To Our Community
This year, we celebrated the retirement of an associate who gave 41 years of compassionate and caring service to hundreds of the most frail and vulnerable members of our community during her remarkable career. The expression of love and compassion by our caregivers remains the cornerstone of the founda-
tion for Community Health Systems (“CHS”) and provides the tangible evidence of our legacy of service
and unwavering commitment to meet the health care needs of the community.
Whether improving the health of those we serve or enhancing the quality of life for those who are unable to care for themselves, CHS associates work diligently to meet current healthcare needs and to anticipate the evolving needs of those we will serve in the future. For example, during the past year, through the
collaborative efforts of many professionals within our health system and Community Health Foundation, the ability to communicate was restored to a client through
the use of an augmentative communication device. In addition, staff members joined together and embraced technology as a solution to the long-standing, multi-dimensional health care issue of delivery and management of pharmacy services. The result of these technological improvements has been to maintain fiscal discipline
and responsibility while enhancing clinical excellence and improving the quality of life for those we serve.
While Community Health Systems has a long history of providing quality customer service to residents in the skilled nursing setting, the System has also focused its efforts in recent years on serving persons in the home setting. System resource utilization information recently indicated for the first time that the number of
clients served in the home setting exceeded the number of residents served in the skilled nursing setting. Services such as emergency response and care management
have become an integral part of our care delivery system and are a reflection of our commitment to assist clients in maintaining their independence and remaining in
their homes. Concurrently, System skilled nursing centers continue to obtain national recognition for delivery of excellence in service.
The Board of Directors of Community Health Systems, working closely with its management team and associates, is dedicated to developing and executing
strategies that implement innovative solutions to evolving health care needs. Through these strategies, we anticipate that Community Health Systems will continue
its legacy of service and meet the healthcare needs of our community both presently and in the future.

Technology Speaks
Communication devices change lives
T
The ability to communicate with their voices is something many people may take for granted. For others,
however, much effort and frustration is involved in the
simplest of requests or comments. Through augmentative communication devices, Integra Rehabilitation
(“Integra”) gives communication-impaired individuals
the chance to once again share their thoughts with
others.
In April 2008, 70-year-old Annie Norris was
given a Communication Picture Ring by her speech
language pathologist (SLP), Jill Duncan, SLP for
Integra. The Ring was attached to her wheelchair,
and she pointed to pictures and words on the device.
“She really wanted a way to talk with people that she
interacted with,” says Jill. “This system did not meet
her needs, so she stopped using it.”
Jill knows that Ms. Norris enjoyed using the computer
at home and that her ability to understand was intact,
but she had difficulty expressing herself to others.
“Through a speech therapy evaluation, I determined
that the Message/Mate, which is a picture/word/
speech-generated device, would best meet the needs
of this patient,” she says. Fortunately, Jill was able to
supply the device to Ms. Norris through a donation
from Community Health Foundation.
Device Use
Ms. Norris has used the Message/Mate since October
2008, and she loves it. “She is constantly grinning
since she has been using it. It has added speech to her
life again,” says Jill. “The Message/Mate has increased
her quality of life and her ability to be involved in
her environment. She is able to communicate after
11 years with family, friends and staff. Her daughter
wishes she had access to this device 11 years ago.”
Augmentative communication devices have
helped other persons remain in the workplace. An architect was able to keep designing buildings. Another
client, a lawyer with multiple sclerosis, continued
representing clients in the courtroom. “No matter
what environment a patient lives in, such a device will
change their quality of life,” says Michelle Moore-Andrews, President of Integra. “It changes not only their
life but the lives of the people who interact with them.
In a skilled nursing facility setting, staff can better understand how to meet their needs. Family and friends
can continue to communicate with their loved ones.
Communication is a key component in our lives.”
Once people obtain a device, the process of
sharing their thoughts again starts almost immediately. “Day-to-day communication becomes easier.
Using greetings to address people is facilitated with
the device,” says Michelle. “Over time, the patient
receives speech therapy to learn how to use the device
for interacting with others and communicating more
independently.”
