Dr. Hibberd’s Patient Information
1.Appointments
If you are running late for your appointment please
contact our office. We ask that you register for the
patient portal if you have not already done so.
2.Rescheduling
If you need to reschedule your appointment
please call 210-396-5250. We ask that you cancel/
reschedule your appointment at least 24 hours in
advance. You may also reschedule your appointment
through the patient portal. If you are calling to
schedule or reschedule your surgery please contact
Monica at 210-396-5250.
3.Check-in Online
If you are already registered on the patient portal,
you may check in online. If you experience any
difficulty with the portal please feel free to contact
our office operators for assistance at 210-804-5400.
4.Calling the Doctor’s Office
Our office hours are Monday through Friday 8am
to 5pm. Our clinic days are Monday, Tuesday and
Thursday. We are not always able to answer calls
during our clinic hours. If you reach our voice mail,
please leave a message. Your call will be returned as
soon as possible. You may also contact us through
the patient portal.
For after hours emergencies, we invite you to contact
Dr. Hibberd directly by cell phone. This number will
be given to you at the time of your appointment.
5.FMLA Forms
Please bring any FMLA or disability forms you would
like completed by the physician. There is a $25 fee
for the completion of FMLA paperwork and a $15
fee for disability forms. We ask that you bring the
exact amount in cash as we are not always able to
make change. Please allow 7 to 10 business days for
forms to be completed. You may contact Monica with
questions regarding your forms at 210-396-5250.
6.Prescription Refills
Please contact your pharmacy and have them fax
us a refill request. Once we receive the request the
doctor will review and authorize if appropriate. If
you are unable to reach us by phone about your
medication, you may also contact us through the
patient portal. You may contact Vanessa regarding
questions about your refill and or medications
