SUBJECT: Spring 2015 AFROTC New Student Enrollment (NSE) Package
1. Congratulations on your decision to join the AFROTC program at Detachment 165, located at Georgia
Tech! We look forward to meeting you and working with you to achieve your goal of becoming a
commissioned officer in the world’s greatest Air Force.
2. This information package is critical to your enrollment in AFROTC. You must follow all directions as
indicated and fill out all paperwork COMPLETELY to ensure proper processing. It is also very important to
pay attention to the appropriate office in which to turn in certain forms. AFROTC Det 165 will not process
cross-registration or GT Health Center paperwork. It is YOUR responsibility to ensure that you are properly
registered prior to all applicable deadlines.
3. Each semester, AFROTC Det 165 offers a New Student Orientation (NSO) Program for all new cadets.
NSO is a one-day introduction and overview to AFROTC. You may bring up to two guests to NSO. NSO
includes a detailed AFROTC program overview, a briefing on AFROTC Scholarship opportunities, and an inprocessing session for new cadet records. Attendance is not required, but it is highly encouraged and very
beneficial to all new cadets. NSO will be held on Friday, 2 January 2015.
4. Please fill out and return ALL required forms, including the New Cadet Checklist, No Later Than 1
December 2014. Do not call with questions until you have read the entire packet. Directions for submitting
all required forms and documentation are included in the New Cadet Checklist (Attachment 1).
5. In this package, you will find the following forms and guides:
a. New Cadet Checklist (Attachment 1): The purpose of this checklist is to ensure that you complete
all the required forms necessary to join AFROTC. Please note that not all forms will be submitted to
the Detachment.
b. AFROTC Form 28, Air Force ROTC Pre-Participatory Sports Physical (Attachment 2): This
form MUST be filled out in its entirety and SIGNED by a qualified physician. The weight standards
on page 2 of the Form 28 reflect AFROTC-wide minimum and maximum allowable body weight for
membership. If you are over or under weight, feel free to contact us if you wish to discuss your
situation. Make sure your name is clearly written in Block 1 at the top of the page. The AFROTC
Detachment (Block 2) is DET 165.
c. AFROTC Form 35, Certification of Involvements with Civil, Military, or School Authorities/
Law Enforcement Officials (Attachment 3): In this form, you are required to provide us
information on your past civil involvements (arrests/citations/tickets/etc). You must fill out this
form completely/honestly. Make sure to write your name in the ‘Certificate’ block of Section I. If
you have no civil involvements, write “NONE” in the first line of Section II; sign/date where it says
“Signature of Cadet.”
d. AFROTC Program Overview – Standard Program (Attachment 4): This is a basic overview of the
AFROTC program. It is intended to provide general information about the progression of classes that
you will take as a cadet. This is only a reference! Please refer to Attachment 7 for scheduling
information.
e. AFROTC Program Overview – Dual-Enrolled GMC Program (Attachment 5): This is a program
overview for those applying for the three-year program. It is intended to provide general information
about the progression of classes that you will take as a cadet. This is only a reference! Please refer to
Attachment 7 for scheduling information.
f.
AFROTC Spring 2015 Curriculum (Attachment 6): This is a “week-at-a-glance” view of the
AFROTC courses offered in Spring 2015. NOTE: Please refer to Attachment 7 for scheduling
information.
g. Spring 2015 Class Scheduling Guide (Attachment 7): Please use this guide to schedule the
appropriate AFROTC courses. This guide also includes the course information required to complete
the ARCHE Cross-Registration Application.

6. If you are NOT a Georgia Tech student, you MUST complete these additional requirements:
a. Cross-Registration. If you are student at one of our twelve “Cross-Town” schools then you can join
AFROTC at Georgia Tech. Please refer to our website for a complete listing of all eligible colleges
and universities. Cross-Town cadets work towards a degree at their home school while attending
AFROTC courses and training at Georgia Tech. All cross-town students MUST register for AFROTC
classes offered at Georgia Tech through the ARCHE Cross-Registration program. For information on
the ARCHE Cross-Registration Program, including a list of Points of Contact by Institution, go to:

* Copies of these forms are located in Attachment 8 (Coordinators) & Attachment 9 (Application)
Please read the form very carefully to ensure proper processing. The Cross Registration Application
must be submitted directly to your home institution’s registrar office no later than 15 November 2014
unless your home school has an earlier deadline. The Cross Registration Application must be
completed on time or you will not be able to join AFROTC. You will receive a GT ID once you have
successfully cross-registered through ARCHE and your application has been processed.
b. Georgia Tech Stamps Health Services Requirements. In order to cross-register at Georgia Tech,
you must submit the Georgia Tech Medical Entrance Form and Certificate of Immunization directly
to the Georgia Tech Stamps Health Services. These forms can be downloaded at:

