Along with great health benefits, Independent Health has partnered with a variety of organizations
throughout the community to provide you with a wide range of valuable wellness discounts. This
unique partnership lets you enjoy special savings on everything from acupuncture and massage
therapy to dental discounts.

away. An informative, online health resource called Independent Me® is also available, providing you
with the freedom to find information and get the latest updates in health care.
Because we’re constantly working to expand the wellness discounts and benefits available to you,
and since programs are subject to change, please visit our website at independenthealth.com

Independent Health members can receive
$100 off a monaural conventional hearing

variety of circuits and can be appropriate
for many types of hearing loss.
Members can also receive $100 off the

and off the fitting of binaural digital
hearing aids. Digital hearing aids offer
greater sound processing power and
can make millions of sound procession

Smoking Cessation Programs
We know that quitting smoking is one of the best things you can do for your health, but we also realize
it’s one of the most difficult habits to break. The best place to start is the New York State Smokers’
Quitline: 1-866 - NY- QUITS (1-866-697-8487).
Why Call the New York
State Smokers’ Quitline?
The New York State (NYS) Smokers’
Quitline is a free and confidential
program that provides stop-smoking
services to New York residents who
want to stop smoking or using other
forms of tobacco. Some of the services
the Quitline offers include:
• Individualized telephone counseling
in English or Spanish.
• Free nicotine replacement therapy
(NRT) starter kit (if eligible per NYS
Smokers’ Quitline).
• Referrals to local stop-smoking
programs (classes).
• Web-based services with interactive
features (nysmokefree.com).
• Self-help quit kit – “Break Loose” – a
comprehensive pamphlet that allows
you to quit smoking at your own pace.
What Does Independent
Health Cover?
If you want to continue therapy beyond
the NRT starter kit from the NYS
Smokers’ Quitline, most Independent
Health plans provide an enhanced stopsmoking program you can participate
in by phone with an Independent Health
support representative.
How Does the
Program Work?
• After an assessment with a Quitline
Specialist, eligible members are sent
a free starter supply of NRT.
• Approximately two weeks later, the
phone coach will call. If the member
and the coach determine that the
NRT is working, and the member
enrolls in the telephonic support
program, an additional six weeks of
NRT will be mailed.
• Members may be subject to
copayments, deductibles and
out-of-network expenses as applicable.

Options and Therapy
SMOKING CESSATION PROGRAMS
For those members who prefer group
support, a smoking cessation program
is for you. These community-based
programs provide the tools and support
you need to help you become and stay
smoke free. No referral is necessary;
however, most Independent Health plans
require a copayment, payable directly
to the provider in advance. You may be
eligible, for 100 percent coverage if you
attend all the classes. Some plans are
subject to deductible and coinsurance
for each class.
SMOKING CESSATION
DRUG THERAPY
Independent Health offers reimbursement
on any brand of the following:
• Gum
• Lozenges
• Patches
• Nasal sprays
• Inhalers
• Zyban®
• Chantix
To be eligible for reimbursement you
must pay for the drug(s) in full at the
time of purchase, and either call the
NYS Smokers’ Quitline, or complete a
smoking cessation program through
one of our participating smoking
cessation providers. Coverage is for up
to a three-month supply per calendar
year, and you will be reimbursed up to
a maximum amount.
How to Get Reimbursed
You will be reimbursed for your drug
therapy products when you submit
proof that you have either called the
NYS Smokers’ Quitline or attended and
completed a smoking cessation program.
Proof of your attendance/completion
will be provided to you by your smoking
cessation course instructor or NYS
Smokers’ Quitline, via a certificate of
completion. When calling the Quitline,
please tell them that you are an
Independent Health member, and
that you would like a certificate of
completion.
Please submit the certificate along with
your receipt(s) and the name, strength,
and package size of the drug to
Independent Health. To expedite your
payment, you may wish to submit the
empty package as well, because it
indicates all the necessary information to
process your claim. NOTE: You will be
reimbursed up to the maximum allowed
amount, which is 95 percent of the
average wholesale price (AWP) of
the drug, which is subject to change
without notice.
QUESTIONS OR CONCERNS?
As you may already know, there can be
common, yet brief withdrawal symptoms
after you quit smoking. Whatever
method you choose to help you quit,
there are numerous ways to lessen and
eliminate these unpleasant symptoms
by adjusting your lifestyle.
You can greatly decrease the symptoms
of withdrawal, maintain a healthy weight,
improve sleep, and increase your daily
energy by incorporating exercise,
good nutrition, and stress management
into your lifestyle.
If you have questions