While these devices take some getting used to,
Michelle envisions bright futures for patients using them. “After initial training, patients can work
toward learning to use this device to carry on complex
conversations. Stephen Hawking, well-known author,
is an excellent example of all the possibilities. He has
been on Larry King Live on multiple occasions and is
able to converse during a one-hour interview by utilizing an augmentative device.”

A Streamlined System
Docutrack betters Elder Care and its service
In any office environment, there is always a goal of
increasing efficiency, reducing costs and enhancing
customer satisfaction. Elder Care Pharmacy (“Elder
Care”) is no exception. Because of this, Elder Care
has implemented Docutrack, a complete workflow
and document-management system. “We put ourselves in a position to handle the changing environment of pharmacy,” says Rusty Lee, Vice President of
Elder Care. “In an efficient pharmacy, you constantly
strive to do more with fewer resources.”
Docutrack helps make pharmacy work more
efficient. Some of the ways it does so are by:
• Effectively managing refill orders. Docutrack’s
reading of bar coded prescription refill numbers
eliminates the time-consuming method of typing
each number.
• Easily searching for and retrieving any document, including delivery manifests.
• Date and time stamping documents (complete
with the operator’s name each time someone
“touches” them). This information cannot be
altered and provides each document’s history.
• Using an integrated fax server that handles
incoming and outgoing faxes.
• Organizing documents into folders and queues,
which can be accessed by multiple users simultaneously.
• Permanently linking patient-specific records to
the patient’s file in the dispensing system.
Docutrack not only does each of these things and
more, it also dramatically reduces the ever-increasing
need for time (to file and find documents), money (to
cover the labor involved and the actual paper cost),
and storage space. “Medicare Part D and Centers
for Medicare and Medicaid Services now require
that we maintain all documentation relating to filled
prescriptions for 10 years. After using the system less
than two years, we already have 1.1 million documents stored, so you can see the amount of storage
space that would be required to handle the amount
of documents we would have at the 10-year mark,”
says Rusty.
Perhaps the top reason Elder Care enjoys Docutrack is for its aid in pharmacists’ relationships with
nurses. Due to the efficiencies of the system, Elder
Care was able to transition without additional costs
to the nursing centers. “They’re just doing the same
thing they’ve done for years: faxing orders to us,”
says Rusty. “That’s the neat thing — nurses have no
idea there are changes in the system.” Docutrack also
allows for faster service of order inquiries. “When you
call, you’re no longer on hold. We can tell you when
it was ordered, who signed for it, etc.” he says. “We
can fax the order from our desk while we have you on
the line.” These changes give nurses faster answers,
which creates more time for them to focus on the care
of patients.
Not only is the system a great resource for pharmacists, it is benefitting Elder Care as a whole. “All
aspects of the organization are using this and have
found ways to further maximize its benefits,” says
Rusty. “For instance, accounting utilizes electronic
copies of checks, which reduces paper and streamlines the process.”
While Elder Care brought in the Docutrack system for a variety of purposes, like with any change,
it was not without some uncertainty. “Our group
wants to be a leader in workplace technology. Document management was going to play a big role. At
the start of anything you worry,” says Rusty. “You
worry you’ll spend lots of money and get no results;
but fortunately, things haven’t turned out that way
for us.”

The Comforts of Home
Home & Community Services keeps people at home
H
Home is a special place – the place where people
feel most comfortable. However, sometimes circumstances threaten our ability to stay independent.
Helping people in this situation is the main goal of
Home & Community Services, Inc. (“HCSI”). Its
three organizations – HelpLink Emergency Response
System (“HelpLink”), Independence+ In-Home Care
(“Independence+”) and SOURCE Care Management
(“SOURCE”) – offer clients assistance they need.
• HelpLink provides emergency response services. Within seconds of pressing a button, the
HelpLink device can reach an operator who can
dispatch the most appropriate help. The call
center maintains current individualized information which has been provided by each client.
