OVERVIEW
A Services Plan is a yearly written plan describing the education to be provided to a student with a disability who is in
need of special education services. It will contain a summary of the programs, services, goals, and objectives to be
provided to the student. This information is documented following a Services Plan team meeting held with one or both
of the student’s parents/guardians, the student’s special & regular education teachers, and a representative from the
local education agency. The Services Plan team may also include the student, if transition services are being planned, or
if the parents/guardians choose to have the student participate. Before creating a Services Plan form, it is
recommended to first create an ‘Invitation Letter’ to schedule & invite the student’s team to the meeting.
FORM INSTRUCTIONS
FORM CREATION
The form creation process includes the following steps:
Step 1: Select a Student
Step 2: Select Options
STEP 1 – SELECT A STUDENT
1. Click Student Management > Manage an Existing Student.
2. Use the Search window to find the student based on either, Last Name, First Name or Student ID.
3. Click Search. Then click the appropriate Name to select a student.
4. Once a student has been selected, the student’s name, student ID, birth date, and age will display. The forms
that can be created for a student will also be listed. Click on the ‘Services Plan’ link.
The ‘Services Plan Summary’ screen will display, listing any prior forms created for the student. The most recent form
will display at the top of the list. To review an existing form, click the appropriate link. To create a new form, click the
‘New’ icon.

STEP 2 – SELECT OPTIONS
1. When the ‘New’ button is selected (see screenshot above), the Form Creation Wizard displays. This wizard is an
automatic template that helps the user to easily setup the form (see screenshot below).
2. Options:
A. Select Process Method:
a. Annual Method - This method should be used to create Services Plans that do not span the year.
Gaps between Services Plans will be ignored.
b. Anniversary Method - The method should be used to create Services Plan that span the year. Gaps
between Services Plans will be detected and displayed for resolution. If gaps are detected, a new
screen will pop-up asking the user to resolve or ignore the gaps between Services Plans. Below is an
example of what that would look like:
The records copied will depend on record types selected in Options > Set System Options > Forms (see section
on System Options for more information).

*Note: If there is a gap between the End Date of the previous Services Plan and the Start Date of the next
Services Plan, an extension reason may have to be indicated depending on the district’s settings in System
Options. For example, if the prior Services Plan ends 12/31/12 and the next Services Plan begins on 1/15/13
(i.e., there is a gap between the prior and next Services Plan that exceeds ‘X’ number of days), the wizard will
prompt the user to enter a reason for the gap (see screenshot below) and will extend the previous plan to
1/14/13. The records would have to be extended manually. The extension reason only applies if the Process
Method for the Services Plan is ‘Anniversary Method’; it will not apply if the Process Method is ‘Annual Method’.
Refer to the System Options section herein for more information on these settings.

Select Options:
Services Plan Start & End Dates – starting & ending dates of the form.
No Services Plan found – the system will try to copy all records within the last 12 months.
Create a blank Services Plan – the system will not copy records.
3. When done, click the “Create Services Plan” link. The user also has the option to click ‘Cancel’ as well to clear
the form. If this is chosen, then the form wizard will return the user back to the ‘Services Plan Summary’ screen.
NAVIGATION/TOOL BAR
Once the form has been created, it will display a toolbar on the left side of the screen, divided into the following areas:
1. Sections
This displays the Main, Educational History, Background Information, and View All sections of the form. Clicking
on any one of these sections will bring the user to that specific section.
2. Maintenance
These options will maintain Form Information, Refresh, Mass Statement Change, Rapid Entry, Rapid Grid Entry,
and Delete.
Form Information provides the ability to change the Form Date Range, Letterhead, Meeting, and Evaluation/Test
Date Range.
Refresh, Mass Statement Change, Rapid Entry, Rapid Grid Entry, and Delete functionality is available, depending
on the user’s security.
3. Validation
These options will change the status of the form (Validate, Ready for Review, Lock, and Finalize).
4. File Attachments
Click the plus icon (+) to add an attachment. Click the minus icon to delete an attachment.
5. Archives
Archives are permanent PDF copies of the form. Click Finalize in the Validation section to create an Archived
copy. The form should be finalized and archived only after it has been fully completed and a copy has been sent
home to the student’s parents or guardians.
Archives will display the form Meeting Date or Form Start Date (if no meeting exists) and the actual date and
time the record was created when hovering over the PDF icon. Archives will display in descending order by date
created.

