With the support of the Ministry of Health and Welfare and city of Daegu, as the “Support and funding project for local leaders in outstanding medical technology”, the center moved to the current location in 2011. The follicular unit hair transplantation technique, developed by Professor Jung-Chul Kim, makes it possible to transplant 1,000-1,500 hairs per hour. With this technique, the survival rate of transplanted hairs exceeds 92%, thus guaranteeing the most natural looking result. Some 150 Korean and 30 foreign doctors have visited the Center to learn this technique. The Center has also trained over 100 technicians and surgical assistants in hair follicle preparation, and they are employed at hair transplantation clinics throughout the country. Technicians receive ongoing education about new technology and current information at the Center. The Center and its nationwide network of hospitals, with Center-trained doctors, provide high-quality service for hair-loss patients. Kim Jung Chul M.D., Ph.D. Kyungpook National University Hospital Hair Transplantation Center 14 _ 15 Professor Jung Chul Kim Development of the first follicular unit hair transplantation technique Professor Jung-Chul Kim, the diredtor of KNUH Hair Transplantation Center, developed the first follicular unit hair transplantation technique in 1992. Among many recognitions, Dr. Kim has received the International Society of Hair Restoration Surgery (ISHRS) Platinum Follicle Award in 1994, the World Hair Research Society Golden Award in 1995. Prof. Kim also served as an international advisory board member and Executive Director of ISHRS between 2001 and 2006. He is a founding member and the first president of Korean Society of Hair Restoration Surgery. Dr. Kim contributed to several chapters in the 4th and the 5th edition of Hair Transplantation (W.P. Unger, R. Shapiro eds). He also published an introductory book for baldness patients entitled Dr. JC Kim’s Hair Story. Dr. Kim has published numerous papers on hair transplantation and research and continues to dedicate his time on research and practice. Initially, punch grafts had been used with using a 4mm punch to extract follicles from the back of the head and placed them on the grafting area using the same size punch. However, the results of this method were unnatural looking because the hair grew like a doll’s hair. In order to improve the unnatural appearance of the punch- and the mini-grafting, Prof. Jung-Chul Kim at Kyungpook National University Hospital developed a new hair transplantation method in 1992 using the world’s first follicular unit transplantation method. Follicular unit refers to the hair unit on the scalp. The hairs on the scalp grow in follicular units. Each unit comprises 1-3 hairs growing from a single opening in the skin. For East Asians, 46% of hairs are single-hair follicular units and the rest are in 2-3 hair follicular units. For most Caucasians, however, the hair generally grows in 2-3 hair follicular units, with only 5% single-hair follicular units. Follicular unit transplantation provides the best results because it moves hairs as follicular units those are in a normal distribution pattern on the scalp, there by preserving the original hair appearance. This method is ideal, especially for Asians, who typically have thick, straight, black hair. Using the KNU implanter, 1,500-2,000 hairs can be transplanted in an hour with 92% hair survival rate. Thus, the KNU implanter allows faster, more efficient transplantation and better survival rates than other methods. There are three different sizes of the KNU implanter: S, M, and L. S size is used for a single hair, M for 2-hair units, and L for 3-hair units. The KNU implanter has been patented in Korea, U.S., Japan, and China. First, why do male sex hormones (androgens) cause the beard and moustache to grow but the hair on the top of the head to fall out? Second, why, in even severe baldness, does the hair on the sides and back of the head continue to grow while the hair, on the top of the head falls out? There are clear differences among the hairs in the beard and those on the top and in the back of the head. The differences result from gene expression. If those genes can be identified, the mechanism of androgenic alopecia can be explained, and new treatment modalities could be found. We have worked to find the genes for baldness since early 1990s. At that time, we collaborated with the world renowned geneticist Dr. Victor A. McKusick, Johns Hopkins Medical School. After a month, we received this response, “There’s good news and bad news. The bad news is that there is not one baldness gene rather, many genes are involved. It will be difficult to find all of them even in a lifetime of research. The good news is that because they would be difficult for anyone to find, you can enjoy researching this subject for your whole life.” Many colleagues have asked us why we study hair rather than focusing on other serious, life-threatening diseases, such as cancer or cardiac disorders. While hair loss is not a life-threatening issue, it is a quality-of-life issue. We have seen that efficient, successful treatment based on solid research provides positive patient outcomes. Kyungpook National University Hospital Hair Transplantation Center 28 _ 29 Hair-specific