The Mandible FixBridge
The Mandible FixBridge is indicated in mandible resection
cases where prepositioning of a reconstruction plate prior
to the resection is impossible due to the location, size and
extension of the tumor. The use of the temporary Mandible
FixBridge ensures the proper positioning of the remaining
bone segments postresection.
Preoperative planning
Preoperative planning is especially important during placement of the Mandible FixBridge to ensure that the appliance
does not interfere with plate placement.
Warning: This description is not sufficient for an immediate
application of the instrumentation. Instruction by an
experienced surgeon in handling this instrumentation is
highly recommended.

Mark the planned resection zone on the bone.

Place the Bone Plate. Use the appropriate Drill Bit for drilling
(the first hole may be drilled without the plate in place). If
necessary, measure depth now. Insert the chosen screw
(2.0 mm or 2.4 mm diameter) through the Bone Plate.
Repeat steps on the opposite side of the mandible.
Note: It is important to place the bone plates posterior
enough on the mandible to avoid interference with the
application of the fixation (reconstruction) plate.

Choose the appropriate length Vertical Post (30 mm or
50 mm) and fix one to each Bone Plate using the Vertical
Post Screws and Hexagonal Screwdriver.

Place a Tube Clamp over each Vertical Post. Clamp screwheads
should face anteriorly. Pass the Vertical Post through the
Tube Clamp distal to the screwhead.
Important: When assembling the Tube Clamps, take care
that the screws pass through the unthreaded clamp first
and then into the threaded one. The latter must be positioned
so that the threaded part of the clamp finishes off the
construction.

Choose the correct length Extension Bow (50 mm or 70 mm)
and insert its ends through the remaining holes of the Tube
Clamps.

Adjust the position of the Extension Bow and tighten all
screws carefully, since they are fixing the original shape of
the mandible. Mandible FixBridge is now in its final position.
If the mounted Mandible FixBridge does not hinder the
resection, resect and go on to Step 11. Otherwise, continue
with Step 7.

Remove the two Vertical Post Screws.

Remove the assembled Mandible FixBridge from the
mandible as a unit (consisting of Extension Bow, Tube
Clamps, and Vertical Posts). Store Mandible FixBridge carefully
to avoid any distortion before it is reapplied after resection.

Perform the resection.

Reapply Mandible FixBridge by inserting the Vertical Post
Screws into the Bone Plates. Take care to position FixBridge
accurately, without deviation. The bone stumps are now in
their original relationship to each other.

Select and contour the appropriate reconstruction plate and
secure it with the screws in the mandibular bone segments.
Once the reconstruction is complete, with the condylar position
confirmed and the plate in place, remove all components of
the FixBridge.

Resection is complete with definitive fixation in place.
Condylar position and occlusal relationship are indispensable
requirements for reconstructing the mandible. The use of the
FixBridge ensures that these relationships are maintained
throughout the resection.
