rehabilitation services. Our full time Nurse
Practitioner works closely with your physicians,
nurses and therapists to design and carry out an
individual care plan based on your personal and
health care needs.

highly trained Physical Therapists, Occupational
Therapists, and Speech Language Pathologists
will work with you or your loved one to reach
your highest level of function.
While at our facility, you will receive therapy
in our state of the art Rehabilitation Gym.
On average, our short term residents are able
to return to the community and their normal
way of life after about 20 days.
Should you decide to stay with us long term,
our staff are experts in understanding the
physical and emotional needs of those who live
in our facility for an extended period of time.

Pulmonary
Rehabilitation Program
at FutureCare Canton Harbor
Our Pulmonary Focus
Our short-stay pulmonary rehabilitation
program is designed for patients with chronic
respiratory diseases. Our interdisciplinary team
of physicians, registered nurses, respiratory,
physical, occupational, and speech therapists,
restorative nursing aides, social worker,
psychologist, dietician and pharmacist work
together to provide patients with the tools they
need to be as independent as possible.
Patients with respiratory diseases such as
Chronic Obstructive Pulmonary Disease
(COPD), Cystic Fibrosis, disordered
breathing associated with morbid obesity, and
neuromuscular diseases, as well as patients
preparing for – or recovering from surgery, can
benefit from our individualized pulmonary
program. Program oversight is provided by our
consulting Pulmonologist.
