President’s Note
Dear Friends,
In 2014, philanthropic support helped us to maintain our mission-driven focus, one that we will not
waver from although the financial challenges in healthcare loom large. With strong leadership from
our Board of Trustees and executive management team, we remain firm in our 119 year tradition of
delivering quality care to meet the needs of our community. No other organization has the range and
depth of expertise, the innovations in technology, and the level of connection with the people served
as the agencies of Home Health Foundation. As an independent, not-for-profit family of agencies,
we rise to meet the challenges of an uncertain time by looking forward and planning ahead.
We have spent the past year finding innovative ways to better meet the increasing needs of our community
and adapted our organization to do so. We have forged relationships with healthcare partners to further
enhance the role we uniquely play in the care of patients as well as the healthcare system. We have
invested in communications infrastructure to work more collaboratively and efficiently to improve
quality of care for our patients.
While we work hard to control the factors we can, more than ever, the generosity of each and every
donor allows us to bridge the gaps in payments and challenges in reimbursements. This charitable
support from throughout the community allows us to ensure that no one is denied the medical care
they need, while upholding the services and care we uniquely provide to each of the thousands we
care for every day.

maintain the quality of our services for those who rely upon us.

of expert health, hospice and home care providers follow along – to a lifetime family home, a new apartment in
an independent living community, a skilled nursing facility, or at Merrimack Valley Hospice House.
Our three not-for-profit agencies, Home Health VNA, Merrimack Valley Hospice and HomeCare, Inc., work together
to help our patients and their families enjoy the special moments - large and small. We keep people in the places
they feel at home, with comfort and safety. We are responsible and accountable to the communities and people we

What it means to care for all
Our mission is to create and deliver innovative
and responsive community health programs which provide
effective and compassionate care.

Serving our mission
As a not-for-profit agency, we are committed to providing vital healthcare to all
patients, regardless of their type of insurance or ability to pay. Since 1895 we have
provided care to the underserved – most who would not otherwise receive medical
care, as the result of financial hardship or physical isolation. Many of these people
are homebound and alone without family to assist. For them to seek and receive care
outside the home is nearly an impossible hurdle. Without Home Health VNA to
care for these people, there would be incredible suffering, unnecessary hospitalizations,
and a population of people with no advocate to address or to deliver the medical care
and social services that can provide human dignity and quality of life.
Our quality of care, medical expertise and best practices go above and beyond basic
standards of care. It is care deserved by all. By accepting all patients, we care for those
who would not otherwise receive care.
The ability to continue to do this, as today’s health reimbursements cover less and
less of the costs of the care, is made possible in large part by the generosity of
our donors. We are so grateful to the many who believe that others are worthy of
receiving the same care they have received; that the impact of the work we do matters,
for them, their families, their neighbors, and the community we are a part of.
Caring for others is part of the human experience. By supporting the agencies of

We are healthcare’s connection
to the patient at home
We work collaboratively with a wide network of
healthcare partners that mutually provide care to the
110+ towns in our service area. We are instrumental
in management of high risk patients, those who are
most vulnerable to complications related to their
disease and repeated hospitalizations. Integrating
technology with health systems, hospitals and
physician practices, we seamlessly coordinate the care
of our patients – making each and every home visit
valuable to patients’ goals and desired outcomes.
We are the first and only home health agency to
lead a Massachusetts Health Information Exchange,
an important initiative bringing together partners
to share clinical information of our mutual patients
across the many access points of care.
We are here to fill the needs of healthcare today
and tomorrow as we understand, define and help
guide the integrated systems in the healthcare
networks that care for thousands of patients across
Massachusetts, New Hampshire and Maine.