*Note: Archives may only display the date created and not the meeting/form date, depending on district
settings maintained under Options > Set System Options > Form:
The following options will display at the top and bottom of the form (see screenshot below):

Save
Information from each section should be saved before moving to another section.
Return
Click the Return icon to return to the Form Summary screen.
Print Preview
Print Preview will display the form in Adobe Acrobat. Mouse over the bottom of the form preview to see the toolbar
illustrated below. Select the Print icon or click the Save icon to save a copy of the form to your computer. The plus
(+) icon will enlarge the preview for ease of viewing.
REVISING A SERVICES PLAN
To revise a finalized Service Plan form, click the ‘Amendment’ link illustrated in the screenshot below. This link only
appears for finalized active or future Service Plans. If the form has already expired, the user will not see the ‘Revise’ link.
Clicking this link will unlock the form for editing.

NOTE: The ‘Revise’ link can be relabeled – to do this, navigate to Home > Maintenance > Manage Screen Configuration >
FormSummary. From here, the user can edit the Item Name, change the Display Text, or hide it:
When revising/amending an IEP or Service Plan through the link on the form summary screen, the program will allow the
user to re-select the original meeting and any meeting type, without changing the original EMIS reported meeting type.
The meeting type will save/print to the form only and will not update the selected meeting record. The meeting type
selected on the form will update the meeting record only if it is not a revision/amendment.
REMOVE AMENDMENT
This functionality allows the user (based on security permissions) to remove an Amendment/Revision when the form
was inadvertently amended/revised.
To remove the Amendment/Revision and return the form to its previous state, click on the ‘Remove Amendment’ link
(Screen #1) from the Form (e.g. Services) Summary page. Upon clicking on the link, a pop-up screen (Screen #2) will
display, alerting the user that any modifications they made to the form must be manually changed back prior to the
‘Remove Amendment’ process. Please insure that the applicable meeting is selected prior to removing the
amendment. The Remove Amendment process does not remove any changes that were made after the form was
revised. Clicking the ‘OK’ button will perform the amendment removal process, while clicking the ‘Cancel’ button will
return the user back to the Form Summary Page and the form will remain in the revised state.
The ‘Remove Amendment’ process will perform the following:
• Re-finalizes the form.
• Adds additional information in the “Form History” area of the form (see “Form History” section below for more
details).

FORM HISTORY
Upon removing the revision, two audit records will be inserted into the Maintenance > Form Information > Form History
indicating the Remove Amendment process:
1. The first record displays the ‘Amendment Removal’ process, date, time and user.
2. The second record displays the form returning to the ‘Lock’ state, date, time and user.
SECURITY
To grant a group or user permission to the Remove Amendment link, navigate to Security > Manage Groups >
Permissions, or Security > Manage Users > Permissions, then check on the ‘Edit’ checkbox on the Permission > Form >
Remove Amendment line item and then click the ‘Save’ button. The Remove Amendment security option is global to all
forms.

