Program
The aim of the Program “Health system strengthening with a
focus on reproductive health” of the Malawi-German Cooperation is that the Ministry of Health (MoH) in cooperation
with non-state actors ensures effective and efficient health
care of appropriate quality, with special consideration for the
health of mothers and newborns. Three program components
contribute to the attainment of this aim, in accordance with
the national Health Sector Strategic Plan: Quality assurance;
HR management and development; Partnership with non-state
actors.
In response to the risks and impacts, GIZ supported in a first
program phase in Malawi the implementation of the National
Care of the Carer HIV and AIDS Workplace Policy (2005). This
document represents the Ministry of Health’s HIV and AIDS
workplace policy for all health sector institutions. It strives
for the creation of a safe working environment for health
care workers with regard to HIV and AIDS, Hepatitis B and
other infectious diseases. The Policy addresses the following
strategic objectives: to prevent the transmission of HIV and
other infectious diseases in the health care setting; to provide
support for health care workers caring for AIDS patients as
well as to support health care workers who are infected with
the virus (including the provision of post exposure prophylaxis - PEP). In this respect, the German Technical Cooperation
used the multilevel approach to reach the national (Ministry of
Health, National AIDS Commission), zonal (Zonal Health Support Offices - ZHSOs) and decentralized (central and district
hospitals) level. At national level, the Program supported

the formulation, review and distribution of the Care of the
Carer Policy as well as the Guidelines on the allocation of 2%
of “Other Recurrent Transactions” to HIV & AIDS workplace
programs.
Impact
There has been important progress made in implementing the
Care of the Carer Policy in the institutions supported by the GIZ
Program in particular concerning behaviour change among health
care workers and workplace-based responses to HIV in hospitals
which lead to the prevention of new HIV infections. The attitude

health care facilities although stigma and discrimination continue
to be strong forces and barriers to service uptake. The Program has
supported the four central hospitals as well as a number of district
hospitals through ZHSOs in the elaboration of HIV and AIDS
workplace programs (WPP) and the creation of HIV and AIDS support committees through trained focal persons. The committees
established “staff clinics” which provide health care workers with
PEP whenever necessary. Moreover, the Program supported the
establishment of a network of peer educators in health care settings to sensitize and support their workmates.
It is anticipated that these measures will contribute to the
mitigation of new HIV infections within the health care workforce therefore reducing absenteeism, creating a healthier work
environment and increasing employee satisfaction.
and where required the ARV treatment and the prescription of
PMTCT drugs to pregnant women and newborns are promoted
and enhanced. Access to essential HIV services is improved
through stronger engagement of private commercial organizations and other non-state actors.

Present Phase
The GIZ Health program continues to mainstream HIV and AIDS
response measures into its activities, following the respective
BMZ policy 2010. Moreover, in line with the BMZ policy “Linking
SRHR and HIV & AIDS: Possible GDC contributions for improving
maternal and child health” (2011), the present second phase follows a more integrated approach to combat the adverse effects
of the HIV pandemic. The program operates at several levels in
order to improve the quality of health service delivery in collaboration with the Ministry of Health. Through the implementation
of quality assurance measures, clinical expert advice, on-the-job
training of health personnel and strengthening referral systems