• Independence+ offers daily assistance to clients
in their own homes. Its individualized home care
services range from companion and personal care
services to limited skilled nursing care.
• SOURCE Care Management works with members who voluntarily enroll. SOURCE coordi-
nates and navigates the extremely complicated
health care system, assisting members with life
management strategies and enabling them to
receive care and services needed to keep them
in their homes. Such services include homedelivered meals, personal support, emergency
response systems, home health and more.
Meeting the Need
Hattie Butts is a recipient of services from all three
HCSI organizations. Her SOURCE case manager
checks in routinely via phone and home visits.
The case manager coordinates her care, including working closely with her physician. Mrs. Butts
wears a HelpLink device at all times, which makes
her feel more secure. She enjoys daily visits from
Independence+’s Portia Chapman, who helps her
with everyday tasks. Without the help of HCSI’s organizations, Mrs. Butts would not be able to live in
her own home, and that is something she cherishes.
True Success
HCSI monitors its work through Quality Assurance
Programs and other audits aiming for high quality
standards throughout each organization. “We have
created a culture of continuously striving to find
ways to enhance our programs,” says Jean Ginn,
President of Home and Community Services. “Of
course we track client outcomes, census data, financial data, etc. We also look for ways to give back to
our communities, whether it’s through health fairs or
education on various health topics.”
Jean cites additional ways HCSI has helped
clients: “Sometimes they don’t have money to pay
their electric bill, and we work to get that done.
We’ve had husbands of staff members volunteer to
repair broken pipes with parts donated by the local
hardware store,” says Jean. “The multiple acts of
kindness and going the extra mile that happens every
day are, I believe, a huge part of being successful.”
Martha, Mrs. Butts’ daughter, truly appreciates all that HCSI makes available to her mother.
“We have really enjoyed the company. They have
been great for us for the past two and a half years.
I couldn’t ask for better help,” she says. “If I didn’t
have their help to go through what I need to do, I
don’t know what I’d do.”

The Definition of Quality
Ethica Health & Retirement Communities prepare for future excellence
Q
Quality service can take a company or organization
to the next level in success. This is certainly true at
Ethica Health & Retirement Communities (“Ethica”).
A belief in customer-centered service and the pursuit
of excellence defines Ethica: Quality is not a campaign, it is a culture.
Advancing Excellence
Just one example of Ethica’s client centers’ ever-present desire to further enhance their quality is their
voluntary participation in Advancing Excellence,
a national quality campaign. This campaign also
provides the opportunity for the centers to measure
themselves against other U.S. centers. Along with
other centers in the state, Ethica worked to decrease
physical-restraint use and health-care acquired pressure ulcers. Ethica client centers also committed to
measure and improve family and associate satisfaction. Participants in Advancing Excellence have
reached their goals for improved care faster than
non-participants.
To help them achieve these commitments,
Ethica’s client centers takes part in award programs,
such as the National Quality Award and created
internal campaigns, including Encounter Excellence
and satisfaction surveys.
Awards
The AHCA/NCAL National Quality Award is bestowed upon member organizations that apply and
demonstrate continuous quality improvement, as is
emphasized in the Advancing Excellence campaign.
Ethica’s client centers each make an independent
decision to apply for an award. This is based upon
how they measure against the criteria of the Malcolm
Baldridge model, which is part of the criteria for the
National Quality Award. These criteria address the
key requirements for excellence in long-term care
organizations.
The award includes three steps, each of which
builds upon the previous. Currently, Ethica’s client
centers have won awards in both Step I and Step II.
With Step I, organizations describe their mission,
characteristics and key challenges, as described in
the Baldridge criteria. This helps both the applicant
and the evaluators understand the center and its approach. Step II applies once a center has received a
Step I award. Applicants describe how they address
key elements of the core values and concepts of the
Baldridge criteria, which helps them stay organized
and achieve their desired results.