The medical forms must be submitted directly to the Georgia Tech Stamps Health Services.
Failure to meet this requirement will result in a registration hold that will prevent you from
being scheduled for AFROTC classes.
7. If you have prior military service, to include prior enrollment in a Service Academy, you must provide a
copy of your DD Form 214 as indicated on the New Cadet Checklist. Also, if you have been administratively
disenrolled from an officer candidate program in the past (military service academy or any ROTC program in
college), you must provide your Form 785 and contact information for the Service Academy or ROTC Unit.
In either case, please contact the Recruiting Flight Commander to discuss your particular situation.
8. All cadets in AFROTC will be scheduled for a medical examination through the Department of Defense
Medical Examination Review Board (DODMERB). This is your “Military Entrance Physical.” Only High
School Scholarship Program (HSSP) selectees are required to have a certified DODMERB physical before
entering AFROTC. All other new cadets will be scheduled for their DODMERB physical within the first year
of AFROTC. You must be certified by DODMERB in order to continue with AFROTC. If you have any
medical concerns that you think may preclude you from military service, please refer to the DODMERB
website: https://dodmerb.tricare.osd.mil/Default.aspx. DODMERB is the ONLY authority for all medical
qualification issues. Detachment 165 does not have personnel on staff that are qualified to make a
determination about your medical suitability for military service.
9. All required forms and documents (New Cadet Checklist items 1-3) must be returned to the Detachment
NO LATER THAN 1 DECEMBER 2014. Checklist items 11-15 must be brought with you – do not send
these items with your NSO Packet. Checklist items 9 and 10 must be coordinated with the appropriate office;
Detachment 165 will not process Cross-Registration (item 9) or Georgia Tech Health Center paperwork (item
10).

I.
STATEMENT TO THE APPLICANT/CADET
A. The Detachment Commander must know if you have ever been arrested, convicted, involved with law enforcement officials or authorities for him/her to
determine if you meet the character requirements for membership in Air Force ROTC. It is necessary for you to report any involvement with civil, military,
or school authorities/law enforcement officials regardless of its insignificance , disposition, or finding on the certification provided below. Include traffic
violations and any incidents which resulted in your being judged a juvenile offender. A finding of not guilty or advice by an attorney, court official, or
anyone else to consider your record as clear does not constitute authority to leave the involvement off of the certification.
B. In the future, you must report any civil involvements to the Detachment Commander or his/her designated representatives within 72 hours following its
occurrence. If such incidents occur during a period of leave from the institution (e.g., student teaching or foreign study), attendance at Field Training, or
during normal vacation periods, the 72-hour time limit will apply effective with the official date of your return to the institution.
C. Concealing or failing to report an involvement with civil, military, or school authorities/law enforcement officials, giving false information or claiming
subsequent to initial certification that you were unaware of the contents of this document may result in elimination from consideration for membership
in the Air Force ROTC program; or, if already a member, may result in your discontinuance from the Air Force ROTC program. The information reported
on this certification form will be treated as confidential matter, subject to the provisions of the Privacy Act of 1974 and the Freedom of Information Act.

NOTE: The "home institution" is the ARCHE institution at which a student is currently enrolled in a program for which
academic credit is awarded (the definition of a "program" shall be determined by the home institution). The "host
institution" is the ARCHE institution at which a student would like to attend a course through cross registration.

In good academic standing as defined by the home institution and meet all eligibility requirements set by home
institution. Students should check with their home institution’s Cross Registration Coordinator for detailed eligibility
requirements and deadlines specific to that school.
Currently enrolled at home institution in at least one course at the undergraduate, graduate, or professional level in a
program for which academic credit is awarded (the definition of a "program" shall be determined by the home
institution)
Not a first-semester freshman (unless cross registering in courses for purposes of ROTC training or marching band)
Requested course not offered concurrently during semester at home institution, and not a non-credit continuing
education course, workshop, seminar or for audit
Meet prerequisites (if any) for requested course, as defined by host institution
Meet all immunization and health requirements as required at host insitution
Meet all application deadlines

Student's Cross Registration Procedure
A. Obtain a Cross Registration Application from home institution’s cross registration coordinator or registrar’s office. The
form also is available at http://www.atlantahighered.org/CrossRegistration. Complete first three sections.
B. Verify that the course requested is offered at host institution. Identify whether enrollment restrictions or prerequisites
apply for the requested course(s) offered by host institution.
C. Consult with home institution academic advisor to determine eligibility for cross registration. Complete the Course(s)
Requested section of the Cross Registration Application and have academic advisor sign it.
D. Consult with home institution’s cross registration coordinator to determine host institution’s registration procedure,
immunization requirements, etc., and have coordinator sign the Cross Registration Application. The home institution’s
cross registration coordinator will forward the application to the host institution and request approval for cross
registration. If the host institution course instructor’s or department’s approval is required, the student may be
directed to obtain the required signatures.
E. Once all approvals have been obtained, the host institution’s cross registration coordinator will register student for the
approved course(s) on a space-available basis after host institution's regularly enrolled or continuing students have
had first priority for registration. The host institution’s cross registration coordinator will inform the student of approval
and registration status. All applicable course fees must be paid to home institution when due by the home institution.
However, special fees (lab, parking, equipment lease, etc.) and security deposits must be paid to host institution.
F. Student must abide by all home and host institution policies and schedules regarding grades, exam dates, absences,
drop/add/ withdrawal, etc. To withdraw from a course, both the home and host institutions’ withdrawal procedures
must be followed. Changes in cross registration must be approved by both institutions’ cross registration
coordinators.
G. Some institutions do not allow cross registration during the summer. These institutions are indicated below.
H. If home institution does not participate in cross registration during the summer, student must seek admission to the
host institution as a transient student by contacting host institution’s admission office for application deadlines.
I. Seniors who are graduating at the end of the semester in which they are cross registered shall be responsible for
contacting the host institution coordinator to obtain an alternate grade report to provide to the home institution.
J. Students requiring disability accommodations for the requested classes should contact their home institution Disability
Support Coordinator at the time of application in order to permit time to arrange accommodations.
ARCHE Cross Registration Application Deadlines
Home institution’s Cross Registration coordinator must submit request to host institution by this date.
Students should check with their home institution for earlier internal deadlines.
