Parents may submit their form online, however they have to deposit the required documents and
registration amount of Rs. 25 before the last date, along with the print out of online receipt.
Following documents to be submitted at the time of Admission:
1. Attested Certificate of Birth, issued by the Municipal Corporation.
2. Medical Fitness Certificate of child by registered medical practitioner.
3. Proof of Residence
4. Identity Proof of Parents/Guardian.
5. Proof of Guardianship (If applicable).
6. SC/ST/OBC Certificate (If applicable).
7. In Sibling cases, last fee receipt must be enclosed.
8. Any other documents validating data entered in the form.
9. Four copies of Passport size Photograph of Student.