“The greatest benefit of the application process,
and hopefully, resultant award, is the need for the
center’s team to objectively evaluate systems and
processes that are in place,” says Pam Clayton,
President of Ethica. “The assessment allows the team
to mature in the performance-improvement process,
further develop analytical skills, and provides a format for sharing positive outcomes.”
Ethica-based Customer Service
Ethica client centers have implemented an Encounter
Excellence philosophy. This encourages associates on
all levels to actively ensure that patients, families and
other customers are recognized as individuals with
individual needs. “Encounter Excellence permeates
our culture and guides all interactions with patients
and customers,” says Pam. “It is a cornerstone in our
decision-making.”
As part of this philosophy, Ethica obtains independent client center satisfaction surveys. “We want
the customer’s feedback and actively seek it. This
may mean making personal calls to encourage family
members to assist patients with survey completion,
approaching representatives personally, and meeting
with staff to discuss the satisfaction survey process’
importance,” says Pam. Once the results are in, the
centers analyze the information and then meet with
all involved to determine the accuracy of the analysis.
With this information, an action plan is developed
around the most prevalent opportunities, which is
then shared and reviewed throughout the year.
The Impact
Through each of these campaigns and awards, Ethica’s
client centers continually
build upon their goal of quality improvement. “Because
detailed center-specific action
plans are developed and
acted upon, trust develops.
In an environment of trust,
much can be accomplished,”
says Pam.

Service at Every Level
A
ECP Distributors reaches out to its customers
At ECP Distributors (“ECP”), customer service is its
number one priority. ECP, a medical supply company
that specializes in long-term care, strives to ensure
that its customers are well taken care of. It works
primarily with long-term care centers to provide
everything from housekeeping and laundry supplies
to a wide range of medical products.
Exceptional customer service is how ECP differentiates itself from other suppliers. “Our associates
understand this, as well as the fact that we provide
products and services that enhance and sometimes
maintain someone’s well-being,” says Cal Franklin,
President of ECP. “We take that responsibility
seriously.”
This sense of responsibility further explains the
organization’s commitment to top-level service,
which is shown throughout the entire organization, including customer
service representatives,
warehouse personnel and
drivers. “Our associates
are committed to providing excellent service,” says
Cal. “That commitment
is stated in our mission
statement: The mission of ECP Distributors is to deliver a level of service that exceeds the expectations
of our customers in an environment that promotes
excellence and achievement for our associates.”
At the Office
When products are ordered, ECP customer service
representatives (“CSRs”) continually go the extra
mile for customers. “On multiple occasions, the
person in the client center who places the ECP order
may have been out on vacation or leave; our CSRs,
due to their extensive knowledge of their respective
centers, will place the order for those persons so they
will have enough stock to last until the next week’s
delivery,” says Cal. “On those occasions when a
center does run out of stock, ECP’s CSRs have been
known to deliver products on their lunch hour or on
their way home from work.”
Warehouse personnel strive to ensure orders
are processed correctly. When substitutions are
made, they alert the customer and get approval for
the substitution. “Each of the warehouse personnel
is acutely aware that our customers’ ability to run
streamlined centers is in part due to us making sure
their orders arrive on time and complete. This makes
our commitment to their service our first priority,”
says Cal.
On the Road
Because they travel certain routes daily, ECP drivers
develop strong relationships with the customers to
which they deliver. This creates an atmosphere where
drivers enjoy providing great service to each client.
Cal shares a story that illustrates this commitment:
“One of our drivers commented, ‘Getting to know
the administrators and associates one-on-one at each
center – so they know they can trust you to do what
needs to be done – is one of the best parts of my
job.’”
Another example of driver commitment is their
willingness to work demanding schedules. Each
week, ECP drivers log over 4,000 miles on 16 different routes with more than 80 stops. This often
requires them to leave in the early hours of the morning. However, they realize that timely deliveries are a
key component to good service.

