Analog Signal Access Interface This optional
feature provides real time analog voltage
representations of torque, position and velocity
from the dynamometer.
Customized Motor Control The "Researcher’s
Tool Kit" provides the advanced investigator
direct access to the embedded dynamometer

application software. The core of the Tool Kit
is an extension to the System 4 firmware that
exposes the motor controls to an external

the existing equipment floor plan. Install as a
simple loop for continuous ambulation over
stairs or through parallel bars, or add side

FreeStep SAS can be installed… wood beam,

This simple pulley system provides a 3/1 mechanical advantage

Testing Protocol:
The testing protocol consisted of three 20 second trials using the postural stability testing feature
within the BSD software. During each test trial, the platform gradually became less stable at four
second intervals. The initial stability level was set at level 12 and ended at level 8. Participants
were given a 10 second rest between each of the three trials.

ICD-9 Code: V15.88 Personal History of Fall
The Centers for Disease Control and Prevention (CDC) and Centers for Medicare and Medicaid
Services (CMS) have created a code, V15.88 History of Fall, on the list of diagnostic codes
implemented on October 1, 2005. They also listed At Risk for Fall as an inclusion term. Both
terms will be indexed to direct the user to the code V15.88. A specific fall code should
heighten health care provider awareness of the importance of falling as a manageable medical
problem, and enhance the likelihood that fall risk evaluation and management services would
be considered medically reasonable and necessary services, and therefore covered.

Mercy Rehabilitation Hospital in Daphne,
Alabama now offers a high-tech approach for
identifying risk factors for falls among older
adults. The Biodex Balance System provides
fast, accurate fall risk screening and
conditioning for older adults. In addition, the

assessment and training for lower extremity
patients. According to Jeff Nobles, director of
rehabilitation services at Mercy, “The Biodex

The Gardens Court is a 120 resident facility
located on the east coast of Florida, part of the
Life Care Centers of America. Besides The
Gardens Court, Life Care operates more than
200 skilled nursing, assisted living,
retirement, home care and Alzheimer’s centers
in 28 states - from Boston, Massachusetts to
Hilo, Hawaii. The Garden Court has the
Biodex Balance SD, as do sister facilities Darcy
Hall Nursing and Rehabilitation in West
Palm Beach and Lakeside Health Center also
in West Palm Beach.
A patient’s score reflects
how many times a target
is hit. Training modes include maze control
training, stability training, random control
training, weight shift training, weight bearing
and game-like balance training. A fall screening
test protocol compares balance test results to
age- and gender-dependent data consistent with
American Geriatrics Guidelines for Fall Risk
Assessment. The athlete knee injury screening
test is consistent with leading sports medicine
programs. One of Mercy’s rehabilitation
patients who utilizes the balance system said,

The Biodex Balance System is used for
patients with a number of common diagnoses
including fall risk in older adults as well as
sports/orthopedics injuries, ankle sprains and
fractures, amputation and joint replacement.
In addition, the system is useful in treatment
of movement and neurological disorders
including stroke, head injury, Parkinson’s
disease, muscular dystrophy, multiple
sclerosis, balance/coordination /postural
weaknesses and cerebral palsy. Advantages of
the Biodex Balance System over traditional
balance therapy include biofeedback, objective
documentation, measurement and reporting of
postural stability and limits of stability, fall
risk assessment and normative data. Another
advantage is the system’s ability to offer both
static and dynamic capabilities.
“The degree of recovery a patient can achieve is
greater on a dynamic system than on a static
system,” Nobles added. “Traditional low-tech
methods can’t compare to the benefits of

to patients on an inpatient and outpatient basis.

One of our patients, Josephina, a spunky
resident of 65+; had a severely broken leg
caused by an accident with two large dogs.
She underwent surgery in January 2008 where
doctors inserted pins and a plate into
Josephina’s leg and released her to The
Gardens Court for rehabilitation. Josephina
was non-ambulatory for over 8 weeks prior to
beginning rehab; she was depressed and
anxious to “get better”… to quote Josephina
“I can walk again, this machine is amazing, a
miracle...it is hard sometimes, but that is
good, it makes me work and I can see myself
getting better.”