Genebank and Gene Chip Today, we can analyze tens of thousands genes in a day; however, in the early 1990s, it took several years. Up until the late 1990s, the gene bank at the KNUH Hair Center was the largest in Korea. As many as 8,300 genes from human hair follicle stem cells were analyzed and submitted to the GenBank database at the U.S. National Institutes of Health. This unique collection of genes can be a valuable tool for hair research. By spotting these genes on the slide glass, gene chips were developed. At the same time, these hormones inhibit the growth of hair in frontal and vertex areas in genetically predisposed individuals. It is called ‘the paradox of androgen hormones’. In order to study this phenomenon, hair follicle stem cells from the beard, the front and back of the head were cultivated, and their gene expression differences were analyzed. Androgen hormone-regulated genes were screened using the gene chip, which we developed, and their functions in hair growth are studied to determine their part in causing male-pattern baldness. We discovered that the DKK-1 gene may cause male-pattern baldness. DKK-1 expression is increased by androgen hormones, thereby causing the death of hair follicle cells and ultimately inhibiting hair growth. Substances, including TG-H7, that inhibit the expression of DKK-1 are now undergoing clinical testing at the KNUH Hair Center and at cooperating hospitals nationwide. Identifying the mechanism of male-pattern baldness and developing drugs to treat it will create a worldwide, billion-dollar industry. For animals, it has been possible since the 1970s but is yet to be achieved in humans. However, the dream will come true.’ For severe hair loss patients, the number of hairs at the donor site is very limited, thus new methods are needed. Hair cloning is a method of regenerating hairs by culturing the patient’s own hair follicle stem cells. If successful, this technique could provide sufficient hairs for transplantation. However, when hair follicle stem cells are cultured, they gradually lose the capacity to regenerate hair. We are currently searching for ways to overcome this problem. In boys, beard and other body hair becomes thicker during puberty because of male hormones. New hair is not created rather hormones simply change the hair’s texture. Individual hairs fall out after growing for 3-5 years, and in that spot, new hair grows after 3 months. Hair, therefore, continually repeats the cycle of growth - regression rest. Human have 70,000-100,000 hairs, on average. About 70 hairs fall out each day, and the same number grow in, maintaining the approximate number of hairs. On average, hairs grow 0.35mm/day, 1cm/month, 12cm/year, and, theoretically, 40 to 60cm in 3-4 years of one hair cycle. However, in case of balding, the hair cycles become shorter as balding progresses, resulting in shorter hair and more hair loss. In other words, hairs in the front of the head where balding takes place become shorter and fall out more compared to the hairs on the back of the head where balding does not take place. While the genes that cause one to be bald have yet to be discovered, it is known that several genes are involved. The probability of passing those genes to an offspring is 50%. In addition to genetic makeup, other factors contribute to hair loss. These include male hormones, stress, diet, and aging. Male hormones give rise to balding Although balding is caused by male hormones, it is not triggered by the general male hormones of testosterone, but rather by DHT, a substance where testosterone is metabolized by a specific enzyme, 5α-reductase. Propecia, currently used for balding, inhibits this enzyme. Medical Treatment of male-pattern baldness As drug therapy is especially effective at initial stages of hair loss, early diagnosis is all the more important. Hair loss can be minimized with early diagnosis and adequate drug treatment, and the time to complete baldness may also be increased. Because drugs for treating hair loss must be used for a long period, their safety is extremely important. Currently, two drugs have been approved by the U.S. Food and Drug Administration. They are minoxidil (Rogaine) and finasteride (Propecia). Rogaine (minoxidil) Rogaine is a hair growth stimulator originally developed as a treatment for hypertension, but had the side effect of growing hair. While Rogaine’s hair growth mechanism is as yet unknown, it is most effective on the crown. The drug is applied daily and takes about 6 months for its effects to be seen. The recommended formulation for men is 5% and for women is 2~3%, to avoid thickening of the fine, soft hair on face. Minoxidil (Rogaine) is also marketed as Minoxyl, Moxidyl, and Scalpmed. Propecia (finasteride) Propecia inhibits the production of DHT, the cause of baldness and enlarged prostate, by restraining the function of the necessary enzyme. Finasteride (Propecia) was originally used to treat enlarged prostate, and its effect on hair growth was observed. Like Rogaine, Propecia takes at least 6 and up to 12 months to produce the effects. 