Home Health VNA
We bring healthcare home
The importance of healthcare in the home setting is increasing exponentially as patients are living with more serious illnesses, coping with more
complicated health challenges and receiving care from a number of providers. We provide services and rehabilitation for those with medical
needs related to an incident such as a traumatic injury, stroke, or surgery, as well as chronic or long-term illnesses for children as well as adults.
For each patient we care for, we have the ability to see and work with them in their own home. We are a unique partner in healthcare today,
relied upon by patients as well as the medical community.
For generations, we have understood the value of community-based care at home and the distinct benefits we bring to patients and the healthcare
providers caring for them. We have built a solid foundation on providing innovative care and programs to meet the needs of our community, in
the places they call home, and our care teams are on-the-road, in-the-home, medical experts.

Caring for the population’s most vulnerable
More than ever, we are caring for the most vulnerable of patients
in the community, those with complex care needs resulting from a
combination of serious medical conditions. These people are at an
elevated risk for serious injury or rapid decline in health. Our goal
is to reduce the controllable risks, intervene with proper treatments
before complications, and improve quality of life for these patients.
These patients are cared for with specific protocols that involve
our home nursing staff to identify and carefully address each
patient’s risks with a care plan that closely monitors and meets
their needs. In doing so, the home care nurse becomes the hub –
communicating with the patient’s support system and medical
providers. The care plan is managed across all providers, ensuring
that a full picture is kept in view.
Finding solutions that work
In 2014, several new programs and approaches to care brought
improved results to manage some of the most challenging types of
care. The Clear Path Project improves care for homebound seniors
with mental health conditions by providing psychiatric training to
the visiting clinical team tending to these people. The health team
can help identify and assess risks, help manage medications, and
connect them to proper resources before crisis.
We have incorporated best practices into every patient visit with
clinician toolkits and defined pathways for chronic diseases. While
every patient receives a personalized care plan, we ensure a standard
of consistent, effective, quality care, with each and every visit.
Interdisciplinary teams remain an important part of care collaboration.
Working together, complex care can be managed and often prevents
serious declines and related hospitalizations. Identifying risks as a
team and finding appropriate services to meet them, helps improve
an individual patient’s care while extending the solutions to other
patients who may benefit from the new approaches.
Getting a healthy start
Motherhood is a joyful, exciting time, but it can be stressful too.
Ask any new mom and she’ll tell you that loving and caring for a
newborn can be both wonderful and difficult. The Massachusetts
Department of Public Health teamed up with Home Health VNA
for their Welcome Family program in the Greater Lawrence region.
This program offers a free visit from one of our Maternal and Child
Health nurses – free to any new mom and her newborn. During the
visit, the nurse does a health assessment, answers questions and
connects them to other helpful information and resources.
The Welcome Family program was started in July 2014 and will
run through June of 2015 to help ensure a solid start by sharing
expertise in calming an infant; bathing and changing techniques;
healthy nutrition; breastfeeding; and home safety. In helping our
youngest members get settled in at home, we say “Welcome Family”!
Technology to support
The ability of our agency clinicians to communicate with each
other as well as share patient information with physicians and
hospitals, is at the core of our communication and care management.
From real-time reporting, to patient information, our staff is
connected by mobile computing. We received a special challenge
grant from the Amelia Peabody Charitable Fund which was followed
by an incredible outpouring of donor support at the end of 2014.
With these funds, our clinical teams will have updated equipment
to enhance the connectivity they need to receive and share the
most important information related to every patient they visit.
The high-tech aspect of healthcare is very much behind the
scenes as the backbone and a critical component of working
within healthcare systems of today.

August and September from your staff members following surgery. I was completely dependent on
your staff as I healed and credit them for my ability to gain confidence and the ability to get back

We ease worries and restore peace of mind
Hospice isn’t about giving up hope, it’s about creating new hopes and carrying out wishes for comfort, dignity and the life that remains.
Hospice is an important part of caring for those who are no longer seeking curative measures for illness or conditions that are life-limiting.
Merrimack Valley Hospice provides compassionate, comprehensive care designed to improve quality of life by meeting physical, social and
emotional needs with a team of professionals and volunteers. At home, in a hospital, in an assisted living facility or at our own Merrimack
Valley Hospice House in Haverhill, hospice care fosters peace and comfort with support for patients and their families.

