Message from the Chair
What a privilege it has been to serve as the Chair of North Hamilton Community Health Centre over
these last three years. The gifted and capable Board, dedicated and competent Leadership team,
compassionate and skilled staff and exceptional Executive Director has made it a joy to serve in this
capacity. We have the opportunity to journey with over 9,000 Hamiltonians and our nearly 90 staff
as we long to realize our vision of “No Obstacles to Health”. We are thrilled with the numerous
accomplishments in our recent history which includes the launching of Pathways to Education, our
Feet First: Steps for Health initiative, our IT partnerships with other Community Health Centres and
of course the stunning new facility that allows us to offer better care and enhanced programs for our
community.
We are thankful to the Ministry of Health and Long-Term Care for their funding of this facility. We also want to
acknowledge the support we have received from our Hamilton Niagara Haldimand Brant Local Health Integration Network
(LHIN), local politicians and the City of Hamilton during this process. Our crowning accomplishment however isn't found
in a program or even in the completion of a new facility. Our crowning accomplishment is the privilege we have to walk
with neighbours, community members, families and individuals in a moment of need, distress, loneliness or pain and watch
someone's crushing burden be lifted so they can continue to live life well, achieve their dreams and help others do the
same. That's what I love most about North Hamilton Community Health Centre. In all the meetings I have sat in, and all the
committees I have been a part of, and all the conversations I have participated in, the core has always been the same...we are
here for this community.

Message from the Executive Director
I invite you to our new facility! Not to see the fantastic architecture, not to learn about how we are
wonderfully energy efficient, not to see all the open meeting and community space. I invite you to
see what is possible.
This is the first Annual Report in several years where we haven’t identified space as a barrier to goal
achievement. We embark on the “next steps”.
I invite you to the next steps of what is possible – give us your opinion, become a volunteer, join a
program, participate in our education sessions.
I invite you to the next steps of what is possible – take your health in your own hands. Come to our kinesiologist-led, costfree fitness centre, meet with our Asthma specialist, talk with our Chronic Disease Self Management staff. All of whom will
teach you to develop goals, to understand small steps that lead to large successes, and to recover if your goals are not met.
I invite you to the next steps of what is possible – stand and view our future. Watch the children of the community who play
in the adjacent playgrounds, we are convinced they will be our future doctors, nurses, social workers, educators, and
administrators. They are what is possible.
We say goodbye to four Board members this year – Patti Idenouye, Nancy Leslie, Brian Pogson, and Florine Lobo. They
have contributed significantly to the strategic governance of the Health Centre and we are tremendously grateful. I wish to
extend a special thank you to Dwayne Cline, our outgoing Board Chair, who has challenged, inspired, and been a positive
force for change. He is one of those individuals with whom you witness the “greater good” when in his presence. You will be
greatly missed.
I want to take this opportunity to thank an exceptional team of staff and volunteers who work tirelessly with our
communities to ensure that we are enabling health through healing, hope and wellness. Board, staff and volunteers continue
to be inspired by our community – their strength, innovation and resiliency. It is a privilege to be part of your health
journey.

Health Wellness
2010/2011 was an eventful year for the Health Wellness Team. The team grew significantly with the addition of many new
staff members and programs. Here are some of the new initiatives that made the past year a very memorable one for the
Health Wellness team and the Health Centre.
Diabetes Education
· In January 2011 the new diabetes expansion program began in
partnership with Hamilton Urban Core CHC and the Shelter Health
Network to provide diabetes education and self-management for people
living in at-risk communities including shelters in the city core.
·
Health Promotion
· The garden team is looking forward to
an extended growing season this year
with the building of a new greenhouse made possible by a generous donation from the
Hamilton Port Authority. Twelve new garden beds will also be built on-site for
children’s programming.
Last winter was very cold and programs providing assistance to low income families for
utility bills were busier than usual. Applications were up 97% to 645 and number of
households helped was also up 48% to 212 from last year.
Rehabilitation and Fitness
· Fitness and rehabilitation clients are delighted with the
new facility, especially the gymnasium area. Diabetes
and community fitness programs have seen a major increase in attendances compared
to the same time last year.
·
Feet First: Steps for Health
· The Feet First Program expanded to the
Niagara region with the establishment of sites
in Welland and Niagara Falls, Bridges CHC in
Fort Erie and Port Colborne, and Quest CHC
in St. Catharines.
· New Hamilton satellite clinics opened at the
New Village Retirement Home in Stoney
Creek and the Joseph Brant Community Health Centre in Burlington.
By year’s end the program provided evidence based foot care to over 2,300 atrisk diabetic clients throughout the HNHB
LHIN.

