CASE MANAGEMENT
KENTUCKY HOMEPLACE
Kentucky Homeplace was created to help address issues of chronic
diseases, such as diabetes. The program offers the support of a
Community Health Worker (CHWs) for individuals who meet the
program's criteria. The CHWs are trained to help medically
underserved residents access appropriate health services. Emphasis is
placed on preventive care, health education and disease selfmanagement. CHWs provide an important bridge between clients with
the greatest needs and the primary care physicians and other health
providers in the community. They facilitate communication between
an individual and primary care physicians, help the clients learn to
effectively comply with medical care instructions, and help educate
clients to improve their health behaviors, such as improved nutrition,
increased physical activity, better weight management, smoking
cessation, and improved diabetes self-management. Call for more

Kentucky Homeplace was created to help address issues of chronic
disease such as diabetes. The program offers the support of a
Community Health Worker (CHW) for individuals who meet the
program's criteria. The CHWs are trained to help residents access
appropriate health services. Emphasis is placed on preventive care,
health education and disease self-management. CHWs provide an
important bridge between people with chronic disease and the
physicians and services available in the community. They help
individuals to better understand the steps they need to take to manage
their diabetes every day. Call for more information about services,
costs, scheduling and to see if you qualify for the program.

The class currently meets at the Floyd County Health Department at 5
PM on Tuesdays. If unable to come in at that time individuals may
come in on Tuesdays between the hours of 10 a.m. and 5 p.m. for oneon-one counseling. The program follows the Cooper Clayton format.
The classes vary in time depending on participation. The program
consists of an introductory class, 10 weeks of nicotine patch therapy
and counseling, and 2 weeks of follow-up patches are provided for
free.

This program is an American Lung Association’s Freedom from
Smoking course. Each week teaches about different triggers that
cause someone to want to reach for a cigarette and how to avoid such
triggers. It also helps in regards to managing stressors and why people

The Cooper Clayton Smoking Cessation Course meets once a week
for one hour over a 13 week period. The class structure is as follows:
discussion among participants and counselor, a video presentation and
discussion of video. Each client is encouraged to quit smoking.

prefer one-on-one counseling they may contact Rita Dials for more
information. Nicotine replacement is provided in the form of patches,

What is diabetes?
There are three main types of diabetes:



Type 1 diabetes Your body does not make insulin.
This is a problem because you need insulin to take the
sugar (glucose) from the foods you eat and turn it into
energy for your body. You need to take insulin every
day to live.
Type 2 diabetes Your body does not make or use
insulin well. You may need to take pills or insulin to
help control your diabetes. Type 2 is the most common
type of diabetes.
Gestational (jest-TAY-shun-al) diabetes Some women
get this kind of diabetes when they are pregnant. Most
of the time, it goes away after the baby is born. But even
if it goes away, these women and their children have a
greater chance of getting diabetes later in life.
Diabetes is COMMON and INCREASING

Today, 1 in 10 adult Kentuckians have been diagnosed
with diabetes. In 1995, this number was only 1 in 25!

It is estimated that as many as 1 in 40 Kentucky adults
have diabetes but have not been diagnosed, leaving
them at risk for serious complications.

40 percent of Kentuckians age 40-74 have prediabetes and are at very high risk for developing the
disease (pre-diabetes means blood sugar levels are
elevated but not high enough to be classified as
diabetes).

Diabetes is SERIOUS

Diabetes is the leading cause of blindness, kidney failure
and amputations.

