This project is part of the Healthy Communities Initiative called Project 2020, implemented in 2009.
Educational programs of the Texas A&M AgriLife Extension Service are open to all people without regard to race, color, sex, disability, religion, age, or

INTRODUCTION
The Texas A&M AgriLife Extension Service
has been dedicated to educating Texans for
nearly a century. In 1915, the agency was
established under the federal Smith-Lever Act
to deliver university knowledge and agricultural
research findings directly to the people. Ever
since, AgriLife Extension programs have
addressed the emerging issues of the day,
serving diverse populations across the state.
Through a well-organized network of
professional educators and some 100,000
trained volunteers, Extension delivers practical
research-based knowledge to Texans in all 254
counties. Our expertise and educational
outreach pertain to the food and fiber industry,
natural resources, family and consumer
sciences, nutrition and health, and community
economic development. Among those served
are the hundreds of thousands of young people
who benefit annually from Extension’s 4-H and
youth development programs.
Texans turn to Extension for solutions. Its
agents and specialists respond not only with
answers, but also with resources and services
that result in significant returns on investment
to boost the Texas economy. The agency
custom-designs its programs to each region of
the state, relying on residents for input and for
help with program delivery.
PURPOSE
The purpose of this resource manual is to
provide service providers with a resource
directory that focuses on services and
assistance for Liberty County residents.
Texas A&M AgriLife Extension Service is most
noted for the community involvement,
leadership and development of educational
programs and quality research based
information that is available for Liberty County
residents through the local Extension Service
Office. With limited resources Extension staff
relies on input from the local communities and
volunteers. One of the most significant efforts
of coordinating and collaborating with partners
in the community is through formation of
grassroots community organizations.
The Family & Consumer Sciences Advisory
Board is a program area committee that guides
the County Extension Agent(s) in the process
of needs identification and program planning,
marketing, implementation and evaluation.
The mission of the Family & Consumer
Sciences Advisory Board is to maximize the
efforts in the community and offer high-quality,
relevant information through education and
resources to improve the lives of people in
Liberty County.
Through the program area of Family &
Consumer Sciences, resources and education
efforts promote Healthy Families through the
following objectives:
- eat well, stay healthy, manage money,
and raise their children to be successful
adults.
- efficiently help themselves through
preventing problems and using tools for
economic stability and security.
- improve stewardship of the environment
and of the state's natural resources.
Project 2020:
Healthy Community Initiative
Project 2020 was initiated in 2009 by the
Family & Consumer Sciences (FCS) Advisory
Board, whose members are key stakeholders
and Extension volunteers in Liberty County.
The board has laid the foundation for Project
2020 to strengthen families in Liberty County
by year 2020.
Project 2020:

History
The Family & Consumer Sciences Advisory Board hosted a Community Forum at the end of 2009 to
develop a process to identify current resources and current needs of the communities within the
county.
The board created a community survey and distributed both the printed and electronic version in
2011. Project 2020 and the community survey in English and Spanish was promoted on the local
radio station, in fairs and educational events, area churches, and more.
A Leadership Breakfast was held to interpret the results of the survey which communicates what the
community thinks we need. Presentations made by board members highlighted activities.

86% of the participants believe they feel safe in their neighborhoods/community.
1/3 of the participants are unsatisfied with the local health and medical services.
58% of the participants are aware of the 211, but only 18% have ever utilized the service.
Many expressed concerns related to having a safe and engaging place for teens, workforce
training opportunities, and accessibility to local healthcare services, and generational
poverty.

• A case manager will assess the needs of the consumers, and from that assessment will develop a
treatment plan with other programs to address those needs. Case managers link the consumer to the
various programs and resources that are available to them. Case managers will then determine if the
services are being offered in a timely fashion, if they are the appropriate services, etc. Serving people
with mental retardation with onset before age of 18 and priority population of mentally ill persons
with severe and persistent mental illness. These services include crises individuals and family support
which are intended to provide assistance to people with mental illness or mental retardation in the
community. Case management/service assists in coordinating services by other agencies in the private
sectors to coordinate available services.

• Serves eligible adults who have a diagnosis of Schizophrenia, Major Depression, and Manic-Depressive
Disorder. A diagnosis other than the priority population, and except for a sole diagnosis of substance
abuse will need to be accompanied by a GAF of less than 50 in order to be screened for Case
Management Services. Persons diagnosed with mental retardation are eligible for Case Management.
OBRA provides services to eligible individuals who reside in nursing homes. All consumers served live in
Montgomery, Liberty or Walker counties. Referrals are received from our access staff.

The County Judge is both the presiding officer of
the Commissioners Court and the Judge of the

The Liberty County Attorney's Office is a multifaceted operation that handles many diverse
aspects of law. The County Attorney's Office is
comprised of three attorneys and seven support
staff. The County Attorney's office not only
prosecutes Class "A" and "B" misdemeanors, it
also handles juvenile prosecution as well as
representing the State in Justice Court cases. The
office also represents the Texas Department of
Protective and Regulatory Services in CPS and APS
cases as well as handling Mental and Chemical
Dependency Commitments for the County. The
County Attorney's Office provides legal advice to
the Liberty County Commissioner's Court
regarding the Local Government Code as well as
assisting department heads in the County who
need legal advice. The office assists victims of
domestic abuse with protective orders and
information regarding the crime victims
compensation program. The County Attorney's
office collects hundred of thousands of dollars for
local merchants and individuals in our Hot Check
Department. The Liberty County Attorney's Office
is service-oriented and tries to assist "all the
people all the time."

Liberty County Treasurer
The job of the County Treasurer is to serve as the
chief custodian of county finance. Duties include
receiving, depositing, applying and disbursing all
monies belonging to the county. Additional duties
include: correspondent for social security; county
retirement (TCDRS); credit union; unemployment;
workers compensation; deferred compensation;
and supplemental insurance. The County Treasurer
is the payroll officer for the county charged with
maximizing the yield and minimizing the fees on
the county's investments; personnel/human
resources officer of the county; and
liaison/member of Board of Trustees for the Liberty
County Group Benefits Pool for partially funded
health, dental, and life insurance.

The District Attorney's office is responsible for
the prosecution of felony level crimes and
misdemeanors over which district courts have
jurisdiction. In Liberty County, the District
Attorney's Office represents the State of Texas in
the 75th and 253rd Judicial District Courts. Our
staff includes four assistant district attorneys, three
investigators (one of whose primary duty is as a
D.A.R.E. Officer), and support staff. We strive to
handle each case equally, thoroughly, and mindful
of our duty to the citizens of Liberty County.
