THE HEALTH CARE MARKETPLACE IS RAPIDLY EVOLVING
and today more than ever, providers are pressed continually to improve clinical outcomes
and reduce the cost of care, while enhancing the patient experience.
The urgent need to achieve those critical goals is driving innovative change at Chase
Brexton Health Care now and into the future.
For example, our leading-edge approach to HIV prevention and care, providing a
full continuum of services through a team-based approach and extensive community
outreach, demonstrates our expertise and is helping patients lead healthier lives.
We understand that Chase Brexton is just one part of the equation. Community agencies,
health care organizations, and our patients are essential partners in our work. During Fiscal
Year 2015, we sought out strong partnerships and implemented new programs to bolster
the care continuum and reduce costs. We created opportunities to increase our capacity
and reach and developed ways to expand our capabilities.
Our new partnership with Northwest Hospital is a picture of what the future will look like
(p. 5). This close partnership connects hospital patients with Chase Brexton primary care
providers and ensures coordinated, cost-effective care.
Our new Anchor Program (p. 12) identifies chronically ill patients with complex medical
problems and helps them take an active role in improving their health. Together, we assess
all aspects of a patient’s situation and find solutions to the barriers that preclude optimum
health. At the same time, the program is reducing hospital visits and emergency room use,

allowing resources to be used more efficiently.
Working with state and federal partners, we expanded our services to Anne Arundel
County in response to the unfortunate closing of a local primary care provider. Our swift
response provided a health care home for thousands of residents who may have otherwise
resorted to local emergency rooms for routine care.
We partnered with generous corporations – new and longstanding – that supplement
funding and provide volunteers to help us deliver on our mission. We have also opened
our doors to the next generation of health care providers through our partnerships with
Baltimore’s MERIT program, Cristo Rey Jesuit High School, and Community Health Corps
(p. 9).
Moving forward to FY 2016, we will continue to improve on the ways we deliver care
to help our patients lead healthier lives and to make our community stronger, while
preserving Chase Brexton’s long-term success. And in the midst of so much change we
remain, as always, committed to our mission to provide compassionate, quality health
care.

One example is Chase
Brexton’s recent partnership
with Northwest Hospital in
Randallstown, MD, created
to foster a healthier, more
educated, engaged and
connected community. The two
organizations work together to
coordinate care and improve
patient access to primary care.
Our partnership is helping
ensure that patients have a
seamless continuum of care
from the time they spend in
the hospital through ongoing
health management back at
home. Through the partnership,
a Chase Brexton liaison and
a Chase Brexton provider are

on-site at Northwest Hospital
weekly, Monday through Friday.
The Chase Brexton team,

patients who lack health
insurance and schedules followup appointments for these
patients prior to discharge. The

any issues and concerns to be
addressed in her follow-up. This
visit also allowed Nikita to know
what her next steps were after
her hospital stay and where she
would go for continued care.
“There are still many people
who don’t yet have primary care
or lost care due to insurance
issues or other barriers,” says
Allard. Allard has been leading
the partnership effort and
expects it to continue to grow
and provide solid continuity of
care. “Through our partnership
with Northwest, we are able
serve as a bridge between
hospitalization and primary
care and address barriers that
could prevent a patient from
accessing care.”

THE SUMMER OF 2014
BROUGHT A POTENTIAL
HEALTHCARE EMERGENCY
TO THOUSANDS OF
ANNE ARUNDEL COUNTY
RESIDENTS.
In financial distress, People’s
Community Health Centers,
the only Federally Qualified
Health Center (FQHC) in the
county, closed on June 30. As a
result, 11,000 under-insured and
uninsured residents faced the
prospect of going without care, or
having to rely on local emergency
rooms for routine medical visits.
Federal, state and local officials
worked quickly to find solutions,
ultimately tapping Chase Brexton
Health Care and another FQHC
to step in. The opportunity to
serve Anne Arundel residents fit
with Chase Brexton’s mission and
plans, and the organization quickly
sought permission from the U.S.
Department of Health and Human
Services to expand. Given that
approval, Chase Brexton opened a
temporary Anne Arundel clinic on
July 25.
The new clinic initially occupied
space in an Anne Arundel County
Department of Health building
until a permanent site in Glen
Burnie could be found. A full
medical team including physicians,
nurses and support staff operated
at first on an abbreviated schedule,
offering adult and pediatric primary
care, obstetrics and gynecology
services, HIV testing and care,
and case management. A focused
outreach effort led to a high
volume of patients, and the clinic
quickly expanded its operating
schedule from three days a week
to five.
“We hit the ground running and
the patients came quickly,” says
Katie Meara, Chase Brexton Health
Care’s Glen Burnie Center Director
of Operations. “Our mission
is to provide compassionate,
quality health care that improves
communities, and this was the
ultimate opportunity to do just
that.”
The entire effort was a true
collaboration between Chase
Brexton staff and the local health
community; it was a true success.
Family Health Centers of Baltimore
also stepped in, arranging
transportation for former People’s
patients to access care at its site
in the Baltimore neighborhood
of Brooklyn. The community
welcomed Chase Brexton with
open arms, and appreciated the
swift response to a critical need
and complex situation.

the community, Dr. Jinlene Chan,
Anne Arundel County’s Health
Officer, said having Chase Brexton
open the Glen Burnie clinic just
three weeks after People’s closed
“is nothing short of a miracle.”