or any of Independent
Health’s Wellness

Tips for Healthy Living
At Independent Health, we understand that the choices you make about the way you live are important to your
health. Here are some choices you can make to help yourself stay healthy.
Eating for Good Health
Food provides you with the energy
and substances you need for growth
and survival. The choices you make
throughout the day can make a difference
in your overall health, and help you
control your weight.
Choosing Wisely
• Whole grain breads, cereals, rice and
pasta have more fiber, vitamins and
minerals than white processed varieties.
Choosing whole wheat and oat grain
products will help you stay full longer
and help you achieve the 25-35 grams
of fiber you need a day.
• Choose lean beef like sirloin or round.
Avoid prime and choice cuts; they have
more saturated fat. Remove the skin
from poultry before eating; the skin
has a lot of fat and calories.
• When buying fruits and vegetables,
choose the most colorful. They are
usually highest in vitamins and minerals.
Buy your produce fresh or frozen.
Canned vegetables and soups are
often high in sodium.
• For dessert, have fruit or frozen yogurt
instead of cake and cookies. Snack on
a handful of nuts or a little dried fruit
instead of chips and candy.
Exercise for a Healthier Life
The No. 1 cause of obesity in America
today is due to physical inactivity. More
than 60 percent of American adults do
not get enough physical activity to provide
health benefits. Regular activity can reduce
your chances of future health problems, as
well as improve overall health.
18
Here are just a few of the benefits of
staying active:
• Lowers resting heart rate
• Strengthens muscles
• Increases bone density
• Helps control blood pressure
• Controls blood sugar
• Increases alertness, productivity
and helps to manage stress
Get Started
• Set aside a scheduled time of the day
to exercise. It will become a natural part
of your routine.
• Choose activities that you enjoy.
Walking is an excellent way to increase
your activity level. If you enjoy walking,
you’re more likely to stick with it.
• If you haven’t had any physical activity
recently, start slowly. Increase your
frequency and duration with time.
Have a Routine
• Warm up for 5-10 minutes to help get
your blood flowing and your heart rate
elevated safely.
• Exercise 3-5 days a week for at least 30
minutes or more.
• Engage in all elements of physical
fitness, including aerobic fitness,
muscular strength and flexibility.
Types of Exercise
• Aerobic fitness includes activities like
walking at a fast pace, jogging, swimming
and cycling. These activities strengthen
your heart. Engaging in regular aerobic
fitness burns calories, which aids in
weight loss.
• Muscular strength – your muscles
support your body and allow you to
pull, push or lift objects. Building
muscular strength can help improve
posture, increase your energy level
and lower your risk of injury. Muscle is
metabolically active; the more muscle
mass you have, the more calories you’ll
burn – even in a resting position.
• Flexibility is the ability to move your
joints in a range of motion without pain.
Stretching will allow greater freedom
of movement which in turn gives you
better coordination and balance.
Managing Stress
Stress is a natural and unavoidable part
of life. It is your body’s response to
any change that you experience, good
or bad, real or imagined.
When you have no positive outlet to
release stress, your body can begin to
display physical, emotional and mental
symptoms. Fatigue, headache, muscle
aches, upset stomach, colds, anxiety,
depression and difficulty concentrating
are some of the symptoms of stress.
There are ways to keep stress under
control, including:
• Exercise – Regular exercise is a great
way to relieve anxiety and depression.
• Eating Healthy – Avoid caffeine and
foods that are high in sugar.
• Communication – Share your
thoughts, feelings and worries with
someone close to you. Another
person can offer support, comfort
or a different point of view.
• Choosing a positive mental attitude –
Negative thoughts can bring on stress.
View changes as opportunities for
learning and growth. Surround yourself
with positive people.
• Managing your time – Make lists.
Prioritize and delegate. If you have too
much to do, learn to say “no.”
• Getting adequate sleep – You’ll be
able to cope with stressful situations
better when you are well rested.
• Practicing relaxation exercises –
Deep breathing and meditation relieve
anxiety and muscle tension.
