Exagen Genomic Technology
Exagen Diagnostics understands that most
diseases are complex, and that the closer you
get to the cause of the disease, the quicker you
can identify and treat it. We have a proprietary
discovery engine, Coperna™, that enables us to
look simultaneously at all of the 33,000 genes
that make people who they are.
We search for combinations of small sets of
genes that are associated with a disease
process, resulting in a “gene signature” that
can be developed into a molecular test to
provide prognostic, diagnostic, treatment, or
monitoring information for physicians to
better manage their patients.
How Exagen Gene Expression Tests Differ from Antibody Tests

Technology
Introduction
Genomic technology is new—
developed in the last 5-7 years.
Based on 18-20 year old
technology
Data Source
Genome wide, analyzing tens of
thousands of marker
combinations
Limited number of markers that
are each fairly insensitive, but
with improved diagnostic
performance when combined.
Analytes
Gene expression measurements
reflect the dynamic
communication of interacting
biological systems
Antibody levels remain fairly
static after exposure to an
antigen; exposure may have
occurred weeks or months prior
to measurement
Biological
Responses
Gene expression levels reflect
physiological changes in a tissue
or an organism
Antibodies develop in response to
a single specific antigen
Test
Measurement
Genomic tests measure the RNA
levels of specific genes. They
quantitate gene expression
activity in patients with active
disease.
Antibodies detect target proteins,
which can be modified, and the
same protein can be different in
each patient.
Test Result
Single test result that identifies
patients with disease
Multiple test results interpreted
together to improve the prediction
that a patient has disease
Test
Performance
High sensitivity and accuracy
provide a tool to identify patients
that are positive for the disease
Multiple sensitivities and
accuracies need to be considered
together for an evaluation

PCR Technology
Real-Time PCR Data
Exagen Gene Expression tests use a
patient’s whole blood shipped at room
temperature. All the cells in the blood are
lysed without a prior white cell preparation
step; the mRNA is extracted from the total
nucleic acid. cDNA is made from the mRNA
to stabilize and finalize the sample for
testing. The patient’s cDNA is amplified
using a technique called Real Time PCR, and
an expression index is calculated based on
the expression of the genes measured. A
test result is generated: Patients with an
expression index >0 have a positive test
result while patients with an expression
index ≤0 have a negative test result.
Patient focused. Discovery driven.
