If your child is not currently enrolled in a CCS school, you must complete an enrollment packet, and take the
packet, required documentation (on CCS website under Enrollment) and this lottery application to the Central
Enrollment Center, located at 430 Cleveland Ave., Columbus, OH 43215 to apply.
If your child already attends a CCS school, you may apply for a different school with this application through the School
Choice Office. For information: CCS website: www.ccsoh.us Click on Parents then School Choice/ Lottery
USE THIS APPLICATION IF YOU ARE:
A CCS resident and you want your child to attend a school other than the school assigned for 2014-2015.
(Non-CCS residents must use the Interdistrict Open Enrollment form and are not included in lottery draws).
A CCS resident who previously accepted a school through School Choice and now wish to apply for a
different school, including the home school by address.
APPLICATION PROCESS
Please read carefully. Some procedures are new this year.
You may apply for up to 3 middle schools on the attached form.
Be sure to apply for the correct grade level.
Application Deadline:
Applying for an incorrect grade level may disqualify the application!
April 11, 2014, to be included in the electronic lottery drawings.

to 365-7468 or by first-class mail addressed to the School Choice Office, 430 Cleveland
Ave., Columbus, OH 43215.
NOTIFICATION OF LOTTERY RESULTS
Results from the electronic middle school lottery drawings will be mailed to parents in June. On or after June
16, parents may also call the FACTline (221-3228) for results M-F from 9:00 a.m. to 3:00 p.m. Subsequent offers will
be made through calls to parents using phone numbers supplied on the application.
RESPONSE TO AN OFFER
If you are offered placement through a result letter, mark your decision by each school’s name on the letter, sign
and return it to the School Choice Office as soon as possible, but no later than the deadline listed in the result
letter. If a response is not received by the deadline, the application will be cancelled and the next student on the
waiting list will be offered the placement. Generally, ten days are given for response to subsequent offers.
Accepting an offer for a school cancels all other applications. The seat at the previous or home school will be
offered to another student. If you decline an offer and are on waiting lists for your other choices, those waiting list
applications remain active.
.If you change your mind about an acceptance, you must reapply for another placement including the student’s
home school by address. Your child is committed to the original accepted school until a new placement is approved.
If not placed in a school of choice by the first day of school, the original assignment remains valid.
Students should attend the assigned school on the first day of classes.
INTERDISTRICT OPEN ENROLLMENT
Those students who live outside of CCS boundaries must apply using an Interdistrict Open Enrollment (IOE) form,
available on the CCS website or at the School Choice Office. A form must be filed every school year. Lottery
applications for CCS residents will be processed first. Transportation is the responsibility of the parent.

While all middle schools use the same curriculum, alternative schools offer a specific instructional focus. Check the

Arts IMPACT

The Arts IMPACT MS provides a nurturing atmosphere where academic study is enhanced by its integration with music, dance, visual arts and drama.
Meaningful connections throughout the curriculum will allow students to use multiple ways of learning to promote academic excellence. Art, music,
dance and drama will be taught as separate core subjects, as related arts and as points of connection with the academics. Commitment to the program,
both academically and artistically, is expected of all students, parents and staff. Students must abide by our school dress code which requires pupils to
wear collared shirts and pants other than denim jeans.

Students learn through African-centered holistic perspectives and beliefs. The K-12 staff uses smart board technology to enhance learning. Students can
take college-level courses beginning in 9th grade and earn an associates transfer degree in arts or science from Columbus State Community College.
Problem based learning strategies are utilized for students to become critical thinkers and problem solvers. CAEC serves students K-12th grade, but the
building is divided into separate elementary, middle and high school programs. Students must be in uniforms daily.
Columbus Spanish Immersion Academy

In this K-6 school, students learn to speak Spanish naturally by hearing it used as the language of instruction and in their regular activities. After several
years in this program, students are expected to speak, read and write fluently in Spanish and English. Because most classes are taught in Spanish, any
middle school applicant must be fluent in Spanish and must be approved by a review panel. Seventh and eighth grade students will attend school in the
Columbus International 7-12 school building at 100 Arcadia Ave. where they will have the opportunity to enjoy a full internationally enhanced
curriculum including academics, activities and athletics.

The new Hubbard regional lottery school will focus on mastery learning using personalized curriculum. Students will work in flexible groups based on
their demonstrated understanding of academic standards. Hubbard will have an international focus, incorporate Mandarin language instruction, and is
part of the International High School feeder pattern.