MERIT (Medical Education Resources Initiative for Teens) is an innovative,
intensive three-year program which aims to eliminate health care disparities by
transforming under-represented high school students into health care leaders.
The MERIT program includes weekly Saturday sessions, summer internships, and
mentoring through a student’s sophomore, junior, and senior years. At Chase
Brexton, MERIT students shadow providers and learn about the many aspects of
primary care.
The program is competitive; in its most recent year, 80 students from 18 Baltimore
high schools applied for just 20 openings. Tryouts are required and include courses
in health care careers and interviews with students and their parents. Now in its sixth
year, the MERIT program has placed 100 percent of its students over the past few
years into a four-year college or university.

Now in its second year, Chase Brexton’s program
with Cristo Rey Jesuit High School places students
interested in health care professions alongside
providers who act as mentors and encourage their
dreams. Each student is present at Chase Brexton’s Mt.
Vernon Center one day a week throughout the school
year.

health care and finance, saying her “ideal job” would
involve helping others.
This year’s students coming to Chase Brexton from Cristo Rey also include junior Jade
Johnson, who hopes to become an OB/GYN specialist or a surgeon; sophomore
Armanda Mushayuma, who is returning to Chase Brexton for a second year in the
program; and freshman Zaire Wilson, who is interested in law and health care.
Founded in 1995 by the National Association of Community Health Centers, Community
HealthCorps is the largest health-focused, national AmeriCorps program that promotes
health care for America’s underserved, while developing tomorrow’s health care
workforce.
Over the past several years, Chase Brexton Health Care has been honored with the
opportunity to accept Americorps Community HealthCorps recruits who come from a myriad
of different backgrounds and share a common mission: bolstering community health by
ensuring delivery of the best possible patient care.
During 2014-2015, five Community HealthCorps members were placed in the Case
Management and Outreach departments of different Chase Brexton sites.

“At Chase Brexton, it’s about a person’s health first
and their finances later,” David notes as he reflects on
going from insured to uninsured as a Chase Brexton
patient. And he’d experienced that first hand. Even
when he lost his job – and his health insurance along
with it, he didn’t lose Chase Brexton as his health care
provider.
But, more importantly, her care team wanted to make
sure Evelyn was able to get to her medical, dental,
and other appointments. So, through the Hardship
Fund, they were able to give her bus tokens so she
could get to her important appointments.
To Chase Brexton, how healthy can you be if you’re
worried about your electricity being shut off? Or
what about new eyeglasses? And, if you can’t afford
dentures, what do you do then?
For Susan, a patient who lost her job of three years
and wasn’t able to afford to see her Chase Brexton
providers, the Hardship Fund was a huge relief. She’d
planned to just do without the care she needed
until she could find a way to pay. But, instead, her
provider connected her with a Case Manager who
assisted her in applying for Medicaid. Still, this
insurance was going to take anywhere from 3-6 weeks
to be reviewed and (hopefully) approved. Susan was
willing to delay accessing her primary care provider
and psychiatrist during that time, but the Case
Manager provided her with something different: three
hardship vouchers. These hardship vouchers allowed
Susan to see her primary care provider and access her
critical behavioral health services.
In 2009, Chase Brexton Health Care created the
Hardship Fund to help our most in-need patients
(those with income from 0 to 100% of the Federal
Poverty Level) with the expenses that might not be
covered by insurance or other traditional aid.
And that’s just what the hardship fund is just for.
“Chase Brexton makes it so easy to get the care
you need,” says Evelyn, a mother of four and
grandmother of five who is dealing with multiple
chronic diseases. “My providers work together as a
team, and they get together to brainstorm about how

Achieving optimum patient health isn’t merely
about a patient going for an exam. Every day,
our health care providers - from our OB/GYN
teams to our medical providers, from our nurses
to our behavioral health providers, from our
pharmacists to our case managers - partner with
patients to help them on the road to their best
health.
But sometimes a patient may need more to
reach their healthiest possible life. Introduced
in June, Chase Brexton’s Anchor Program is
one example of a new type of care delivery
model specifically geared toward chronically ill
patients with complex medical problems and
a history of frequent use of emergency rooms
and hospital services to access health care. The
program engages patients to restore their health
through a collaborative, coordinated approach
that increases primary care visits and medication
adherence, all the while decreasing unnecessary
trips to the hospital.
Each patient’s designated care team includes
a case manager, medical provider, nurse care
coordinator, behavioral health specialist,
pharmacist, and medication adherence staff.
The case manager assesses barriers to care,
such as transportation and insurance, and finds
solutions to overcome them.
An important component of the Anchor Program
is accessibility for patients with chronic and
complex health concerns, so Anchor patients
have 24/7 urgent access to their care team and
can access services without an appointment
daily from 9AM to 5PM.
Though the Anchor Program is designed for
patients who struggle with more complex
medical issues that have led them to a greater
number of hospitalizations, it is just one example
of how Chase Brexton is innovating patient
care through a focused effort to build care
teams. Because patient outcomes are stronger
when there is collaboration between all areas
of care, this model of care is being replicated
throughout all the programs and services of
Chase Brexton Health Care to build greater
integration of care and better outcomes for all
our patients.
