Your membership is simple and easy to use. You may begin saving money as soon as you receive this
membership packet. It is quick and easy to locate participating providers in your area by using the toll-free
number listed in your membership literature or the attached provider lists. Simply present your membership
card at provider locations to receive immediate savings. Refer to this booklet for complete information on the
use of each service.
We believe in customer satisfaction. Please keep the Plan Membership booklet in a safe place for future
reference. Put your membership cards in your wallet so they are with you at all times. This will help you if you
need to call us for information.

Terms and Conditions of Your Membership
This discount health care card is NOT insurance.
Dorsey Discount Services is not available in WA, VT and MT.
RENEWAL CONDITIONS: By joining a plan, you are
authorizing Dorsey Discount Services to bill your
credit card or checking account for the plan you have
selected. This charge shall remain in force until you
notify Dorsey Discount Services of request to cancel.
By joining, you indicate you have read the terms and
conditions of the plan. This plan will automatically
renew at the end of your membership term on an
annual basis, and your credit card or bank account
will be automatically charged or drafted for the
appropriate amount.
Termination Conditions: Dorsey Discount Services
and Careington reserves the right to terminate
plan members from its plan for any reason,
including non-payment.
Cancellation Conditions: You have the right to
cancel within the first 30 days from the date you join
the plan to use the plan risk-free.
FL Residents: You have the right to cancel within
the first 30 days after effective date. If for any
reason during this time period you are dissatisfied
with the plan and wish to cancel and obtain a refund,
you must submit a written cancellation request.
Dorsey Discount Services will accept and cancel
plan memberships at any time during the
membership period and will cease collecting
membership fees in a reasonable amount of
time, but no later than 30 days after receiving a
cancellation notice. Please send a cancellation
letter and a request for refund with your name
and member number to Dorsey Discount Services,

cancellation by email: [email protected].
If Dorsey Discount Services is billing you quarterly,
semi-annually or annually, Dorsey Discount Services

will, in the event of cancellation of the membership
by either party, make a pro-rata reimbursement
of the periodic charges to the member.
Description of Services: Please see the enclosed
materials for a specific description of the programs
that you have purchased.
Limitations, Exclusions & Exceptions: Savings
are based upon the provider’s normal fees.
Actual savings will vary depending upon location
and specific services or products purchased. Please
verify such services with each individual provider.
The discounts contained herein may not be used in
conjunction with any other discount plan or program.
All listed or quoted prices are current prices by
participating providers and subject to change
without notice. Any procedures performed by a
non-participating provider are not discounted.
From time to time, certain providers may offer
products or services to the general public at prices
lower than the discounted prices available through
this program. In such event, members will be
charged the lowest price. Discounts on professional
services are not available where prohibited by law.
This plan does not discount all procedures.
Providers are subject to change without notice
and services may vary in some states. It is the
member’s responsibility to verify that the provider
is a participant in the plan. At any time Dorsey
Discount Services has the right to eliminate a

Provider networks at its sole discretion. Dorsey
Discount Services cannot guarantee the continued
participation of any provider. If he or she leaves
the plan, you will need to select another provider.

and treatment rendered to members and Dorsey
Discount Services disclaims any liability with respect
to such matters. Services and service providers may
change or be discontinued at anytime with notice as
required by law.
Complaint Procedure: If you would like to file
a complaint or grievance regarding your plan
membership, you must submit your grievance
in writing to: Dorsey Discount Services,
14413 Cornerstone Village Drive, Houston,Texas
77014. All complaints or grievances are documented
in the monthly Quality Assurance log along with
the date and content of the complaint or grievance.
Members have the right to request an appeal of
the complaint and grievance resolution. Appeals
will be sent to the Committee and will be entitled
to a second review with different individuals. After
completing the complaint resolution process and
you remain dissatisfied, you may contact your state
insurance department.
TX Residents: All complaints will be completed
within 72 hours of receipt with the exception of
billing inquiries that require further research or
documentation.
Disclosures: THIS PLAN IS NOT INSURANCE. THIS IS
NOT A MEDICARE PRESCRIPTION DRUG PLAN.
* This plan does not meet the minimum creditable
coverage requirements under M.G.L. c. 111M and
956 CMR 5.00. The plan provides discounts at
certain health care providers for medical services.
The range of discounts will vary depending on the
type of provider and service. The plan does not
make payments directly to the providers of medical
services. Plan members are obligated to pay for all
health care services but will receive a discount from
those health care providers who have contracted
with the discount medical plan organization. You may
access a list of participating health care providers at
www.wellnessshield.com or dorseydiscountservices.com.
Upon request the plan will make available a written
list of participating health care providers. You have the
right to cancel within the first 30 days after receipt of
membership materials and receive a full refund, less a
nominal processing fee (nominal fee for MD residents

have no liability for providing or guaranteeing service

What Is CallMD?
CallMD is a national network of medical doctors
available for consultation and may write a prescription
for a non-narcotic or non-controlled medication at any
time day or night.* CallMD is staffed with registered
nurses to speak to members regarding their medical
needs and to arrange consultations with CallMD
doctors 24 hours per day/7 days per week at no charge
up to 12 calls. All additional calls are at a reduced rate
of $35.00 per call.

1.'JMF.% A personal health profile accessible via
a secure line over the internet. When a member
contacts the Physician Access Hotline for the
first time, a registered nurse asks the member to
provide a full medical history. This history is stored
via FileMD, which is HIPAA-compliant, and creates
an Electronic Medical Record. This gives members
the convenience of releasing medical data,

updating information, and storing documents
(eg. Insurance Cards, Consent Forms, Power of
Attorneys, Living Wills, etc.)
2. 'BNJMZ,FZT A personal data organizer software
program that allows the member to conveniently
organize, record, and store financial, health
and other vital personal information without
compromising privacy. This program downloads
onto the member’s computer in a “notebook”
user-friendly format for easy access and updates.
Member can store data such as family health
records (immunizations, surgeries, genealogy,
etc), which help with the transition of a new
doctor or updating your current doctors records.
Another feature of Family keys is the financial
tool. Members can organize financial services
accounts, insurance policies, retirement accounts,
and includes a financial calculator to amortize
mortgages, personal loans, interest rates, etc.

1. Coordination of Products: Dorsey Discount Services is NOT an insurance company or dental HMO. The Dorsey services are designed to supplement the
exclusions and limitations of dental insurance policies. This means that you can use Dorsey once you have reached your maximum limit on your dental
insurance policy. Dorsey can also be used for dental procedures not covered by your dental insurance. It is the Dorsey policy not to submit Dorsey fees
to insurance companies.
2. Missed Appointments without 24-hour notice: Too many missed appointments may result in cancellation of membership. A missed appointment
charge will be made of $10 per 15 minutes with a maximum charge of $40.
3. Specialists’ charges: Specialists’ charges for oral surgery, periodontics (gum problems), and pedodontics (children’s specialist) are discounted 20% - 25%
off normal fees. Endodontics (root canals) are discounted 20% off normal fees. Orthodontics by Orthodontists are discounted 25% - 30%. This includes
the braces, records, initial exams, x-rays, casts, consultation and retainers, etc. Not all specialists are available in every area. Call 1-800-880-4343 for
specialists near you.
5. Your Dorsey membership may be cancelled if your account with the provider is not paid in a timely manner.