Primary Health Care
Over the last year, the structure of the Primary Care team was
expanded to include a team of mental wellness professionals in addition
to the physicians, nurses, physician assistants, lab technician, clinic aide,
pharmacist consultant, and reception staff. The enhancement of the
team composition will allow us to better meet the social, advocacy and
mental health needs of our clients.
Our Pharmacist Consultant, Janie Bowles-Jordan, was invited to
establish a Regional Medication Management Clinic that will serve
patients in the Kitchener-Waterloo area and act as the
University of Waterloo School of Pharmacy teaching site.
This exciting opportunity allows a Pharmacist working
the Community Health Centre sector to be involved in
the training of future pharmacists.
Health Information Systems
Integrated Decision Support Tool (IDS)
· IDS is an innovative partnership with Hamilton
Health Sciences that enables us to see the health
care journey of our clients.
· With the IDS tool now in place, our Health Centre
and other HNHB LHIN Community Health
Centres can identify health enhancing opportunities
based on their health care utilization.
Evaluation Framework
· As part of an overall quality improvement initiative, a self-management
program will be piloted among the most frequent users of primary health
care at our Health Centre.
· We hope that a self-management program will lead to an improvement in
health outcomes for individuals who require greater care.
·
The next generation Clinical Management System
· North Hamilton Community Health Centre was the first CHC to
transition from paper to an electronic medical record system in 2006.
All Community Health Centres (CHCs) will have access to a new state-of-the-art electronic medical record (EMR),
which will provide a powerful new tool for the 3,100 members of CHCs’ inter-professional teams who will be using the
system to care for clients efficiently and effectively.
Pathways to Education
This program serves high school students in the Health Centre’s catchment area namely, the Bennetto and Keith neighbourhoods, by providing four key supports:
Academic - Tutoring program twice a week, Social - Group, Community and
Career Mentoring to develop life skills and build relationships, Financial - financial
support such as bus tickets or lunch vouchers and a scholarship for post-secondary
education (up to $4,000 per student in the program), Advocacy - Student-Parent
Support Workers help connect teens, parents, school administration, teachers and
community agencies. This is all in an
effort to ensure the youth in our
community complete high school and
continue on to post secondary
programs.
2010-2011 Program Highlights
Recruitment – Pathways Hamilton has currently reached 73%
enrolment of grade 9 and grade 10 eligible students bringing our total
registered students up to 173 students from both school boards!
Community
Support –
Pathways has a terrific team of approximately 70 adult
volunteers, 30 Community Partners and 14 school partners
working together to ensure students reach their maximum
potential in school and in life.

Shaping future professionals – Career preparation is an
ongoing theme in Pathways Mentoring sessions. This summer
the Health Centre will be providing one of our Pathways
students the opportunity to be employed for 6 weeks as a paid
summer student.

Innovation of the Year Awards
Staff – Lexi Jones-O’Rourke for introducing the Good Food Box program to clients
and staff at the Health Centre. In partnership with Welcome Inn and Environment
Hamilton, the program provides participants with healthy fruits and vegetables at
significantly lower prices every month. Congratulations Lexi!
Honourable Mention – Terri Petrachek for suggesting a shuttle
bus to bring clients from our John Street building to our new
facility on Hughson Street for the first two weeks after our move.

Health Centre’s new mission statement of “to enable

consultation, our Health Centre was able to save thousands

In June 2010, Shelley Esposto, Diabetes
Nurse Educator, was awarded the
national Public Service & Education
Award by the Canadian Diabetes
Association. This award recognizes the
efforts of diabetes educators who have
done exceptional work in promoting
awareness of diabetes and developing
programs to assist people who live with
diabetes.
