exactly what
they’re going to need. We take them
right to the catheter lab and advance
a catheter up through the arterial
system to the heart, where we insert
X-ray fluid into the arteries. Then we
thread a wire through the blockages
and try to reestablish blood flow.
We try to do it all within one hour;
we call it the golden hour.
I collaborate with fifteen
interventionalists. We might do
about 1,500 interventions a year.
abuse in our adolescents and that seems
wrong on some very fundamental levels.
There seems to be so much more in the media
about bullying. Is that a big problem?
Bullying is becoming more and more
prevalent. Let’s be honest, putting up a sign
in the school cafeteria is not enough to stop
bullying. Efforts toward kindness and social
integration are imperative. Social networking
can be an extraordinarily divisive force.
You’re not ‘social’ when you’re on a device.
Our technology, instead of bringing us
together, is invariably pulling us apart.
Is this happening with adults too?
The news has us thinking differently
about cholesterol numbers...
It’s a complicated story. People got
the message to know [their] number;
lower [it] and you lower your risk.
Once you have identified someone as
‘at risk,’ it’s pretty clear that taking
statin drugs lowers that risk. So we’ll
have to see what happens with the
numbers, but I think that if you identify
someone whom you think is a risk,
you should give them a real dose of
statin drugs.
Another item in the news is the potential
for increased risk of heart disease in men
who inject themselves with testosterone.
This is a hard thing to pinpoint. Just
because some guy uses testosterone
and has a heart attack doesn’t mean
the testosterone caused the heart
attack. However—I’m just one guy and
this is one guy’s opinion—thinking
that [testosterone is] the fountain
of youth? It’s not. It’s a huge
industry and it’s not being studied in
a rigorous way.

What are you seeing more of today?
Depression is the big topic these days,
especially adolescent depression. It almost
seems like an epidemic. We are treating it in
all its permutations. A child might say, ‘I’m
suicidal,’ and the parent might think, ‘Oh,
they’re being so dramatic.’ Sometimes there’s
a dismissive quality to that but comments
like this are not to be taken lightly.
Substance abuse among teens is also huge
around here, and that includes alcohol and
opiate abuse. To some extent there’s a
tolerance to alcohol abuse and marijuana
abuse. But in reality, studies have shown
there really is cognitive impairment at that
age. We are much more tolerant of alcohol
Yes. My wife and I were out to dinner and
there was a table nearby with five people
sitting there. For twenty minutes nobody
talked to each other. They were all on
their devices!
One of the most important tasks of
being an adolescent is the emulation of
adult behavior. In most Western cultures,
adolescents spend a lot of time with adults.
But here our adolescents spend a lot of
time with other adolescents, and it’s over
electronic devices. There are disinhibitory
effects of electronic media. [Users] will say
and do things they would not say to your
face. They allow people to be harsher.
Then, when they do interact face-to-face,
they actually do become harsher. We are
fragile creatures and we do need a little
more social integration. The more social
isolation people are in, the more they are
going to act idiosyncratically to the world.
Speaking of social isolation, we seem to be hearing
more about Asperger’s syndrome than in the past…
There’s been much in the media about
Asperger’s, and there’s a change in the works
in the DSM (The Diagnostic and Statistical
Manual of Mental Disorders) regarding the
way it will be designated. The Asperger’s
diagnosis is going away. It will be considered
on the mild end of the autism spectrum. How
much autism is enough to warrant a disorder
and how much is it just the quirky kid in the
corner? We all have quirks toward depression
and anxiety. The focus is on functionality.