Site for Service
CHS launches its user-friendly Web site
I
In May 2008, Community Health Systems (“CHS”)
launched Version 1 of its new Web site, www.chsga.org. This site, according to Ken Brouse, President
of System Administrative Services Inc., is a gateway
to understanding geriatric and long-term care, life
management strategies and vital health information
via the Internet.
Site Features
Because the site will potentially provide information to millions, 85 percent of the site’s content is
general information. Some of the topics include
retirement planning, understanding long-term care,
planning your doctor visit, and what to look for in
skilled nursing centers and assisted living communities. Based on the site analytics, the most popular
area for visitors is the Health Resource Center. This
interactive library provides a plethora of health
information, which is updated frequently to make
every effort that the latest technologies are included.
It is easily searchable and offers videos on a range of
procedures.
There is also CHS-specific content for those
desiring more facts about the organization itself. The
site has helped increase community awareness of
Community Health Systems. When potential users
have questions, concerns or are simply researching
their options, the site provides information needed
to make educated choices. “We are committed to the
communities we serve,” says Ken. “We believe in
creating a community that connects seniors, the disabled and their families to services designed to meet
their needs. We feel that the CHS site is another tool
to provide information and key health knowledge to
the community.”
Keeping Visitors First
As part of CHS’ visitor-satisfaction goal, the site’s
creators wanted to make sure the vast amount of
information on the site was well organized and created a valuable user experience. They also continually pay close attention to the feedback they receive
from site visitors. “It gives us valuable insight into
the user’s needs, habits and wants,” says Ken. He
says they are closely monitoring the analytics of the
site and making appropriate changes. “We want to
make every effort that our users are easily gaining
the knowledge they seek. The site’s design is easily
navigable, user-friendly and ADA compliant,” says
Ken. “Moving forward, we plan to poll users to find
out more information and analyze the feedback,
aiming for the maximum effectiveness of the site.”
To further create an appealing experience, the site is
advertisement-free.
Even though the site is still new, it is already
impacting its visitors. One person took the time to
voice her opinion about the quality of information
she discovered. Geneva Perkins of Locust Grove,
Ga., wrote: “The health library has been a blessing
to me and my family. There is so much information
regarding aging and health that I can actually use in
my day-to-day life. I find it easier to navigate than
WebMD.”
Reaching Even Further
Ken expects the site to continue to grow. “Enhancing the site frequently and maintaining its content
will only increase the amount of attention the site is
receiving,” he says. He adds that Version 2 of the site
is already in the pre-planning stages. This upgrade will
further assist the public in its search for information.

No One Can Do Everything;
Everyone Can Do Something

The primary role of Community Health Foundation (“CHF”) is to discover ways
to positively impact the lives and communities touched by all member organizations of Community Health Systems, Inc. Our goal is to provide a way where one
person, regardless of their financial status, can significantly impact their world.
You might ask: “What can one person do?” One person can do a lot.
One person can keep 19 families from going without food.
One person can make it possible for three dying mothers to receive a visit
from a child who could not afford to make the trip.
One person can help 11 people with transportation needs so they can get
back and forth to work so they can provide for their family.
One person can ensure 29 others receive the medical care that they so desperately need.
One person can prevent 15 families from losing their homes.
One person can stop 31 families from being evicted by their landlords.
One person can make sure that 73 families keep their utilities connected.
This is a lot for one person. But one person can do a lot more.
One person can provide 198 heaters for senior adults in Georgia
to help them stay warm during
winter.
One person can purchase 450
gifts for 250 senior adults who
would otherwise receive no gifts
during the holiday season.
One person can make sure that 266 homes have at least one fan to circulate a
little air during Georgia’s hot summer days.
One person can build five houses in one week in an economically depressed
area of Georgia.
One person can add a 16x48 foot addition to the home of a great grandmother who cares for the three children of her murdered granddaughter.
Can one person really do all that? Yes, they can, and even more. When one
person joined with over 3,000 fellow associates to make contributions to CHF in
fiscal year 2008, they provided $197,400 in assistance to a total of 1,520 people.
For those people, one person made a world of difference.
