What You Should Expect
The photographer will deliver to you a proof sheet of four poses and your order
form. Please verify and initial that the birth information printed on your proof sheet
is correct.
Please complete the customer information section on the Portrait Order Form and
review the catalog to familiarize yourself with our packages and gift options.
Orders may be placed by bringing the completed order form to the HMH Gift Shop,
located on the first floor. Remember to choose one pose per package. We accept
cash, check, Visa, MasterCard, American Express and Discover. Checks should be
made payable to Hardin Memorial Hospital Auxiliary with “Baby Photos” in the
memo section.
Once you have placed your order, photos will be brought to your room or mailed to
your home (if you have been discharged) within one business day. You will receive
them on full, uncut sheets.
Website Posting
Your baby’s photo can be posted on hmh.net’s “BirthPlace Online Nursery” website.
The web postings are free and will be posted to the server with your written
permission.
After You Leave The Hospital
You may choose to take your proof sheet, order form, and catalog home with you
and place an order at a later time.
To place a credit card order from home call 270-706-1714. Please have the baby id
number (located on proof sheet) when placing phone orders.
Or, mail the completed order form with a check, money order, or credit card payment
to the address listed below. A small shipping fee will be added to all orders placed
after discharge.
