Benefit Eligibility
You are eligible for benefits if you are a regular, part-time Team Member
You may also enroll your legal spouse.
Dependents under the age of 26 or dependents over the
age of 26 with a disability are also eligible for benefits.
New Employees: If you enroll in benefits
within 30 days of your hire date, your benefits
will be effective on the 1st of the month
following your date of hire.

Qualifying Life Event
If you experience a QLE, you may make corresponding changes to
your benefit coverage. Changes must be reported to Ruby Tuesday
Benefits Administration within 30 days of your QLE date. Examples of QLEs
include, but are not limited to, marriage, divorce, birth, adoption, change in
benefits eligibility, and death. Do this at www.awd.benselect.com/enroll.

Dental and Vision Plans
Good dental health is a critical component of your overall health. Your dental plan is offered by Guardian, provided by
Allstate, and provides benefits for preventive, basic and major services. Orthodontia is available for children under age
19. The plan allows you to go to any provider you choose, but benefits are paid at a higher level when you use a Guardian
network dentist. To find a Guardian network dentist in your area, call 800-541-7846.

If you wear glasses or contacts, you know vision expenses can add up. At Ruby Tuesday, vision benefits are offered by EyeMed Vision,
provided through Allstate Benefits. Even if you don’t wear contacts or glasses, annual eye exams are recommended. To find an innetwork optometrist or ophthalmologist near you, call 866-679-1358.

Dependent Care FSA
What is a Dependent Care FSA?
A Dependent Care Flexible Spending Account (FSA) allows you to set aside pre-tax funds to pay for dependent care expenses.
Since these contributions are not subject to federal, state or payroll tax withholding, you receive more value for the dollars you
spend on dependent care.
Each year you must elect the amount you want to contribute to your FSA. Your contribution will be deducted from your
paychecks in equal installments throughout the year and deposited into your account. If you are unable to estimate your
dependent care costs accurately, it is better to be conservative and underestimate rather than overestimate your expenses.
What can it be used for?

spouse or your dependent for the care of your

• Services in a day care center
• Services of a housekeeper whose duties
include providing care for a qualified
dependent
• FSA funds are NOT yours to keep.
• By law, whatever funds you do not use by the

60% of income up to $5,000 per month
Employee, Spouse, and Child Coverage
Allstate’s individual short term disability insurance replaces
a portion of your income if you are unable to work due to a
covered injury or illness. This coverage can pay a monthly
benefit to provide some income during a time of need.
Common reasons people use this coverage include
injuries, a covered pregnancy and digestive problems —
such as gall bladder surgery.
Life is unpredictable, but we all know that life has an
expiration date. If something happens and you are gone, will
your family be financially prepared to cover the expenses
left behind? With Term Life insurance from Allstate Benefits,
a cash benefit is paid to your beneficiary that can be used for
any financial need that arises. Plus, an Accidental Death and
Dismemberment benefit can be added to provide additional
coverage.
