Participant agrees that he/she is physically able to proceed with the exercises and instruction and other facilities
provided by Curranʼs Karate. Participant agrees that he/she has disclosed all medical conditions in writing that he/
she may have had prior to commencing training with Curranʼs Karate. Participant agrees that Curranʼs Karate shall
not be liable for any damages arising from personal injuries sustained while attending Curranʼs Karate and using
facilities and equipment therein and receiving instruction therein. Participant recognizes that there are inherent
physical dangers in martial arts training and agrees to participate at own risk. Participant agrees that Curranʼs Karate shall not be liable for the loss or theft of, or damage to, the personal property of the Participant. The terms
outlined within this waiver remain in effect for the duration of Participant's training with Curranʼs Karate.
