Home health care provides medical treatment for
patients in the comfortable, familiar environment
of their home. The very concept of home health
care is rooted in the most basic human need; that
of independence, familiar surroundings and the
companionship of caring individuals. The scope
of services that will be required for each individual
are determined by the patients’ physician, in
conjunction with the patient and the family.
OSPTA Home Healthcare’s staff is experienced
in entry level care, whether it is for accident
rehabilitation, post-hospital convalescence or short
or long-term disability or illness. For patients who
are homebound following surgeries or illness,
home health care helps families to adjust to a
new lifestyle while maintaining a sense of dignity
for the patient. Home health care aims to make
it possible for patients to remain at home rather
than use residential, long term, or institutionalbased nursing care. The services may include
some combination of professional health
care services and life assistance services.
Our homecare staff is dedicated to meeting
all of your health care needs.

Joint Commission on Accreditation of Healthcare
Organizations (JCAHO).
The “home office” of OSPTA Home Healthcare
is located at 625 Lincoln Avenue, Charleroi,
Pennsylvania. OSPTA Home Healthcare clinicians
are willing to commit the time and skills needed to
assist homebound patients in achieving their goals
by providing “hands on” care. The goal of OSPTA
Home Healthcare is to develop a home treatment
plan for each patient that is individualized and
progressive from hospital to outpatient services.
An interdisciplinary team approach is utilized
to meet the needs of our patients. The policy of
OSPTA Home Healthcare is to admit the patient
within twenty-four hours of discharge from an
inpatient facility. Our physical therapists evaluate
the patient within twenty-four hours of the nursing

therapy group. OSPTA Home Healthcare
was established in June 2003 as a result of
the owner’s dismay at seeing patients in
the outpatient setting that had received
home physical therapy in the past that was
not of rehabilitative benefit. OSPTA Home
Healthcare is a licensed homecare agency

start of care. The staff will also provide quality
care on the weekends. An unique aspect of OSPTA
Home Healthcare is that the provider assigned to
the patient’s case, whether it be skilled nursing,
physical therapist, occupational therapist, etc.,
will remain consistent throughout their course of
treatment.
OSPTA Home Healthcare is also unique in that
the company employs many homecare physical
therapists who also work in the outpatient setting.
This provides the patient with a clinician who
possesses higher expectations of home health
care and the knowledge to more easily bridge the
gap between home health and outpatient care if
needed. This also allows for continuity of care as
the patients’ homecare physical therapist can also
serve as their outpatient therapist. For instance,
a patient who has recently undergone a total joint
replacement who is rehabilitated better in the
home health care setting may see better results
and have a much easier transition into outpatient
care.
OSPTA Home Healthcare provides a wide spectrum
of services by a variety of health care professionals
in the home. Our staff provides and coordinates
the care and/or therapy with the doctor orders. In
support of the doctor’s orders, our staff develops
a written plan for patient care which includes the
services that patients will receive to improve and
maintain physical, mental, and social well being. Services Provided:
Skilled nursing provides treatments such as skilled
observation and assessment including vital signs,
glucose monitoring, medication management,
reminders and teaching, and pain management.

changes, post-surgical care, high-tech infusion,
administration of injections, catheter care, diabetic
management and disease management for a variety
of disease processes. OSPTA Home Healthcare also
provides the Telehealth home monitoring service
for those patients that are high risk
for readmission to the hospital.
These patients may include
those that have the diagnosis of
congestive heart failure (CHF),
chronic obstructive pulmonary
disease (COPD), diabetes, or have
undergone coronary artery bypass
grafting (CABG), or other more
high risk patients. This service
monitors vital signs such as heart
rate, blood pressure, weight and
blood oxygen saturation. This
system provides a way for the
nurse to maintain high risk patients’ vital signs
within a set parameter and can be alerted if any
abnormal vital signs occur in the patient. This
system is a quality assurance measure and is of no
charge to the patient or the insurance provider.
Physical therapy provides assessment of
musculoskeletal, neurological, and vestibular
systems. These assessments allow our physical
therapists to develop individualized interventions
to help patients regain physical function through
improved range of motion, muscle re-education
and strength, balance and gait training / safety.
Licensed physical therapists (LPTs) and doctors of
physical therapy (DPTs) train patients to properly
use assistive devices such as wheelchairs, walkers,
prosthetic limbs, and utilize therapeutic exercises
to help restore mobility and movement, prevent
disability and to aid in falls prevention. Our
PTs also work in conjunction with a licensed
physical therapy assistant (PTA). PTs also make
home modification adjustments to ensure
patient safety and educate the patient about joint
protection, energy conservation, and pain control
techniques.
Occupational therapy develops rehabilitation
plans and performs ongoing evaluations to help
patients re-train for self-care skills of daily living
and working environments. Licensed occupational
therapists (OTs) work in conjunction with certified
occupational therapy assistants (COTAs). OTs work
to improve activities of daily living such as feeding/
cooking, bathing, grooming, toileting and personal
hygiene. These skills are improved to promote
greater independence at home. Patients with
various disabilities such as decreased motor skills
or short term memory loss benefit from treatment
by an occupational therapist to learn how to work
around their limitations. OTs also have the ability
to fabricate splints and casts for positioning, joint
protection, spasticity, and maintenance of rangeof-motion. They assess safety conditions in the
home, need for adaptive equipment, and prepare
the patient to resume daily activities. They may also
work to improve patients’ fine motor coordination
and skills when needed. OSPTA Home Healthcare
employs a certified hand therapist (CHT), to assist
with specialized cases.

patients with activities of daily living (ADLs) such
as bathing, dressing, grooming, and other personal
care needs/hygiene. They may also participate in
improvement of instrumental activities of daily
living (IADLs) such as light housework, laundry,
light meal set up and/or preparation, shopping,
telephone usage and money management to
enable the patient to live independently in the
community. HHA will follow through with orders
given by the other disciples as well. OSPTA Home
Healthcare employs a home health aide who is
also a certified phlebotomist and competent in
venipuncture as ordered by a licensed health care
provider.

communication problems related to language
and speech. Patients may have an articulation,
language, fluency or resonance disorder which
prevents them from understanding others or
being understood. Speech therapists (STs) provide
an effective speech program to assist patients
in improving receptive and expressive speech,
language skills and to enhance cognitive language
through vocal exercises and cognitive therapies.
STs also intervene to improve eating function
in patients with swallowing disorders through
recommendation of liquid thickness, dietary
texture, and compensatory safe swallow strategies.
They work closely with caregivers, providing
education and recommendation / referral for
alternative augmentative communication for
those unable to express themselves.
Dieticians develop meal plans and provide
nutritional counseling and education for healthy
eating to patients with a variety of medical
conditions. Dieticians evaluate individual and
family nutritional needs, caloric intake and
areas requiring improvement. They may make
nutritional recommendations and ensure proper
caloric intake for people with health conditions
such as diabetes, cancer or heart problems.
Social services are a very important part
of the health care team because of their
links to valuable community resources that
may help improve a person’s quality of life
through education and support. Patients and
medical professionals rely on social service
workers for their expertise and ability to
handle difficult and complex situations.
Social services consult with patients and
their families on coordinating community
resources,
transportation,
financial
assistance, and family counseling. The social
worker performs assessments with patients
to determine physical, mental or emotional
needs, and assessment of the family support
system.