38 _ 39 Hair Transplantation Surgery Is the problem with the field or with the tree? Patients considering hair transplantation often ask if the transplanted hair might also fall out. In male-pattern baldness, the problem is not with the skin of the head(the field), rather with the hairitself that grows there, the tree. So, good quality trees, hair from the back of the head, are transplanted into the field where it may grow with its growth qualities intact. Types of hair transplantation surgery Hair transplantation began with the punch grafts and has been great improvement throughout the world in last decade. From the early punch grafts which had results of unnatural appearance and later going through the mini-grafts, now the standard method of hair transplantation is the follicular unit transplantation. More recently, FUE (Follicular Unit Extraction) has been developed and is being attempted. Total hair numbers and follicular unit distribution is much different according to the ethnic groups. East Asians, 46% of hairs are singlehair follicular units and the rest are in 2-3 hair follicular units. For Caucasians, however, the hair generally grows in 2-3 hair follicular units with only 5% single-hair follicular units. Follicular unit transplantation is the method of transferring the hair follicle unit, basic unit present in scalp, where it appears the best and obtains the most natural results. 1st session Safe donor site 2nd session Donor harvesting area FUE method Unlike the current standardized method of follicular unit transplantation, involving harvesting a long strip of donor hair through incision, FUE method is extracting individual follicular unit with the use of a punch about 1mm in size. Currently, this method is being promoted with exaggeration as the best method on the internet. However, there are many disadvantages for FUE, such as, the procedure takes too much time, the survival rate may low due to possible damage of hair root during extraction, and diffuse white spot scars over the donor area. Attempts are being made to develop automatic machine or powered punch, but FUE is not yet considered as a standard method. Follicular unit extraction (FUE) How will the hair line be determined? The hair line must be carefully determined in terms of the shape of the face and the desired appearance. The face is divided into thirds so the distance between the chin and the tip of the nose, the tip of the nose and the eyebrows, and the eyebrows and the hair line are roughly the same. As one ages, however, the natural hair line is a bit higher. Therefore it is best to move the original hair line 1~2 cm higher. Younger patients often want the hair line to be the same as before hair loss however, an excessively low hairline may be a mistake. Arranging the transplanted hair While individuals have between 20,000 and 30,000 hairs on the top of the head before hair loss, only 3,000~4,000 hair scan be transplanted at one time. In most cases, then, it is difficult to achieve the same pre-hair loss density by one hair transplantation procedure. In order to achieve a satisfactory result, we do not distribute the transplanted hairs evenly across the top of the head. Rather, we transplant more hair units along the front hairline and fewer as we move back toward the crown. Density gradient hair arrangement 40 _ 41 Female-Pattern Hair Loss Do women experience balding? Balding is frequently seen in women. In the U.S., women account for 40% of the hair-loss population. Unlike men, whose hair loss often begins at the front forehead, women typically experience thinning of the hair on the crown. However, the genetic aspect or mechanism of balding in women is similar to that seen in men. Further, women have a small amount of male hormone, and hair loss occurs in those women who have the balding genes. Grade l Grade ll Grade lll Can women undergo hair transplantation? Unlike male pattern baldness, women can lose hair from the back of the head. If hair at the donor site maintains sufficient density, transplantation can be done. The transplants can be placed where they are needed, often at the top of the head and the area where the hair parts naturally. After hair transplantation, it is helpful to use 2~3% of Rogaine. Propecia is not effective in women. Transplanting hair on other parts of the body For cosmetic purposes, hair transplant is possible to restore eyebrows, eyelashes, and pubic hair. This is done using a single-hair unit graft of hairs taken from the back of the head. The transplanted hair continues to grow by one centimeter each month, thus it must be cut every two to three months. Regarding before and after surgery Before surgery 1. Abstain from drinking alcohol 3~5 days before operation. 2. Stop taking aspirin or vitamin E (tocopherol) drugs 1 week before surgery. 3. Stop using Rogaine 2 weeks before surgery. 4. Do not cut your hair too short. 5. Do not color your hair within 1 week of surgery. 6. Tell your physician in advance what drugs you take or have drug allergies On the day of surgery 1. Wash your hair well but do not apply any hair products. 2. Eat a regular breakfast but do not overeat. 3. Bring a loose cap (an alpine cap or a cotton hat). 4. Wear a comfortable shirt that opens in the front. After surgery 1. It is best to a rest the day of the surgery and the next day, if possible. 2. When you visit the hospital on the day after surgery, the transplantation area may be treated and you may shampoo. 3. You may wash your hair at home on the fourth day after surgery.
