Home Health Aides to assist with activities of daily living, as required
Lifetime Pharmacy - Infusion Services in the Home
Working together to provide IV services benefits both patients and providers. For patients, in most cases there is no
additional infusion pharmacy co-pay; 24/7 delivery; and less phone calls, scheduling and clinicians in the home.
Providers have reduced phone calls and paperwork.

Certified wound, ostomy and continence nurse consults
on cases by visit and/or digital photo review
Chronic Wound Management using
evidence-based guidelines
Evaluation and referral for specialty beds and

Vacuum Assisted Wound Closure (VAC)
Ostomy, fistula care and drainage management
Diabetes Education/Care
Certified Diabetes Nurse Educators and
specially-trained team nurses instruct newly
diagnosed patients in blood glucose monitoring,
meal planning, exercise and medications

Advances in medical technology for the home can increase patients’ ability to monitor their own condition, reduce
rehospitalizations, and improve collaboration between patients, physicians and Lifetime Care nurses.
• Daily Home Monitoring of vital signs to identify changes in condition; reporting vital signs trends to physicians
• Video Visits provide face-to-face contact with patients and physicians for high risk patients
• Zoe Monitor measures fluid status on patients with CHF
• State-of-the-Art Digital Cameras assist in wound care treatment