SCREEN CONFIGURATION
The District has the ability to change the “Amendment Removal” label. Navigate to Maintenance > Manage Screen
Configuration > Form Summary and then click on the ‘Edit’ button on the Item Name called “Remove Amendment”. In
the ‘Display Text’ column, rename it as desired, and save the changes.
FORM COMPLETION
COVER PAGE
The cover page includes general information about the student, the meeting and other dates relating to the special
education timeline and form status by section. Most of the fields in this section will auto-populate. For more
information on these fields or any other areas of the form, please refer to the Data Mapping section of this guide.
Complete the Cover Page as follows.
1. District of Residence
This field will populate based on the default Organization in System or User Options. The user can select
another Organization from the drop down list. The selection will create/update an Associated Organization
record (Type = District of Residence) as of the form Start Date. To view or edit this record, return out of the
form and navigate to Educational Information, Associated Organizations via the Student Toolbar.
2. County of Residence
Same as #1 above (Associated Organization Type = County of Residence).
3. District of Service
Same as #1 & 2 above (Associated Organization Type = District of Service).
4. Meeting Date
The meeting record is created typically from the Invitation Letter form. Select the Meeting from the drop down
list. Only Meeting records NOT included previously on a finalized form will display. Only Meeting Types with an
EMIS Alternate Code = IISP and RISP will display.

Parent/Guardian information will display based on the student’s Contacts. To maintain contact information,
click Return and select the Contacts link from the Student Toolbar.
6. Services Plan Time Lines
This section displays important dates relating to the special education timeline, such as the date of the last
completed Evaluation Team Report (ETR), the Next ETR Due Date, Services Plan Effective Date, Services Plan
Completion Date, and the Next Services Plan Review Date. The ETR Completion Date and Next ETR Due Date
dates will automatically populate based on Educational Information; Important Date, or Meeting records
accessible through the Student Toolbar.
7. Services Plan Form Status
Checks are made in individual sections upon their completion and will display on the cover page.
SECTION 1: MEASURABLE ANNUAL GOALS
This section consists of statements describing the student’s annual goals and objectives (or benchmarks). Click the ‘Add
Narrative’ or ‘Add from Goal Book’ button to enter text, or select a pre-defined statement describing the student’s goals.
After selecting/editing a goal book statement, click the ‘Overview’ button to return to the Services Plan. Next, click the
‘Edit’ button to add the Present Levels of Performance, and Method and Frequency of Reporting Progress. Start and End
Dates will default to the Services Plan date range, but can be modified if needed. When finished, click the ‘Update’
button. After adding a goal, additional buttons will display for creating the objectives (or benchmarks) related to that
goal. Click the ‘Add Narrative’ or ‘Add from Goal Book’ button to create objectives (or benchmarks) similar to what was
done for the goals.
When the user is done with this page, they should mark the ‘Check when complete’ checkbox and then the ‘Save’
button. This will automatically check the “1. Measurable Annual Goals” checkbox on the Cover Page under the
“Services Plan Form Status”.
SECTION 2: SPECIALLY DESIGNED SERVCIES
This section provides the ability to add Specially Designed Instruction, SDI Services, Related Services, Assistive
Technology, Accommodations, Modifications, Support for School Personnel, School Personnel Services, and Service(s) to
Support Medical Needs for a student. Click the applicable ‘Add’ button to enter text or select a pre-defined statement
from a goal book. After selecting a goal book statement, click the ‘Save and Return’ button to return to the Services
Plan. Next, click the ‘Edit’ button to select Goals Addressed, Provider Title, Location of Services, Duration, and
Frequency. The Start and End Dates will default to the Services Plan date range but can be modified, if needed. When
finished, click the ‘Update’ button.