Creating special moments
For 35 years Merrimack Valley Hospice has
provided compassionate physical, emotional
and spiritual care for those with advanced
illness. Meeting these needs by properly
managing pain and symptoms, and focusing
on quality of life, hospice allows for time to
strengthen relationships and create special
moments with loved ones.
Providing a compassionate team
The hospice team involves many professionals
who tend to symptom management,
medication and supplies, care coordination
and specialty care and support. Each person
in the team helps to meet the many and often
changing needs that arise. With hospice and
palliative nurses, board certified practitioners,
aides, social workers, multi-denominational
chaplains and spiritual advisors, bereavement
counselors and trained volunteers, caring
experts provide help along the way.
Addressing the unique
needs of chronic conditions
We developed programs for those with
chronic conditions such as advanced cardiac
and pulmonary disease, dementia and memory
impairment, and advanced cancer. With
medical and emotional support, hospice can
dramatically reduce anxiety and symptoms of
distress, keeping patients more comfortable
and able to enjoy greater stability.
The support of the greater York community has been tremendous, and is
allowing us to continue to enhance the offerings available year-round with
regular community events, bereavement programs and support groups,
and a growing volunteer network that cares deeply for its neighbors with
their time and talent.
Thanks to hospice, the last nine
days of my mom’s life were such
an amazing experience for me,
and one that I will never forget. I
actually loved every minute of the
time we had together in her own
home in York, with no anxiety and
no fear. I thank my mom’s hospice
aide, Elizabeth, for all she did. Her
loving care of washing, grooming,
and making my mom feel good was
beyond belief. With her observations,
she communicated with the hospice
nursing team and advised me on
the medications my mom needed to
stay comfortable. With her helpful
guidance, we were able to care for
mom as well as plan for the days
ahead. There are so many things
that Elizabeth told me to help care
for my mom in between her visits
and it made such a difference.

York Hospital Hospice provides medical support, bereavement, and dedicated
volunteers to the York, Maine community. Its affiliation with Merrimack
Valley Hospice brought a professional level of advanced medical care and
breadth of resources to this region and has impacted hundreds of families.
York Hospital Hospice has been embraced by the medical community and
families alike. 2014 was a year of growth as our clinical team expanded into
this region to care for people in their homes, keeping them in the place they
feel most comfortable. For instances when medical needs become too great
for remaining at home, Merrimack Valley Hospice House becomes that
“home away from home” with its traditional New England charm and
comfort for patients and their visitors.

Merrimack Valley Hospice House
We have a place like home
Most hospice and palliative care is provided to patients in their own homes. But when it’s no longer possible for someone to remain at home
for advanced care, Merrimack Valley Hospice House provides a comfortable, welcoming home-like alternative. Patients who may need more
intensive pain management for a short time or require the oversight of a physician to help address complex symptoms may turn to the
specialized care at the Hospice House and then return home.
Merrimack Valley Hospice House is a place that brings trained hospice professionals together – physicians, registered nurses, nursing assistants,
chaplains, social workers and volunteers – to provide highly specialized care to patients with advanced illness as well as support to their loved ones.

Specialized care in a place like home
One of only two inpatient facilities in Massachusetts to care
for children as well as adults, Merrimack Valley Hospice House
addresses specialized medical needs of those with advanced illness,
with compassion and dignity. Patients may come to Merrimack
Valley Hospice House after being cared for by a hospice provider
at home or in an assisted living or skilled nursing facility. They may
not have had any type of hospice care before coming, or perhaps
they have been receiving only palliative services. Some families turn
to Merrimack Valley Hospice House for respite, a break from care
giving. Every patient is unique and an experienced admission team
guides the transition to or from the Hospice House.
So moments can be shared together
During life’s journey we celebrate many events and milestones.
Merrimack Valley Hospice House’s expansion to 21 patient suites
is a milestone that benefits every community served by our agencies
– whether they are receiving care from Merrimack Valley Hospice,
Home Health VNA or another home health or hospice provider.
The expansion to the House has increased access for short-term
acute symptom management, respite care, and those who need or
want to be in a setting with a higher level of specialized care as their
illness progresses. With more rooms to accommodate patients and
their families, more treasured moments can be spent together as
fears are erased and comfort is regained.

