Executive Summary
In Arkansas, more than one-third of
children and adolescents are overweight
or obese. Children are now being
diagnosed with chronic diseases
typically found in adults, and the cost of
health care continues to increase.
Children who are overweight or obese
and have associated chronic conditions
have annual health care expenditures
up to 32.2 percent higher than children
who are of healthy weight.1 It is
estimated that this costs the health care
system $14.1 billion annually in direct
medical costs associated with childhood
obesity.2
To combat this issue the Arkansas General Assembly passed Act 1220 in 2003.3 This landmark
legislation called for the creation of the Child Health Advisory Committee (CHAC) to make
recommendations to the Board of Education and Board of Health related to nutrition and
physical activity in the schools. The Act also required improved access to healthier foods and
beverages in schools, the creation of local committees to promote physical activity and
nutrition, and confidential reporting of each student’s body mass index (BMI) to his or her
parents annually.
The following report details the results of statewide student BMI data collection, including
weight classifications and BMI assessment participation rates for the past four years. The results
from the 2013-2014 school year show that together we have continued to halt progression of
childhood obesity statewide. Although our work over the past decade has produced positive
changes in schools and communities across the state, continued efforts are needed to improve
the health of Arkansas children.

Introduction
The Arkansas General Assembly
passed Act 1220 of 20033, enacting
the first state-level legislation in
the country to address the
alarming rates of childhood
obesity. The Act is aimed at
making changes within schools and
school districts that encourage
healthier lifestyles for students,
school staff, and their families.4
Components of the Act include:

Creation of the Child Health Advisory Committee (CHAC) to develop nutrition and
physical activity standards and make policy recommendations to the Arkansas Board of
Education and Board of Health;

Requiring each public school district in Arkansas to establish and convene a Nutrition
and Physical Activity Advisory Committee;

Requiring the Arkansas Department of Health, in collaboration with the Arkansas
Department of Education, to employ staff with experience in community health
promotion to assist in the implementation of Act 1220;

Prohibiting elementary school students in-school access to vending machines offering
food and beverages;

Requiring schools to include, as part of the annual report to parents and the community,
the amount and specific sources of funds received and expenditures made from
competitive food and beverage contracts; and

Assessment of body mass index (BMI) for all public-school students and reporting of
results and associated health risks annually to parents, regardless of BMI classification.
In 2007, Act 2015 was passed, which altered the BMI screening requirements. Originally, the
legislation mandated that all Arkansas schoolchildren be assessed on an annual basis. Under the
new requirements, only youth in even grades, excluding grade 12, are assessed yearly.
Furthermore, parents are offered the option to choose that their child not be screened.6
Additional legislation was passed in 2007 (Act 3177) that changed requirements for physical
activity and physical education. First, it reduced the required 150 minutes of physical activity per

week to only 90 minutes of physical activity in grades K-6, while maintaining the physical
education requirements of 60 minutes per week. While children in the fifth and sixth grades had
the same requirements for physical education, no additional requirement for physical activity
was included. Changes also included reducing high school student requirements to one-half unit
of physical education to graduate, with no additional requirements of physical activity. Of the
states with general physical activity requirements, Arkansas has one of the lowest minimum
requirements for physical activity in the nation.8
Arkansas was the first state to enact legislation that directly addressed childhood obesity,
including the BMI assessment and data collection. Since the pivotal 2003 legislation, there has
been a national increase in the number of states that measure and report schoolchildren’s BMI.
Twenty-two states have passed legislation that requires some form of assessment and reporting
for public schoolchildren.9,10 Three of these states—Oklahoma, New York, and Massachusetts—
introduced BMI legislation during their 2013 sessions that was carried over to 2014 legislative
assembly.11

BMI Data Collection
The aggregate child BMI assessments presented in this report are an indication of the extent of
the problem of childhood and adolescent obesity in Arkansas schools, school districts, and the
state. The data presented in this report serve as an early indicator of possible future health
problems that Arkansans may face as these children become adults.
Process
As required by Act 1220 of 2003, student BMI measurements are collected by trained school
personnel or student-health professionals. The measurement process is conducted privately
with the child facing away from the scale. Data are entered into a secure, web-based computer
system that is used to generate individual, confidential Child Health Reports for parents. All
students in even-numbered grades, kindergarten through the tenth, are measured, with a few
exceptions (see section, “Reasons Students Could Not Be Assessed,” for more information).
Definition of BMI
BMI is a standard indicator of obesity status based on an individual’s weight-to-height ratio. It is
a more accurate measure of obesity status than weight alone. Using the students measured
weight and height, BMI is calculated using the following formula:

Boys and girls grow and develop at
different rates. Based upon the Centers
for Disease Control and Prevention (CDC)
recommendations, a BMI percentile for
children is calculated individually for each
boy and girl based upon his or her gender,
age, height, and weight.12 BMI percentiles are then used to categorize children according to
whether they are underweight, healthy weight, overweight, or obese. A higher BMI indicates
greater risk for having or developing obesity-related health problems. Health care professionals
group BMI percentiles to categorize children as obese, overweight, healthy weight, or
underweight.

Overview of BMI Reports
Child Health Reports
Confidential Child Health Reports are generated and made available to parents or guardians
regardless of the child’s BMI classification. The reports describe the reasons Arkansas assesses
student BMIs, details the BMI screening process, educates parents about obesity-related health
risks, and provides simple suggestions to help families improve nutrition and increase physical
activity.
There are five types of reports, one for each of the BMI classifications (underweight, healthy
weight, overweight, or obese) and one for those students who could not be assessed. Each
report includes the student’s height, weight, and BMI, and a classification as underweight,
healthy weight, overweight, obese, or unable to assess. In addition, these five types of reports
are translated into Spanish for those students whose records indicate that Spanish is the
primary language spoken at home. Sample Child Health Reports are available on the Arkansas
Center for Health Improvement (ACHI) website at www.achi.net.
School Reports
BMI profile reports are available through the secure online entry system for each school once
BMI data entry has been completed. The reports, which are available on the ACHI website
(www.achi.net), describe the distribution of BMI status among students in each school. BMI
classifications for schools are presented separately for male and female students by grade and
collectively for all students. BMI classification results for individual public schools in Arkansas are
provided in Appendix A of this report.
School District Reports
School district reports combine information from all available school data in that district.
Summary statistics for each school within the district and overall results for the district provide a
community assessment of the environmental impact of obesity on public school students.
Additional information similar to that included in the school report is depicted using tables and
charts. School district reports are also available at www.achi.net. Results for BMI classifications
of students in individual school districts in Arkansas are presented in Appendix B of this report.

Student and School Participation in the BMI Data Collection
In the 2013-2014 school year, 98.1 percent (1,061 of 1,081) of Arkansas public schools in 255 of
258 school districts participated in the statewide BMI assessment. Table 1 provides information
about school and student participation rates from the last four years of statewide BMI
assessments. Results include all data available for years 8 through 11 and grades K, 2, 4, 6, 8,
and 10. Fluctuations in the total number of public schools each year are due to school closings,
openings, and consolidations. Only schools with students in even-number grades were included
in this report. The totals include BMI assessments that were submitted by June 30, 2014, for
98.2 percent (217,819 of 221,903) of public school students enrolled in grades K, 2, 4, 6, 8, and
10. Among students for whom data was reported, 83.8 percent (182,640) had valid data to allow
BMI calculation (see section, “Reasons Students Could Not Be Assessed,” page 7).

Student participation rates varied from 39 percent to 100 percent of student enrollment in
required grades. High participation rates allow confidence in the results reported by schools and
school districts. For those schools or school districts with low participation rates, underreporting of select categories is possible. Results are reported for 255 of 258 Arkansas school
districts.

Reasons Students Could Not Be Assessed
To complete the BMI assessments, schools collected each student’s height and weight
measurements. Approximately one in six students (35,137) were classified as “unable to assess”
because they did not have valid data for height, weight, or both. Table 2 provides a breakdown
of reasons for the classification of “unable to assess.” Less than a tenth of a percent (42) of data
submitted was invalid for analysis due to data provided being incomplete or incorrect,
measurement protocols not being followed, or the equipment used to complete the assessment
not being accurate.

