If you win, you can choose a:
Trip to Disneyland: Three nights and four days, including hotel, airfare, food and
spending allowance for up to four people
Cruise: Up to four nights, including airfare, cruise package, food and spending
allowance for two people
Home theater: Home theater projector, surround sound speakers and lighting
You must be active on November 30 to be eligible.
You’re eligible if you’re a full-time or part-time employee, even if you don’t participate in
a TeleTech medical plan. (But some activities are only available if you’re enrolled in our
UnitedHealthcare medical plan.) Employees with a Vice President title or higher are not
eligible to win the grand prize.

You don’t have to enroll in a TeleTech medical plan to participate in the wellness program. But if you do, you earn tickets for enrolling and making smart use of
your preventive care benefits.

Prevent health conditions — or catch them early
when they are easiest to treat — by getting an
annual exam. You’ll receive two raffle tickets and
$100 in your Health Reimbursement Account. You’ll
get an additional $100 if your spouse is enrolled in
our medical plan and gets an annual exam, too.

Preventive care is free under our medical plan
and includes (but is not limited to): physicals, PSA
testing, mammograms, well-woman checkups and
vaccinations. If you’d like to get a preventive exam
but don’t have a primary care doctor, find one near
you at myuhc.com or call Health Advocate at
866.695.8622.

Not all surprises are good surprises — especially when it comes to your health. It pays to know where you stand.
Complete the Health
Assessment

Complete a confidential online questionnaire to get a
personalized summary of your health.
•Log in to myuhc.com (or click “Register Now” if you’re
new to the site).
•Click the “Rally Health Survey” option on the
right-hand side.
•Follow the prompts to create a Rally account and
complete the Health Survey once you’re logged in.
Open an ASK NOW ticket if you have any problems.

Visit a StayHealthy/Higi kiosk, either onsite or at a location
near you, to:
•Check your weight
•Get a blood pressure reading in a healthy range
(systolic of 120 or less and diastolic of 80 or less)
•Complete a body composition analysis
in a healthy range (Men: 2 to 24.99 percent.
Women: 10 to 31.99 percent)
If your site has a handheld unit instead of a kiosk, you only
need to complete the body composition analysis. Your
user name is your Oracle ID and your password is the last
four numbers of your Social Security number. If you are
remote or your location does not have the equipment, find
a kiosk near you. After you use the kiosk, log in to your
dashboard to review your results.
Can’t find a kiosk within 30 miles? Open an ASK NOW
ticket to request a form that your health care provider can
complete about your health. Attach the completed form to
your open ticket to earn your raffle tickets.

The wellness raffle is just one part of our wellness program.

Visit with a doctor anytime, anywhere — by phone or secure video — for just $10 (unavailable in Arkansas). The $10 applies toward your out-of-pocket
maximum if you are enrolled in TeleTech’s medical plan.
MDLIVE doctors specialize in primary care, pediatrics, emergency medicine, family medicine and behavioral health. See one whenever you need medical
care, including when your primary care physician isn’t available (after hours, weekends or holidays, for example), you’re traveling and need medical care or
you need prescription refills. They can help you with these conditions and more.