A Compassionate Journey Campaign in 2014. We invite all to continue
or join in supporting this continued effort to ensure a place of
comfort, dignity and care for those nearing the end of their life’s journey.
The need for ongoing support continues
Charitable giving allows this special resource to be available to all when
hospice needs intensify beyond what can be provided in the patient’s

Hopeful for the best things possible
The following is an excerpt from a personal letter of thanks to a donor, written at Merrimack Valley Hospice House.
“While all of us are devastated that Peter’s brother was admitted to Merrimack Valley Hospice House, we are relieved that
he will receive exceptional and thoughtful care so that he can live the best life possible until his last breath. He can be visited
by a variety of people who might not otherwise be able to see him at his house. The large private room facilitates heartfelt
conversations and multiple visitors. The numerous sitting areas around the Hospice House allow visitors to step out and
think about something else for a minute…
I am hopeful that his brother finds comfort in one or more of the alternative therapies, just as I am hopeful that the chaplains, the bereavement counseling and other programs are able to help him and his family to cope with what is happening…
I really feel that most of us could use help with this chapter of life, whether it is our own life or that of a loved one.”

HomeCare, Inc.
We are the extra hands to stay independent
HomeCare, Inc. is a trusted partner for communities and the medical community as we help to identify and fill the gaps to properly manage an
aging and vulnerable population. They are part of the hub – the home care team – that sees and works with patients in their own homes; able to
communicate and share the status of health and well-being to the rest of the team.
As a greater percentage of the population is considered “aging” and many have chronic health conditions, our communities have more residents
at risk for medical crisis. Such threats can be minimized or even avoided with established communication, monitoring, and safe living conditions.

A partner in caring for seniors
In 2014, we provided greater support to regional councils on aging,
elder services agencies and other senior services. Our staff are the
trained homecare professionals that ensure clients’ health needs are
met in addition to connecting them with basic necessities such as
heat, electricity, food and other human services. We are the eyes
and ears for elders at home.
The Healthy Connections program initially launched in 2013 has
been a way to provide a safety net to some of the most vulnerable
patients following discharge. Expanding into the Seacoast region
in 2014, this program helps patients avoid complications of their
chronic conditions and avoid preventable hospitalizations.
More than assistance, watchful care
As a partner in the home care of people, we help ensure their safety
as we assist in their independence, comfort and daily living support.
Meeting the needs of an increasing aging population takes a team.
HomeCare, Inc. and Elder Services of the Merrimack Valley are a
great example of how we work in collaboration with partners in our
communities – providing the kind of care we do best coupled with
the reach of elder services to help identify needs. Together, we keep
our population safe and connected to the local resources available.
Trained paraprofessionals of HomeCare, Inc., such as personal care
attendants and even homemakers, help to manage care of people who
may not be receiving medical care at the time. This staff often becomes
the person closest to a senior’s daily life, a trusted set of eyes on
progress. They ensure that medication routines are being followed and
help communicate changes or concerns to family members and/or care
team members. By watching for changes in conditions or identifying
risks, they can alert an agency nurse who can arrange proper
intervention or communicate the need to the person’s physician.
Private Care Services an
integral part of overall care
Our patients receiving non-medical support through Home
Health VNA or Merrimack Valley Hospice are familiar with our
HomeCare, Inc. staff who provide care and support them in the
home. This same team can be called upon to provide additional,
more frequent visits that coordinate and flex with the care plan
in place – so patients receive greater value to their private services.
HomeCare, Inc.’s paraprofessionals are already a part of the
trusted care team and seamlessly enhance care as it is needed.

of the Merrimack Valley in caring for the city’s seniors during a recent visit with President and CEO John Albert
and Executive Director of Elder Services of Merrimack Valley, Rosanne DiStefano.

