From the President’s Desk...
Greetings, fellow physicians and members. It
continues to give me great pleasure to be the
president of the Medical Society of the County
of Queens. As someone who was born and
raised in Queens, it is especially significant
for me to be given the honor of leading our
organization. This achievement is something
I have aspired to obtain for many years.
My goals and aspirations for this and the
rest of my year as the president are many,
but above all I continue to focus on increasing
our membership by mobilizing young physicians. I believe that by reaching out to this
new generation of professionals and informing them of all of the opportunities we have
to offer, and creating social networks, we can
become a stronger and even more influential
organization.
I want to congratulate Dr. Robert L. Kraft on a
great Past Presidents’ Gala and Mr. Stephen S.
Mills, FACHE, President, CEO of New York Hospital
Queens, the recipient of our Community Service
Award. I also want to take this opportunity to
thank all our gala guests, and most importantly
our sponsors; we would not be able to do
what we do without your continued financial
presence.
Congratulations are also extended to our
elected candidates in this past election of the
New York State MSSNYPAC.
In addition, I want to continue meeting with
the rest of the medical staff societies of the
remaining hospitals in Queens to offer our
institutional membership. I hope to reach out to
our present members to seek those that might
be interested in working on a committee or are
interested in getting more
involved with the society.
Also, we have added more
interesting and informative
lectures to our monthly
Comitia meetings.
Finally, I would like to rejuvenate our
legislative agenda. The healthcare system has
undergone many changes in recent years, and
continues to evolve rapidly. It has become
clear that we as physicians can no longer sit
on the sidelines and allow others to dictate
how we practice medicine. We will continue
to be impacted by many things beyond our
control: from Washington to Albany, from Big
Insurance, to the exchanges, to no insurance,
with tighter regulations and more hassle, you
name it.
The challenge for me, as your President, is to
steer the Medical Society of the County of
Queens in a way that does not fear change
and protects what is in the best interests of
patient care.
As a relevant organization, which I know we are,
we will need to embrace the new paradigms
we face, and not give away things without
reason just for the sake of change. We are a
tough organization with excellent and experienced leadership: my aim is to keep it that
way. I am honored to accept the challenges
of this position and continue to ask for your
support and encouragement.

New York's
e-Prescribing Mandate:
Will You Be Ready?
Advanced Data Systems Corporation
Starting March 27, 2015 all
New York state-based prescribers other
than veterinarians will be required to
electronically transmit all prescriptions
(e-Rx) through a certified system directly
to patients’ pharmacies.
Paper prescriptions will no longer
be accepted.
Additionally, New York state-based prescribers who issue prescriptions for
controlled substances will also be required to register with the NYS Department of Health, Bureau of Narcotics.
As part of registration, prescribers will
be required to name the certified system they are using along with the vendor’s name and the system’s version
number.
If a prescriber never issues a controlled
substance prescription, that prescriber
will not need to register.
However, with very commonly prescribed medications such as Vicodin™,
Ambient™ and Xanax™, we think all
prescribers will need to register.
In any case, all prescribers will need a
certified system for e-Rx of their noncontrolled substances as well.
Controlled substance registrations
are per-prescriber… not per-practice,
meaning that each individual prescriber
at the practice will need to register
individually.
e-Rx Penalty
The penalty for not complying with the
mandate is obvious: New York statebased prescribers will not be able to
issue (write) any prescriptions as of
3/27/15. They will be “shut down” as
prescribers unless a certified system is
used for e-Rx. Other unpleasant penalties may also apply.

New York is the first state to require

Keep Watch on Your
Electronic Health

Question:
I saw something online indicating that
there may be something in the new
federal budget that could affect my
Electronic Health Record (“EHR”) system.
Do I need to be concerned?
Answer:
The 2015 Omnibus Appropriations bill
just passed by the federal Congress
may have a significant effect on the
number of Certified Electronic Health
Record Technologies (“CEHRT”) available to physicians and hospitals. The
bill specifically instructs the Office of
the National Coordinator for Health Information Technology (the “ONC”) to
certify only those new products which
do not block the exchange of health information and to take affirmative steps
to de-certify any current CEHRT that in
any way blocks the exchange of health
information.
The basis for this directive is that any

EHR that actively blocks the exchange
of health information frustrates the
congressional intent behind nationwide implementation of EHR. Many
CEHRT block the exchange of information generated by their EHR products
with competitive EHR products. Under
this new directive, CEHRT that continue
to block data exchange in this way will
be at risk of losing their certification
status.
The ONC must submit a report
within 90 days of the enactment of
the Appropriations bill by Congress
detailing the extent of the information
blocking. Thereafter, a secondary report
will be submitted within 12 months
detailing the challenges and barriers
to interoperability between CEHRT. All
physicians and eligible professionals
should be sure to regularly confirm
the certification of their EHR as this
initiative progresses. If your CEHRT
is decertified, you will be unable to
continue participating in the meaningful use incentive program.

