Applicant for licensure as pharmacy technician:
Pay $80 fee; Supply passport style photo; Complete/sign/date application; affidavit of
completed coursework; fingerprinting via Live Scan Service; no felony record; and
have a high school diploma or the equivalent of a high school diploma & one of:
1. Obtain an associate’s degree in pharmacy technology
2. Complete a course of training specified by the board

3. Graduate from school of pharmacy recognized by the board
4. Certified by the PTCB