C ompassion from the community
Benefit Events
We host two major fundraising events annually to help financially
support our ability to provide charity care as well as patient and family
services that are not covered by insurance. The Legacy of Leading,
a dinner event held in April pays tribute to those who support the
health and well-being of our community as well as the mission of
our combined agencies. Our Food, Wine and Beer Pairing and
Silent Auction is a fun-filled event each September featuring a
festive theme-based atmosphere, activities and cuisine along with
a silent auction and raffles to benefit Merrimack Valley Hospice.
Volunteering
Volunteering comes in many forms – as time spent with patients on
hospice and their families, assisting at the Hospice House, helping
at events, sewing keepsake memories for someone who has lost a
loved one, or checking in with patients by phone. Volunteers are
important to our work and help extend the range of support to
the greater community. Some volunteers were helped by Merrimack
Valley Hospice or Home Health VNA and want to help others face
the challenges they overcame. Others are drawn to caring for those
in their community and want to share their gift of time and talent,
where they can see the difference they make in the lives of others.
Volunteers are needed for all regions served including northeastern
Massachusetts, southern New Hampshire and the York, Maine area.
Friends of Merrimack Valley Hospice
This auxiliary group of volunteers was created in 2014 by
a passionate group of individuals touched by the care of
Merrimack Valley Hospice and Merrimack Valley Hospice
House. They help to raise awareness, support events held by
the agency, as well as host their annual Spring Brunch fundraiser
to raise proceeds in support of Merrimack Valley Hospice
House and other services for hospice patients and their families.

2014 Highlights
After diligent effort, strategic management and
financial accountability we can report an overall
surplus in 2014 which ensures a solid foundation
to continue to move forward with innovative care.
This position allows us to better weather the stormy
uncertainties that face heath care organizations
during trying times, without cutting services or
the scope of care.
We face a critical year ahead in the home health
and hospice industry. Steep cuts in state and
federal reimbursement combined with reductions
in insurance payments require that we find more
ways to be more cost effective and yet maintain
our promise of the highest quality of care to our
patients. Your generosity is an investment in our
stability and strength, and will allow us to continue
to stay ahead of the dangers that fiscal challenges
beyond our control may have.

The generosity of our donors enables us to uphold the promise of quality,
compassionate home health and hospice care for all, especially for those in our
community who are without health insurance or the means to pay. It helps to
ensure that we will remain here as a trusted resource, for many years to come.

Thousands of people are touched by the generosity shared with our agencies,
and support can come in many forms. To discuss any of these options or for

Cash Contributions
Cash contributions are fully tax deductible. Many companies match the gifts
of their employees – check with your employer to see if your gift can qualify.
Gifts of Appreciated Property
You can donate appreciated property (stocks, bonds and real estate) to support
any of our agencies. When you donate property that you have held for more
than a year, your gift is exempt from capital gains taxes. Your charitable deduction
is based on the fair market value of the asset at the time of your gift.
Vehicle Donations
Donate for Charity, our recycling partner, offers an opportunity to support
our agencies by donating a car, truck, van, boat, RV, or motorcycle by calling
800-933-5593. Donors receive a tax deduction of the auction sale price of
the vehicle or $500, whichever is the greater amount.
Income-generating Gifts
You can also use cash and appreciated property to establish a charitable
remainder trust or charitable gift annuity that will provide income and tax
benefits to you, while benefiting one of Home Health Foundation’s agencies.
Bequests
A simple way to support is through a bequest in a will or living trust. Any
part, percentage, or component of estate or trust assets can be designated
to Home Health Foundation or one of its agencies and will help to reduce
inheritance taxes.

There are many ways to get involved and give of your time and talents.