Through the Informal Education program, student reading is taught using a balanced literacy approach. The informal model of instruction is taught
through cooperative learning and features multi-age groupings, and an integrated curriculum with a full-time arts program. The program builds on
student interests and abilities and promotes communication, problem-solving and critical thinking skills. Responsibility, self-direction and self-discipline
are emphasized as important goals for life-long learning.

Students in this program learn a number of scientific skills including observing, describing, comparing, classifying, measuring, and interpreting evidence
through hands-on experiences and themes. Math and science are taught separately, as well as part of other subject areas. The schools’ focus on
environmental studies teaches students to experience the natural and man-made environment, particularly in an urban setting and will include overnight
camping.

The vision of Columbus City Preparatory School for Girls (CCPSG) is to prepare and nurture young ladies through a rigorous college preparatory
curriculum, supported by an engaging fine arts program, to enter into high school programs with high academic skills and confidence. CCPSG’s goal is
to foster a learning environment that focuses on how girls learn, grow and lead. Students must read a minimum of 3 books over the summer and complete
a summer learning packet of reading and math lessons. All CCPSG students will dress in mandatory school uniforms, which are expected to be worn
starting the first day of school. New parents and students are expected to attend the orientation in the spring.

The four STEM academies feature an innovative approach to learning that helps children focus on science, technology, engineering and math study
areas using hands-on learning, 21st century technology, student projects and presentations. Students will automatically transition to Linden
McKinley STEM for grades 7-12 which is located in the newly renovated building at 1320 Duxberry Avenue, specifically designed for STEM
learning, with state of the art Science labs and Special Ed rooms, A/C, two new gymnasiums, plus new weight training and sport facilities. The
STEM methodology will make it easier for students to enter college, win scholarships and secure high paying employment.
The following are conventional Columbus City Middle Schools, listed by the community in which they are located. Conventional middle schools
use the same curriculum as the alternative schools but do so under a more traditional organization and focus. Each school has it own unique
features, activities and honors. Parents are encouraged to visit the schools, especially if it is the student’s home school by residential address, learn

Any seats unfilled after initial offers for conventional schools will be offered to siblings on the waitlist in the order they
were drawn. Seats remaining after sibling placements will be offered to individuals remaining on the waitlist.
Seats remaining at alternative schools after the initial lottery will be offered in an alternating order between the sibling
waitlist and the non-sibling waitlist. Thus, the first seat on the sibling waitlist will be offered, the next will be non-sibling
waitlist, the third from the sibling waitlist, and so forth.

Eligibility to apply for specific middle schools

School Choice Lottery: students may apply for any conventional middle school, any citywide alternative or any
regional alternative available to your community. Transportation to school would be the responsibility of the parent
unless the student met the eligibility criteria for transportation.
Eligibility for Transportation

CCS will provide transportation to the home school based on address, any conventional school in your community or
the Center City community, or to any alternative school to which the student is eligible to apply, provided that the
student lives more than 2 miles from the school.
Disciplinary Transfers

A student who is transferred for disciplinary reasons from any school may not reapply to that school. Students
expelled through the Hearing Office for a period of 10 or more consecutive days during the current school year may
not access the lottery process for conventional schools for the 2014-2015 school year.

If your child is NOT a current CCS student, personally deliver application, along with completed enrollment packet and enrollment
documentation to the CCS Central Enrollment Center located at 430 Cleveland Avenue, Columbus, OH 43215.
Enrollment documentation includes: Parent photo ID, birth certificate, immunization records, address verification, IEP (if applicable),
proof of custody/guardianship (if applicable).
If your child is a current CCS student, personally deliver, fax to 365-7468 or mail applications to the Columbus City Schools’
School Choice Office, 430 Cleveland Avenue, Columbus, OH 43215. If faxing application, please copy on white paper before
faxing. DO NOT turn in applications at a Columbus school as there is no guarantee they will be forwarded to the School Choice
Office by the April 11, 2014 deadline.
Lottery applications received or postmarked after April 11, 2014 will be date-and-time stamped and entered at the end of each
school’s waiting list in the order received. Late applications will not be processed until late spring. Filing your application prior to the
deadline is strongly encouraged.
Special Education and English as a Second Language (ESL)
It is recommended that parents of students with disabilities and students with limited English proficiency who are interested in
submitting an application for an alternative or conventional school first visit the school. Students with disabilities will be afforded an
equal opportunity to attend the schools of their choice through the lottery. For students with disabilities, the services to be provided at
the school will be determined by the IEP team.