What’s new in oncology?
The newest thing in oncology is
personalized medicine, an attempt to
tailor therapy to the molecular changes
of someone’s individual tumor. There
are a variety of techniques, including
sequencing the genes of a tumor and
understanding that tumor as more
than the sum of its genetic types.
We are able to identify driver
mutations in similar cancer types.
In chronic myelogenous leukemia,
for example, there is one mutation
that is responsible for that leukemia
[that] we can totally shut down with
a pill. [Targeted therapies] is probably
the holy grail of oncology.
Radiation treatment seems to be more
targeted as well these days…
Targeted radiation is exquisitely, finely
targeted to a tumor site. This has been
a huge advance in radiation therapy
for some indications. What has made
this possible is exquisitely accurate
imaging for what is sometimes a
moving target.
What types of cancers are we seeing
more?
Pancreatic cancer is on the rise.
So is cancer at the junction of the
esophagus and stomach, related to
chronic reflux. Liver cancer as well.
What’s your specialty?
I’m a general oncologist, but my
practice has evolved to where I see 40
to 50 percent breast cancer [patients].
It’s probably the most common cancer
we see in the community. There has
been spectacular progress in all
aspects of breast cancer prevention
and screening and dramatic
improvements in preventing
recurrence. Breast cancer was one
of the first kind of cancers using
targeted therapies. We already cure
three-quarters of the people who
have it.
You are in “community oncology?”
Historically cancer care has been
administered in a cancer setting.
In an academic model interests may
lie elsewhere, as in clinical trials, for
example. The strength of community
oncology is taking care of people and
learning to practice in an extraordinarily
resource-rich environment. We have
access to novel therapeutics, but
where we excel as an office is taking
care of people. In our group, the house
call is not dead. I get to help people
through a really difficult time.
Being an oncologist makes you a more
serious person. That’s both good and
bad. It has an emotional toll. It does
make you grateful for the gifts you have.
Any common misconceptions about
cancer?
The thing I spend the most time
talking with people about is you
don’t have to do anything to get
cancer. Cancer is an illness.
Anything else you’d like to share?
I wish people would learn how to talk
about cancer. When you have cancer,
the cancer should be the hard part,
not somebody else’s reaction. Being
direct is better than being obtuse
and circumspect. Asking somebody
is better than staring.
People who don’t have cancer don’t know
what to say. What should they say?
Maybe you can say, ‘What kind of
cancer’s ass are you kicking today?’
For someone with cancer, every day
is a butt-kicking day.

A young person should be a Type 1 but she
was presenting as a Type 2. We sent her
for a test and she had just one mutation
in her glucose sensor. One amino acid was
switched for another in one spot and that
caused her diabetes.
What’s hot in endocrinology these days?
Probably the hottest topic in my practice is
the genetic basis of endocrine diseases. I’m
sending more people for genetic screenings or
tests. One of my patients looked like a Type 2
diabetes but she was in her early twenties.

The most well-known is Type 1. It used to
be called juvenile diabetes. Your own body
thinks of your pancreas as a foreign body
and attacks it, like rejection. Because your
body does not produce enough insulin,
you need to be treated with it.
Then there’s Type 2. You may have all
different levels of insulin production but
your body doesn’t ‘hear’ the insulin as well.
There are all sorts of medications that can
do all different things to help. We’re seeing a
lot more of Type 2. Even kids are getting this
one, probably because of the rise of obesity.

They both are common disorders and there

Women with thyroid disease outnumber
men 7-1. Women have more autoimmune
diseases as well. Women have a more
complex autoimmune system. Think about
it: You have to be able to tolerate a whole
other body in you without rejecting it.
Do you mind when patients research a problem
on the Internet before they come to you?
Oh no, not at all. Patients in our geographic
area are medically very sophisticated.
They come in with spreadsheets even!
Endocrinologists like data in general.
Most of it is the patient’s history. They
always said in medical school that if you
listen to the patient, [he or she will] tell you
the diagnosis. We look at your history, your
physical [exam] and then your data. Once
the information is assimilated then [we]
can talk. If you want to be a really good
doctor, you have to use all three together.

DR. JENKINS IS A BOARD-CERTIFIED,
fellowship-trained neurosurgeon based
at the Mount Sinai Hospital in New
York City. A native of the Northeast,
Dr. Jenkins grew up in the Philadelphia
area and attended the University of
Pennsylvania where he earned a
bachelor’s degree in biochemistry and his
medical degree. His post-doctoral training
included a surgical internship at New
York University Medical Center/Bellevue
Hospital, and a neurosurgical residency at
the Mount Sinai Medical Center, followed
by a complex spinal surgery fellowship
at the Brigham and Women’s Hospital
in Boston. Due to his close relationship
with the Department of Orthopedic
Surgery at the Mount Sinai Medical
Center, he has clinical appointments
as associate professor in its Department
of Neurosurgery and the Department of
Orthopedic Surgery.
An innovator in the field of minimally
invasive spine surgery, Dr. Jenkins’s
clinical interests span the treatment
of spinal disorders and injuries as
well as peripheral nerve surgery and
general neurosurgery. Dr. Jenkins’s
clinical research focuses on advances
in minimally invasive spine procedures,
intraoperative imaging and image-guided
surgery, and improvement of fusion rate.
His basic science research focuses on
treatments for spinal cord injury—acute
and delayed intervention—including
surgical techniques to restore lost
function below the level of injury.
Dr. Jenkins utilizes innovative
surgical approaches to treat the entire
spectrum of spinal disorders and
peripheral nerve diseases. His approach
to treatment is to partner with the
patient and identify specific goals for
treatment. His experience with the latest
imaging and surgical technology and
minimally invasive techniques allow him
to offer highly individualized treatment
plans with a focus on improving quality
