I am honored to be a part of an organization that does so much to help those struggling
to get the home medical equipment (HME) and services needed for physical
disabilities. We continue to create community solutions with a focus on local
service first; partnerships allow us to collect and provide HME items across
the state and country during disaster relief.
We have come a long way since our founding in an Atlanta garage in 1986.
With the current cuts to federal programs and the demise of many HME
companies, FODAC is needed now more than ever. We depend on our
donors and corporate sponsors to help us spread the word about FODAC’s
services, especially to donate gently used HME items, vehicle lifts, or used
computers that can be refurbished and redistributed to help those in need.
We also need your financial support; every donated dollar is multiplied to $10
of services and equipment into the disabled community, with only 4% of total
revenue used for overhead.

3. FODAC does not perform repairs on a walk-in basis. Many
people who arrive to get a repair without calling first are asked
to make an appointment and come back on another date.

Vehicle Modification
Vehicle adaptive equipment makes
limited mobility a thing of the past.
However, for many disabled clients and
their families, the cost of vehicle
adaptations are prohibitive and are not
covered by insurance. FODAC’s vehicle
modification program installs equipment
such as scooter hitch lifts, full size van
lifts, 4-point tie downs and more on your
car for minimal cost.
We remove adaptive equipment from
vehicles at no cost and then refurbish
and reinstall that equipment on another
client’s vehicle for a very low price.
Home Modifications
FODAC’s home modification program utilizes government and
community funding sources to build access ramps and other
home modifications to accommodate mobility impairment. We
have completed projects that assisted 42 families in the Metro
Atlanta area who were eligible based on location and income.
We work with state and federal programs, Brain and Spinal
Injury Trust Fund, and The Georgia Department of Community
Affairs. FODAC is also very proud of our new contract with the
Atlanta Regional Commission which has funded home access
projects that allowed 22 people to transition back into the
community who where inappropriately trapped in a nursing
home. We maintain a resource data base to help families in need of
financial assistance secure funding for their modification needs.

FODAC PROJECTS
Wheelchair Repair
FODAC is one of the few organizations in the country with the
resources to operate and maintain an on-site wheelchair and
HME repair facility. For a nominal fee, FODAC technicians can
repair all types of HME, whether acquired through FODAC or
elsewhere. This is a vital service to the community that reduces
wait time for otherwise costly repairs and helps people maintain
access to school, work, and the community.
Some of FODAC’s donated equipment and supplies goes overseas,
either for disaster relief or to support local medical programs in
need of basic equipment such as wheelchairs and hospital beds.
Medical Supplies
We pre-sort donated medical supplies for easy distribution first
to our clients, then local free clinics, and then excess items are
given to capable international service agencies. Catheters, colostomy supplies, wound care and adult incontinence aids are just
a few of the most common requests we get for supplies. So far in
2013, we have distributed over 40 pallet loads to local free clinics
along with oxygen concentrators, nebulizers, suction and other
medical devices.
Computer and AT Repair
This year, FODAC partnered with the Tools for Life Program
to take our previous computer refurbishment program to new
heights. We now have a Microsoft Software agreement and
in-house Microsoft Certified Refurbishers on staff. Computers
and peripherals are displayed and sold in the Thrift Store for
qualified recipients who are disabled or low-income. FODAC
needs corporate and private computer donations for the program.
FODAC is also expanding its assistive technology program as well.

International Relief
Institutional equipment and clinical supplies
that can’t be re-used domestically, such as
exam tables, hospital beds, and therapy lifts,
are given a new life overseas. These items
also include feeding pumps, oxygen supplies,
prosthetics, catheters and surgical kits. 2013,
we partnered with the Buckhead Rotary
Club in Atlanta to purchase and distribute
400 new wheelchairs and many excess
hospital beds to Ecuadorean hospitals.
FODAC also provided many truckloads of
clinical supplies to Medshare and several
other international relief agencies.
Disaster Relief
FODAC leverages our manufacturer and non-profit partners to provide aid during disaster relief. Past efforts have
included hurricanes Katrina, Irene, Rita, Isaac and Ike, as
well as the Haiti earthquake. This fiscal year saw FODAC
trucks filled with HME and supplies headed to tornadoravaged Oklahoma, and areas of New Jersey still recovering
from Sandy. The Georgia Emergency Management Agency
(GEMA) has also provided a contract to FODAC in 2013
that provides pre-staged equipment to be held at FODAC
and depends on FODAC for the distribution of these items
in a major disaster in Georgia.

FODAC Community Thrift Store
Opened Tuesday though Friday from 10 am until 5 pm and Saturday from
10 am until 4 pm, the FODAC Community Thrift Store offers a large
variety of clothing, furniture, household goods, jewelry, books and
music – and so much more! All proceeds of the thrift store
support FODAC’s mission of supplying medical equipment to people
who are disabled at little or no cost
to the recipients or their families.
You’ll get great bargains – they’ll get
better lives.
If you have any gently-used clothing or household goods you no longer need, consider donating them to FODAC for the
thrift store. Donations are always welcome and are tax deductible.

• Secured a consultant to assist with the next three year Strategic Plan.
• Recently changed FODAC fiscal year to allow better adjustment to year end giving.
• Sponsored the welcome home project www.welcomehomejoan.com for a person in Dekalb
with a violent injury to provide local supporters a tax deductible avenue for giving.
• Sport chair collaborative grant with Blaze Sports for children receiving sports
wheelchairs from FODAC.
• Sport chair for Curtis Lovejoy to meet new regulations for Paralympics

Charity events are FODAC’s main vehicle for raising the funds needed to support our work with the
disabled. Every year, we work hard to organize, family-centered events that offer a variety of fun
activities to meet every interest. Breakfast with Santa, our annual Golf Tournament and our Walk and
Roll 10K are just a few of FODAC’s events that entertain while providing the opportunity for
participants to make a difference in the life of a disabled child or adult.

Run, Walk ‘n’ Roll
The wettest Run, Walk, ‘n’ Roll turned into one of the BEST ever! Over $34,000 was raised at the
13th annual event and nearly 100 rain-drenched runners and walkers braved the elements at Stone
Mountain Park on May 4, 2013. Our very special guest this year was Snellville’s Aimee Copeland who
received some temporary HME from FODAC during her recovery from a flesh eating disease injury.
FODAC Golf Classic
FODAC 2012 Golf Tournament was held at Stone Mountain Park on October 15th. The Tournament
was played on the Lakemont course, located at the base of Stone Mountain and running along the
waters edge. It was a great success raising over $30,000 dollars. We are looking forward to our 2013
Golf Tournament to be held in October. Come join us as a corporate sponsor or come out to play a
wonderful day of Golf!
Breakfast with Santa
FODAC held its annual breakfast with Santa at the Marriott Resort at Stone Mountain Park. A
delicious breakfast is served on elegant white linen tables guaranteed to put you in the Christmas
spirit! Children can enjoy festive music, games, a magician, caricature drawings, toys, and a
special visit with Santa! Come join the 2013 Breakfast with Santa to be held December 7th at the
Stone Mountain Marriott Resort.

The FODAC Board is comprised of committed community leaders who are passionate about the work we do for the disabled
community. William Zachary is the current Board Chair who also
practices law at Zachary & Seagraves, P.A. who states “I have
been affected in such a deep way by the passion that the FODAC
employees, the dedication of volunteers, and supporters who
serve the community and the tangible difference it makes in the
lives of thousands in need”. The other officers this year include