At The Beltrone Living Center in Colonie,
NY, an independent living facility operated by
Colonie Senior Services, a new model for
physical therapy has been in business for about
four months. Physical therapist Hector Jasen
operates HectorPT, a fullservice physical therapy
office that offers Beltrone
residents, and the public, a
fully-equipped physical
therapy facility that features
traditional PT services, and
a Balance System that no
one else in the Capital
District has to offer.
“In-house therapy is
unique,” says Jasen, “It’s definitely the new
trend in retirement communities. Therapists
can follow up with clients, and keep track of
changes. Sometimes it does not take much for
the elderly to decline and this way, things are
taken care of right away and a problem does not
escalate,” he explained. Neary and Jasen also
stressed that the services offered at HectorPT
are available to the public, not just to those
residing at the Beltrone. Having an office
located within an independent living facility is
not the only thing that makes HectorPT
unique.
Jasen puts the focus of his practice on
preventing the injuries that often send people
to nursing homes, robbing them of their
independence. Jean Myers, a Beltrone
resident, has fallen three times, and the last
one was a bad one. It left her with
polyneuropathy, a loss sensation in her feet,
and a lot of fear. Three weeks after that fall,
she stands on Jasen’s balance machine, and is
learning how to become stabilized and
strong. Because she does not have good
control of her feet, the ability to see how she
stands enables her to learn how to correct her
balance, and what proper balance feels like.
Jasen will then help her to translate that
feeling to real-world walking. Already, after
only a couple of sessions with Jasen, Myers
said she feels steadier on her feet than she has
dependent.

Step 2:
Preseason Baseline Testing
The ability to quantify balance and cognitive function
in athletes before an injury occurs is an important
consideration in a comprehensive athletic program.
The objective data provided by computerized
assessments provides a performance baseline against
which post-injury performance can be compared.

balance data on all athletes using the Biodex Balance

The Clinical Test for Sensory Integration of Balance
(CTSIB) helps to determine which sensory system
(visual, vestibular, or somatosensory) an athlete relies on
to maintain balance. Research has shown that Mild
Head Injury (MHI) populations rely more on visual cues
from the environment than an able body population. 4

any computerized cognitive software testing program

Objective balance testing is critical
Research shows that athletes demonstrate decreased
stability up to three to five days post injury. This can
be the result of ineffective use of one or more of their
sensory systems.1 Signs and symptoms will vary
following head trauma. With some individuals, signs
and symptoms may resolve immediately after a mild
head injury (MHI) while others have persistent
symptoms. In either case, the recovery period related
to MHI appears to coincide with recovery of postural
stability as well as normalization of eye-head
coordination and return of cognitive function. 1
Recently, there is strong evidence demonstrating the
impact of balance deficits on functional performance
and increased risk of re-injury. (2,3,4)

Another component of normal
gait is pelvic rotation. Twopoint suspension systems have a
tendency to restrict rotation on
the horizontal plane. They

“One example of neural plasticity is the improvement seen in locomotion
of patients with stroke or spinal cord injury through partial body weight
supported treadmill training. This rapidly
developing form of therapy is based on two
fundamental observations. First, that the spinal
cord of all vertebrates, including man, contains
neuronal circuitry that encoded the locomotor
pattern, and which can be activated by
stimulation at supraspinal, suprasegmental, or
segmental levels or even by intrathecal drug
administration. The activation of locomotor
circuitry does not therefore require momentto-moment control of individual muscles by
the brain. The second observation was that
repeated activation of the central pattern
generating circuit by treadmill training could
result in long lasting enhancement of locomotor
output. In human trials, this resulted in improved locomotion
months and even years after training was discontinued.”