Diabetes is the fifth leading cause of death by disease.
Diabetes is CONTROLLABLE

Complications such as heart attack, stroke, blindness,
kidney failure and amputation can be greatly reduced
with proper medical care, good nutrition, modest
physical activity and self-management education.
TYPE 2 Diabetes can be DELAYED or PREVENTED

For those at risk for diabetes, lifestyle changes such
as better nutrition and increased physical activity can
decrease risk by almost 60 percent.
DIABETES SCREENING
Diabetes is diagnosed from a blood test with or without
signs or symptoms of disease. The American Diabetes
Association has outlined the following blood glucose
values to establish a diagnosis of pre-diabetes or
diabetes:

Signs and Symptoms
Of Type 2 Diabetes. Many people have no
signs or symptoms. Symptoms can also be so
mild that you might not even notice them.
Nearly six million people in the United States
have type 2 diabetes and do not know it. Here
is what to look for:







Increased thirst
Increased hunger
Fatigue
Increased urination, especially at night
Weight loss
Blurred vision
Sores that do not heal
What does it mean to have
PRE-DIABETES?
It means that you are at risk for getting
type 2 diabetes and heart disease. The
good news is if you have pre-diabetes you
can reduce the risk of getting diabetes and
even return to normal blood glucose levels.
With modest weight loss and moderate
physical activity you can delay or prevent
type 2 diabetes. If you blood glucose is
higher than normal but lower than the
diabetes range (what we now call prediabetes), have your blood glucose checked
in 1 to 2 years.

Keys to Prevention
The key to success is in actually preventing prediabetes and Type 2 diabetes to begin with.
Identifying risk means asking your- self the following
key questions:
 Am I aged 45 years or older?
 Am I overweight?
 Do I have high blood pressure or cholesterol?
 Do I have a family history of diabetes- ?
 Am I African American, Hispanic, American
Indian or Asian?
 Do I have a history of diabetes occurringduring pregnancy?
 Did I deliver a baby weighing more than 9
pounds?
If you answered ‘yes’ to any of the above questions
then you should be screened but also reduce your risk
of pre-diabetes and Type 2 diabetes by implementing
the following:
 Look for opportunities to move more during
the day
 Exercise 30 minutes at least five times per
week
 Eat a healthy meal plan including grains,
cereals, fresh fruit and vegetables, low fat dairy
and lean meat
 Reduce fat intake
 Reduce food portions
 Maintain an ideal body weight, especially
around your waist (Monitor this with a tape
measure)

Move more.
Getup, get out and get moving. Try walking,
dancing, bike riding, swimming, or playing ball
with your family or friends. It doesn’t matter
what you do as long as you enjoy it.
Eat the healthy plate way.
Focus on eating less and making healthy food choices. Cut down on
fatty and fried foods. You can still eat the foods
you enjoy, just eat less. When eating a meal, fill
half of your plate with fruits and vegetables, one
quarter with a lean protein, such as beans, or
chicken or turkey without the skin, and one quarter
with a whole grain, such as brown rice or whole
wheat pasta.

Take off some weight.
Once you start eating less and moving more, you
will lose weight. If you are just slightly
overweight, or even obese, by losing even 10
pounds, you can cut your chances of getting
diabetes.
Set goals you can meet.
Start by making small changes. Try being active for 15 minutes a day
this week. Then each week add 5 minutes until you build up to 30
minutes, 5 days a week. Try to cut 100 calories out of your diet each
day (that’s one can of soda!). Slowly reduce your calories over time.
Talk to your health care team about your goals.
Record your progress
Write down all the things you eat and drink and
the number of minutes you are active. Keeping a
diary (or using a smart phone diary app) is one of
the best ways to lose weight and keep it off.
Get help.
You don’t have to prevent diabetes alone. Ask
your family and friends to help you out. Involve them in your
activities. You can help each other move more, eat less, and live a
healthier life. There are groups in your county/area that can help, as
well as your health care team.
Keep at it.
Making even small changes is hard in the beginning.
Try adding one new change a week. If you get off
track, start again and keep at it.

How to learn more about diabetes.
 Take classes to learn more about living with
diabetes. To find a class, check with your health
care team, hospital, or area health clinic. You can
also search online.
 Join a support group — in-person or online — to
get peer support with managing your diabetes.

Take diabetes seriously.
You may have heard people say they have touch of
diabetes” or that their “sugar is a little high.” These
words suggest that diabetes is not a serious disease.
That is not correct. Diabetes is serious, but you can
learn to manage it.
People with diabetes need to make healthy food
choices, stay at or get to a healthy weight, move
more every day, and take their medicine even
when they feel good. It’s a lot to do.
It’s not easy, but it’s worth it!