Statewide Results of the Arkansas BMI Assessment
Total public school enrollment in grades K, 2, 4, 6, 8, and 10 for the 2013-2014 school year,
reported by the Arkansas Department of Education, was 221,903 students enrolled in 1,081
schools within 255 school districts. The results presented in this section are based on data from
182,640 individual students (enrolled in grades K, 2, 4, 6, 8, or 10) with valid BMI assessments,
reported prior to June 30, 2014, by 1,061 schools in 255 school districts. BMI classifications are
reported by gender, ethnic group, and grade. This section includes statewide percentages of
underweight, healthy weight, overweight, and obese students for selected demographic
subgroups. Percentages for these four classifications are also reported for the majority of
districts and counties in the state (see Appendices B and C).
BMI Classifications for All Students
Data from four years of BMI screenings reveal that the distribution of students in each of the
four categories (underweight, healthy weight, overweight, and obese) has remained steady. In
year 11 (2013-2014), more than 38 percent of Arkansas children and adolescents are classified
as overweight or obese.

BMI Classifications by Gender
Figure 3 provides BMI classifications by gender for the 2013-2014 school year. Slightly more
males (22.6 percent) than females (20.3 percent) were classified as obese. In the overweight
category, a higher percentage of females (18.2 percent) were classified than males (16.5
percent). The percentage of underweight students was similar for both genders (two percent,
respectively).

Figure 4 provides a graph of overweight and obese status by ethnic group. As in prior years,
Hispanic students had the highest rates of overweight or obese at 49.0 percent. Among African
American students, 41.8 percent were in the two highest risk categories, followed by 39.7
percent of Native American students, 36.1 percent of Caucasian students, and 31.6 percent of
Asian students.

BMI Classifications by Grade
Figure 5 illustrates the BMI classifications by grade level. Findings indicate that 32.3 percent of
kindergartners entered the school system either overweight or obese. Similar to prior years, the
highest percentage of overweight or obese was found in the sixth grade (42.7 percent).

Hispanic males had the highest obesity risk when evaluating results by ethnic group and gender.
More than half (51.4 percent) of Hispanic males were classified as overweight or obese. The
obesity risk was lower among African American males (38.7 percent) and Native American males
(39.1 percent) than females. The lowest risk among males was found in Asian (36.6 percent) and
Caucasian males (37.1 percent). (See Figure 6.)
The highest obesity risk among females was found in Hispanics at 46.6 percent, followed by
African–American females at 45.0 percent. Native American females had a combined
overweight and obese rate of 40.3 percent, while 35.0 percent of Caucasian females and 26.4
percent of Asian females were classified as overweight or obese.
A higher percentage of males than females had BMI measurements in the two heaviest
categories in all ethnic groups except African Americans and Native Americans. The greatest
difference between males and females within an ethnic group was among Asian students, where
26.4 percent of females were in the two heaviest categories compared with 36.6 percent of
males.

BMI Classifications by Gender and Grade
Data show that approximately 32 percent of both male and female kindergarteners enter school
either overweight or obese. In the second grade, a slightly greater percentage of females
(34.6%) than males (34.2%) were classified in the overweight or obese categories. Of students in
the fourth and sixth grades, males had a greater percentage than females of overweight and
obese. Eighth grade males and females had a similar percentage of students classified in the
overweight or obese category. In the tenth grade, the difference in males and females was most
pronounced, where 43.1 percent of males and 41.0 percent of females were in the two highest
risk categories.

Analysis of the BMI data shows that some groups of students face a heightened risk for
childhood obesity. The highest obesity rates were found among Hispanic males, especially in the
fourth grade where 57.0 percent were classified as overweight or obese.

Conclusion
In 2003, the Arkansas General Assembly passed Act 1220. This landmark legislation called for the
creation of the Child Health Advisory Committee (CHAC) to make recommendations to the
Board of Education and Board of Health related to nutrition and physical activity in the schools.
The Act also required improved access to healthier foods and beverages in schools, the creation
of local committees to promote physical activity and nutrition, and confidential reporting of
each student’s body mass index (BMI) to his or her parents annually. With more than 38 percent
of children and adolescents in Arkansas classified as overweight or obese, the results of this
report highlight the need for continued efforts towards preventing and reducing the incidence
of childhood and adolescent obesity.
