Steps to Register for
2015-16 YMCA Licensed
Child Care
1. Fill out all registration forms completely (one packet per child).
2. Turn in the registration forms at the Joe and Mary Mottino YMCA along with the
$50.00 non-refundable per child registration fee, and the first month’s fee
($100 holding deposit will be collected for registration submitted prior to Aug
1st). Children may attend the program two business days after complete registration form has been submitted.
3. Take a parent handbook. It will provide you with essential information about the
program.
4. Payment information:
Fees have been calculated based upon an annualized rate of the total number
of program days divided into10 equal payments (August-May). On days when
school is closed for teacher work days or Holidays, day camp is available at
the Joe and Mary Mottino YMCA for an additional cost. Please visit the
Mottino YMCA website for camp information.
All payments are processed by a bank draft or credit card draft. Bank draft
will occur on the 10th of each month; credit card draft will occur on the 15th
of each month.

The Character Builders program closes promptly at 6:00pm. Any pickup that occurs
after 6:00pm will assess a late fee of $1.00 for every 1 minutes late per child, which

2015-16 Payment Information
1. Parents/Guardians are required to complete the registration forms, including licensing forms
(required by CA state licensing), pay the $50 non-refundable registration fee, and the first month’s
fee per child prior to the child’s registration being accepted into the program.
2. Fees have been calculated based upon an annualized rate of the total number of program days
divided into 10 equal monthly payments. All character Builders payments are processed by bank
draft or credit card draft. Bank draft takes place on the 10th of each month and credit card draft
takes place on the 15th of each month. The first bank draft for the 2015-16 school year will begin
August 10th and 15th. The Payment Tech must be notified of any changes or cancellations of the
Character Builders program 10 days prior to the draft.
3. Please fill out the bottom portion of this document completely. For bank draft we need a voided
check (no deposit slips). For credit card we need the type of credit card, the credit card number and
expiration date.
I hereby authorize the Joe and Mary Mottino Family YMCA to initiate debits to the Bank or Credit Card
indicated below. The authority is to remain in full force and effect until the Joe and Mary Mottino Family
YMCA has received written notification regarding the termination of this agreement. The Payment
Technician must be notified of any changes or cancellation of the Character Builders program at least
10 days prior to my draft. I understand there are no proration or refunds given and that it is my
responsibility to check my monthly bank statement and report any corrections immediately to the Joe
and Mary Mottino Family YMCA. I also understand that I will be charged a $25 fee for any returns.

I, the undersigned parent/person having legal custody/guardianship of the above said minor, give
permission for the minor to participate in all YMCA programs. The minor is physically able and
mentally prepared to participate in all activities as described in the announcement for the program.
In consideration of said minor being permitted to enter any branch of the YMCA of San Diego
County (“YMCA”) for observation, use of facilities and/or equipment, or participation in any program,
I, on behalf of myself (as parent, guardian, coach, aide, spectator or participant) hereby:
1. Acknowledge that (i) I have read this document, (ii) I have inspected the YMCA facilities and
equipment, (iii) I accept them as being safe and reasonably suited for the purposes intended and (iv) I
voluntarily sign this document.
2. Release the YMCA, it’s directors, officers, employees and volunteers (collectively “Releasees”) from
all liability to me for any loss or damage to property or injury or death to person, whether caused by
Releasees or otherwise and while such minor is in or near any YMCA branch.
3. I agree not to sue Releasees for any loss, damage, injury or death described above and I will
indemnify and hold harmless Releasees and each of them from any loss, liability, damage or cost they
may incur due to said minor’s presence in, upon or near the YMCA’s branch; whether caused by the
negligence of Releasees or otherwise.
4. I assume full responsibility for, and risk of, bodily injury, death or property damage due to the
negligence of Releasees or otherwise.
5. I do hereby authorize the YMCA as agent for the undersigned, to consent with respect to said
minor, to any x-ray examination, anesthetic, medical, dental, or surgical diagnosis or treatment, and
hospital care which is deemed advisable by and is to be rendered under general or special
supervision of, any physician and surgeon licensed under the provisions of the California Medical
Practice Act on the medical staff of any hospital, whether such diagnosis or treatment is rendered at
the office of the physician or at the hospital. I understand that the YMCA is not responsible for
costs incurred for medical care.
I intend this document to be as broad and inclusive as is permitted by the laws of the State of
California; if any portion hereof is held invalid, I agree the balance shall continue in full force and
effect.