When the user is done with this page, they should mark the ‘Check when complete’ checkbox and then the ‘Save’
button. This will automatically check the “2. Specially Designed Services” checkbox on the Cover Page under the
“Services Plan Form Status”.
SECTION 3: STATEWIDE AND DISTRICTWIDE TESTING
Use the check boxes and textboxes in this section to indicate the state-wide and/or district-wide assessments that the
student will take, whether or not the student will require accommodations to participate, what the accommodations
are, and if appropriate, why the student cannot participate in the regular assessment.
Before any checkboxes are selected on the page, there are only 4 ‘Yes/No’ questions that need to be answered by
selecting the appropriate checkbox. Each question (#1 – 4) and it’s corresponding additional checkboxes, grids, and/or
date field will open or close, depending on whether the Yes or No checkbox is selected for that question.
Special Note
Questions #1 & 4 cannot both be ‘Yes’, but they both are allowed to be ‘No’ at the same time. As such, the program has
logic built-in to prevent this from occurring:
• If the user selects the Yes checkbox for question #1, then the system automatically selects the No checkbox for
question #4.
• If the user selects the Yes checkbox for question #4, then the system automatically selects the No checkbox for
question #1.
• If the user selects the No checkbox for question #1, then the system automatically selects the Yes checkbox for
question #4, however, the user may change the Yes checkbox to No for question #4 if they choose.
• If the user selects the No checkbox for question #4, then the system automatically selects the Yes checkbox for
question #1, however, the user may change the Yes checkbox to No for question #1 if they choose.
When the user is done with this page, they should mark the ‘Check when complete’ checkbox and then the ‘Save’
button. This will automatically check the “3. Statewide and District Testing” checkbox on the Cover Page under the
“Services Plan Form Status”.
SECTION 4: MEETING PARTICIPANTS
Choose the ‘Team’ drop-down, and/or click the ‘Add Meeting Attendee’ link to indicate the names and positions of the
members of the Services Plan Team that were invited to the meeting.
If the ‘Team’ drop-down is chosen, the grid will populate with the team that is associated to the Tame Name. This is
maintained through the ‘Maintenance, Manage Teams, Main, Team’ screen.
If the ‘Add Meeting Attendee’ is chosen, then a search box will open. Click in the search box, enter the search, and
select from the displayed results. Select a Role from the drop down list, and click the Update link to complete this
process. To edit or delete a team member, click the appropriate link.
Update the ‘Attended’ check box to indicate the individuals that did attend, or the ‘Attended/Excusal Reason’ dropdown to select the reason an individual did not attend the meeting. This information is saved in the Meeting record.
When the user is done with this page, they should mark the ‘Check when complete’ checkbox and then the ‘Save’
button. This will automatically check the “4. Meeting Participants” checkbox on the Cover Page under the “Services
Plan Form Status”.

SECTION 5: SIGNATURES
This section provides printed signature lines for the parent(s)/guardian(s) and check boxes to specify Consent, Consent
except for Area with line to specify area, or No Consent. It also includes information regarding dates in which copies of
the Procedural Safeguards Notice and Services Plan were provided to the parents.
When the user is done with this page, they should mark the ‘Check when complete’ checkbox and then the ‘Save’
button. This will automatically check the “5. Signatures” checkbox on the Cover Page under the “Services Plan Form
Status”.
EMIS SUMMARY
IEPPLUS creates student data in which Ohio uses to upload to their Ohio Education Management Information System
(EMIS) for student data collection. There are 5 pieces of student information contained within the EMIS Summary Form:
1. Disabilities – This automatically populates the EMIS reportable Disability Codes based on the ‘Disability’ Code
Table, Alternate Code (Type = EMIS Code) and Description = (the applicable EMIS Disability). To maintain the
student’s Disability information, click the ‘Return’ button at the top of the screen, and select the ‘Disability’ link
on the Student Navigation Toolbar on the left-hand side of the screen.
2. Services – The Services grid displays the student’s services that are within the date range of the Service Plan. To
add a Service, click the ‘Add Service’ button. This will open up a new section under the ‘Services’ heading where
the user can add the new Service record. Click the ‘Update’ button to update the Service record:
3. Meeting/Event – This field automatically displays the EMIS reportable Outcome Codes based on the ‘Meeting
Result’ Code Table, Alternate Code (Type = EMIS Code), and Description = (the applicable EMIS Outcome Code).
The application will limit the available outcomes based on the Meeting or Referral Result, Alternate Code (Type
= Result Code), and Description = (the allowable Meeting Type Codes – ex: IISP and RISP). This field will
automatically pull the Meeting information from the Meeting record that is associated to the OH Services Plan
form. The meeting can be managed by clicking on the Meeting Type link. The ‘Test Type’ field can be managed
by using the drop-down box provided. The Test Type drop down menu will display Test Types with an EMIS
Alternate Code and a Description value.
4. OGT Subject(s) – The field displays all the Ohio Graduation Test Subjects identified in the Meeting record linked
to the form. A user can manage this field by clicking the Meeting Type link to check the appropriate checkboxes
for the Ohio Graduation Test as illustrated below. After modifying the checkboxes, click the ‘Save’ button to
return to the EMIS Summary section of the Services Plan form.