Taking care of yourself and your diabetes can help
you feel good today and in the future. When your
blood sugar (glucose) is close to normal, you are
likely to:

have more energy
be less tired and less thirsty
need to pass urine less often
heal better
have fewer skin or bladder infections
You will also have less chance of having health
problems caused by diabetes such as:





heart attack or stroke
eye problems that can lead to trouble seeing or going
blind
pain, tingling, or numbness in your hands and feet, also
called nerve damage
kidney problems that can cause your kidneys to stop

A for the A1C test (A-one-C).
What is it?
The A1C is a blood test that measures your average
blood sugar level over the past three months. It is
different from the blood sugar checks you do each
day.
Why is it important?
You need to know your blood sugar levels over
time. You don’t want those numbers to get too
high. High levels of blood sugar can harm your
heart, blood vessels, kidneys, feet, and eyes.
What is the A1C goal?
The A1C goal for many people with diabetes is
below 7. It may be different for you. Ask what
your goal should be.

B for Blood pressure.
What is it?
Blood pressure is the force of your blood against
the wall of your blood vessels.
Why is it important?
If your blood pressure gets too high, it makes your
heart work too hard. It can cause a heart attack,
stroke, and damage your kidneys and eyes.
What is the blood pressure goal?
The blood pressure goal for most people with
diabetes is below 140/90. It may be different for
you. Ask what your goal should be.

C for Cholesterol (ko-LESS-tuh-ruhl).
What is it?
There are two kinds of cholesterol in your blood: LDL and
HDL.
 LDL or “bad” cholesterol can build up and clog your
blood vessels. It can cause a heart attack or stroke.
 HDL or “good” cholesterol helps remove the “bad”
cholesterol from your blood vessels.
What are the LDL and HDL goals?
Ask what your cholesterol numbers should be. Your
goals may be different from other people. If you are
over 40 years of age, you may need to take a statin
drug for heart health.

Learn how to live with diabetes.
It is common to feel overwhelmed, sad, or angry when you
are living with diabetes. You may know the steps you should
take to stay healthy, but have trouble sticking with your plan
over time. This section has tips on how to cope with your
diabetes, eat well, and be active.

Stress can raise your blood sugar.
Learn ways to lower your stress. Try
deep breathing, gardening, taking a
walk, meditating, working on your
hobby, or listening to your favorite
music.
Ask for help if you feel down. A
mental health counselor, support
group, member of the clergy, friend,
or family member who will listen to
your concerns may help you feel
better.
Eat well.





Make a diabetes meal plan with help from your health care
team.
Choose foods that are lower in calories, saturated fat, trans
fat, sugar, and salt.
Eat foods with more fiber, such as whole grain cereals,
breads, crackers, rice, or pasta.
Choose foods such as fruits, vegetables, whole grains, bread
and cereals, and low-fat or skim milk and cheese.
Drink water instead of juice and regular soda.

When eating a meal, fill half of your plate with
fruits and vegetables, one quarter with a lean

Be active.

Set a goal to be more active most
days of the week. Start slow by
taking 10 minute walks, 3 times a
day.
Twice a week, work to increase your
muscle strength. Use stretch bands,
do yoga, heavy gardening (digging
and planting with tools), or try pushups.
Stay at or get to a healthy weight by
using your meal plan and moving more.
Know what to do every day.

Take your medicines for diabetes and any other
health problems even when you feel good. Ask your
doctor if you need aspirin to prevent a heart attack
or stroke. Tell your doctor if you cannot afford your
medicines or if you have any side effects.

Check your feet every day for cuts, blisters, red spots, and
swelling. Call your health care team right away about any
sores that do not go away.
Brush your teeth and floss every day to keep your mouth,
teeth, and gums healthy.
Stop smoking. Ask for help to quit. Call 1-800-QUITNOW (1
-800-784-8669).
Keep track of your blood sugar. You may want to check it one
or more times a day. Use the card at the back of this booklet
to keep a record of your blood sugar numbers.