The Role of Sugar in
Cardiometabolic
Health

The WroResearch on the potential
benefits of sodium reduction for treatment of hypertension has yielded
conflicting results, with some evidence
suggesting that reducing sodium intake could actually increase cardiovascular and all-cause mortality in patients
with diabetes. A review in the journal
Open Heart examines the role of sugar,
not salt, in cardiometabolic health, as
up to 80% of individuals with “essential”
hypertension also exhibit insulin resist-

intake of about 200g.
The American Heart Association currently recommends no more than six
teaspoons of sugar per day for women
and no more than nine for men, but
does not make specific recommendations regarding added sugars and hy-
pertension. Because
added sugars may
increase cardiovascular risk by inciting
metabolic dysfunction and increasing
blood pressure variability, myocardial
oxygen demand, heart rate, and inflammation, the authors recommend
a greater emphasis on added sugar
reduction in hypertension and cardiovascular disease guidelines.

Sleep Specialist
Advises Parents When
to Seek Help for Kids’
Sleep Disturbances
Every expectant parent knows that with
the arrival of a new baby an uninterrupted
night's sleep will become a longed for and
distant memory, at least for some period
of time. But while it's normal for a twomonth-old to awaken frequently during
the night, it isn't normal for a two-year old.
Parents bring this and many other concerns
about sleep behaviors to pediatricians
and seek advice in books and online on
how to develop healthy sleep habits in
their children. “Eventually, most children
outgrow frequent waking during the night
and resistance at nap and bedtime. But
there are more serious sleep disturbances
that if uncorrected can seriously affect the
child's health and happiness and upset the
family's equilibrium,” says sleep specialist
Gerald D. Suh, MD.
Dr. Suh recommends that parents consult
their pediatrician or primary physician for
children who exhibit one or more of these
symptoms frequently or for an extended
period:
• Snoring, snorting, choking and
gasping for breath
• Breathing with an open mouth or
sleeping with the neck extended
• Sleepwalking and/or sleep-talking
• Night terrors or nightmares
• Unusual or excessive movements
during sleep
• Trouble falling asleep or staying
asleep
• Persistent bed wetting
• Excessive sleepiness during the day
• Daytime behavioral issues such as
difficulty concentrating, aggression
or hyperactivity

The physician will take a complete history
and may recommend a sleep study to
pinpoint the cause of the disturbance.
Pediatric sleep disorders cover a wide
range. Primary disorders have a physiological basis and while some may be outgrown, others require medical intervention. Secondary disorders, such as
night awakening and bedtime resistance,
are more often behavioral in nature and
can generally be overcome with appropriate parental attention.
Common primary pediatric
sleep disorders
Obstructive sleep apnea is a disorder in
which breathing is repeatedly interrupted
during sleep due to obstruction in the
upper airway. With air unable to get to the
lungs, the oxygen level in the blood drops
and when the amount of oxygen reaching
the brain decreases, the brain signals the
body to wake up and take a breath. In children, the airway is most often obstructed
by the tonsils and adenoids and removing
them brings significant relief in 85% of
cases. Obstructive sleep apnea affects 25% of all children and is most common in
those with facial abnormalities, chromosomal disorders (such as Down syndrome)
and obesity. A sleep study is generally
needed for a definitive diagnosis. Parasomnias are disorders that are characterized by unusual behaviors during sleep.
Among the most common in children are
sleepwalking and night terrors, which usually occur in children between the ages of
four and twelve. Parasomnias appear to
be reflective of immaturity of the central
nervous system, which allows the intrusion
of wakefulness into various stages of sleep.
They tend to decrease in frequency and
intensity as children get older and disappear over time without treatment.

bed. Children generally have no recollection of sleepwalking. Sleepwalking is
harmless except with respect to injury. The
child's room should be free of obstructions and should have a bell or alarm that
alerts parents to a wandering child.
• Night terrors differ from nightmares,
which are frightening dreams that awaken
a sleeping child who may have vivid
memories of the dream. Night terrors are
scarier to parents than to the child who,
while apparently agitated and terrified, is
asleep and will have no memory of the
event. It is generally best not to intervene
or try to calm or awaken the child, since he
or she will likely return to restful sleep after
just a few minutes.
Other prevalent sleep disorders in children include bed wetting (after the age of
five); confusional arousal, in which the
child exhibits confusion during or after
arousal from sleep; periodic leg movements; and narcolepsy, in which the child
falls asleep suddenly during the day.
“Sleep disturbances are worrisome for
parents and some are potentially harmful
for children,” Dr. Suh concludes. “Sufficient
sleep and regular sleep habits are critical
to a child's health and well-being. We now
have a greater understanding about sleep
and how it occurs in children and help is
available for most of these problems.”

w Peripheral Arterial Disease Test
Discomfort in the lower extremities can
be caused by bad circulation or a blood
clot in the arteries. Unlike other tests,
this PAD test measures the ABI, TBI
(toes), PVR, as Segmental Pressures
along the entire leg.