5. Previous Meetings – This field automatically displays (as read-only) any previously linked meetings to the
Service Plan (e.g. Initial/Annual) and any previous revisions when the Meeting Type (EMIS Alternate Code = IISP
or RISP). The grid enables users to view the existing 'Test Type' selected during their initial or annual review.
6. Placements – This grid gives the user the ability to add, edit, or delete Placement records when the Meeting
Reason = ‘Review other than Annual’. Placements are displayed in ascending order based on the Start Date.
The Secondary Planning drop-down box will display next to the Placement whose Start Date is on or after the
Meeting Date. The user can maintain the Placements by clicking on the Placement record, or by adding a new
Placement with the click of the ‘Add Placement’ button.
PRINT EMIS SUMMARY REPORT
The EMIS Summary includes state reporting information, such as Disabilities, Services, Meeting/Event, and Placements.
Click the ‘Meeting’ link to update the Meeting Outcome. The EMIS Summary does not print along with the Services Plan
form. As such, to print the EMIS Summary, select the ‘Ohio SP - EMIS Summary’ from the Reports’ drop-down at the top
of the screen.

SETUP AND CONFIGURATION
CODE TABLE SETUP
1. Navigate to Maintenance > Manage Code Tables.
2. Select the name of the table from the drop down list (ex: “Meeting Type”).
3. To add a code, choose the ‘New’ icon from the top menu.
4. Enter the Description in the yellow box, then click on the ‘Update’ link, and then click the ‘Save’ icon from the
top menu.

5. To add or edit an Alternate Code, first click ‘Show Alternate Codes’ at the top of the page. Then click Edit on the
applicable Code record to open the record for changes.
Once the record is open, click the appropriate link to add a new Alternate Code or Edit an existing one. Clicking the
‘Delete’ icon will remove an existing Alternate Code. When finished with the changes, click the ‘Save’ icon.
GOAL BOOK SETUP
In order to get the “METHOD(S)” field in Section 1: ‘Measurable Annual Goals’ to appear, the user must have the correct
goal book Alternate Codes setup. The Goal Book Setup is similar to the Code Table Setup mentioned in the previous
section.

1. Go to Maintenance, Manage Goal Books, and select the goal book from the drop-down box. Then choose the
“Filter Goals by” drop-downs boxes as well – Type, Attachment:
2. Next, click on the ‘Code’ in the table where the ‘Category’ = “Progress Method”. (For example: choose ‘Code’ =
“20001”)
3. This will bring up the ‘Statement Overview’ screen. Click on the ‘Alternate Codes’ link:

Code Link’:
5. Select “Alternate Code” from the ‘Type’ drop-down box, and type in the appropriate Alternate Code. In this
example, the appropriate Alternate Code = “a.”:

From this screen, the user can also add a Start/End Date, and Update, or Cancel the entry. When adding or
editing an entry, click the ‘Save’ button to save the entry.
REVISE/AMEND – REMOVE AMENDMENT WARNING MESSAGES
A new option has been added that allows administrators to enable/disable warning messages for the ‘Revise/Amend’ &
‘Remove Amendment’ actions. To do this, the administrator would navigate to Options > Set Systemwide Options >
Form and then enable/disable either or both of the options. By default, both warning messages are turned on (both
checkboxes are unchecked).

To enable the “Revise/Amend” warning message, the administrator should leave the 1st checkbox unchecked.
When this option is enabled, the user will see the following pop-up message when the “Revise/Amend” link on
the Form Summary page is clicked:

To enable the “Remove Amendment” warning message, the administrator should leave the 2nd checkbox
unchecked. When this option is enabled, the user will see the following pop-up message when the “Remove
