Additional information.
The training requirement will consist of a total of three (3) days of Invisalign Training (Training Course 1, Training Course 2 and MasterClass*) (at a cost of: €1,500**) (hereinafter the “Invisalign Training
Course”). The Programme Period is a fixed period of time of twelve (12) months from the date the Doctor attended the first Invisalign Training Course. The Programme Acceptance Date (hereinafter
“Acceptance Date”) is the date the Doctor accepts the Invisalign Training Course Terms and Conditions through the Invisalign Doctor Site (IDS). The discount offer set forth below will become effective as
of Acceptance Date until the Programme Period end date.
* The Invisalign Training Course fee includes the three (3) days of training. **All pricing is subject to the local taxes applicable for that tax jurisdiction.
Discount scheme.
All Qualifying Treatments, as defined below, submitted during the Programme Period and approved after Acceptance Date, will be automatically invoiced as follows: At a 35% discount for the first six (6)
months of the Programme Period. At a 20% discount starting the seventh (7th) month of the Programme Period provided that during the first six (6) months at least three (3) Qualifying Treatments were
ClinCheck® approved. At list price starting the seventh (7th) month of the Programme Period if during the first six (6) months a minimum of three (3) Qualifying Treatments were not ClinCheck approved.
Vivera® retainer* orders during the Programme Period and after Acceptance Date will be automatically invoiced as follows: At a 20% discount for the first six (6) months of the Programme
Period. Continuing at a 20% discount starting the seventh (7th) month of the Programme Period provided that during the first six (6) months at least three (3) Qualifying Treatments were ClinCheck
approved. At list price starting the seventh (7th) month of the Programme Period if during the first six (6) months a minimum of three (3) Qualifying Treatments were not ClinCheck approved.
*Vivera orders do not count towards the three (3) Qualifying Treatment requirements for the first six (6) months of the agreement.
The following are excluded and do not qualify for a discount under the Programme Period and are not considered as Qualifying Treatment or within the calculation for applying the discount: i7, mid-course
corrections, Refinements, Replacement Aligners, Treatment Cancellation fees, VAT, Any other Align treatment option or product not expressly listed in the definition for a Qualifying, Discount Treatment,
Shipping and handling, Submissions through a University Programme / Research Institution.
A “Qualifying Treatment” is one of the following treatment options for which the ClinCheck treatment plan has been submitted and approved during the Programme Period (submission must include: all records
and materials required for treatment including the online prescription form, PVS impressions or intra-oral scans and online photos, hereinafter “Required Records”): (i) Invisalign® Full treatment, (product code
8003) (ii) Invisalign® Teen (product code 8019) (iii) Invisalign® Lite treatment (product code 7887). Submissions without all Required Records will NOT be counted as a Qualifying Treatment. Any Qualifying
Treatment cancelled during the Programme Period will be charged according to the current "Pricing, Terms and Conditions for Invisalign". Actual list price may change from time to time at Align’s sole discretion.

Terms and conditions.
1) Certain restrictions apply. The Programme cannot be combined with any other Invisalign discount Programmes or offers. University Residents and members of Research Institutions are not eligible to
participate. Only practices in Austria, France, Germany, Ireland, Italy, Liechtenstein, Spain, Switzerland and the UK are eligible. 2) Align reserves the right to terminate the Programme in whole or in part,
or stop accepting further participants for the Programme, or disallow a doctor to continue in the Programme at any time and at its own discretion which may result in no further discounts being provided
under this Agreement. 3) Only doctors who are in good standing with Align (as solely determined by Align), including any of Align’s affiliates, and compliant with all Align and Align affiliates policies will be
eligible to participate. Eligibility for this or any other Align Programme is entirely at Align’s discretion. 4) In order to register for an Invisalign Training Course, a payment of €1,500.00 must be received
by Align no later than ten (10) working days prior to the first Training Course date. 5) Align may add or remove treatment options, training courses, and/or products that qualify under this Programme at
any time at its discretion. 6) Doctor attendance at the three (3) days of the Invisalign Training Course is required. Cancellation of Doctor attendance for any portion of the Invisalign Training Course must
be notified ten (10) business days prior to the start of the Invisalign Training Course selected by the Doctor. If written cancellation is received within thirty (30) days of the scheduled Training Course,
the Doctor has the choice of receiving a refund of the €1,500.00 training fee or rescheduling the Invisalign Training Course to another date within twelve (12) months of the first Training Course date,
subject to the provision of an IBAN code. If the Doctor provides written cancellation not less than ten (10) business days prior to the Invisalign Training Course, the Doctor may reschedule the course
to another Invisalign Training Course within twelve (12) months of the first Training Course date without any penalty. 7) Failure to attend the Invisalign Training Course selected by the Doctor without
notification to Align constitutes forfeiture of the full Training Course fee paid. 8) No refund or discount will be provided for partial attendance of, or cancellation of the second or third day of the Invisalign
Training Course. 9) The Doctor may not attend the second or third day of the Invisalign Training Course until completion of the first day of the course. Due to the nature of the second and third days of
the Training Course – the aim is to have the same speaker and the same group of attendees for consistency and to aid the Doctor’s continuum of learning – Align advises to attend the full Training Course
the Doctor has selected. If the Doctor is unable to attend the corresponding second or third day, he/she will need to contact the local sales office to arrange another date. The training received at each
day of the Training Course will focus purely on the use of Invisalign in the treatment of orthodontic patients. No orthodontic theory will be presented. 10) Align reserves the right to change dates and/
or location of the Training Courses, the fees of the Training Course, the speakers presenting or cancel a training course in whole or in part without notice, at any time, at its sole discretion, and with or
without cause. In the unlikely event of a cancellation of an Invisalign Training Course by Align, fees will be refunded if an alternative Training Course cannot be scheduled. Where possible, Align will arrange
an alternative Training Course and if Doctor agrees to the alternative Training Course or agrees to attend another Training Course on another date then no fees will be refunded to the Doctor. Align is
not responsible for any other expenses such as travel, food, accommodation or other expenses that the Doctor may incur. 11) Payment of aligners is due within thirty (30) days of invoice date. The invoice
date is the day the aligners are shipped to the Doctor. 12) Discounts only apply as stated herein and the discount only applies to the initial treatment fee. 13) If a Qualifying Treatment is cancelled after the
ClinCheck treatment plan is approved, then the Doctor agrees to pay the applicable treatment fee. If the treatment fee is received during the Programme Period, then the treatment will be considered a
Qualifying Treatment. If a treatment is cancelled prior to the ClinCheck treatment plan approval and before the end of the Programme Period, the then current treatment cancellation fee will be owed by
the Doctor plus the VAT for the treatment, however the treatment does not count as a Qualifying Treatment and will not be entitled to any discount hereunder. 14) Doctors can only submit his/her own
patients’ treatments. Doctors cannot transfer treatments from this Programme to other Doctors or submit other Doctors’ treatments to his/her account. Any treatment submitted after the expiry of the
Programme Period will not be eligible for a discount hereunder and will be invoiced at the then current list price for the treatment as set forth in the then current Pricing Terms and Conditions for
Invisalign for Align Technology, B.V. 15) The then current terms of the Pricing, Terms and Conditions for Invisalign for Align Technology B.V. apply for each sale under this Programme. To review these conditions,
please visit www.aligneu.com. 16) This Programme is subject to the laws of the dentist’s/orthodontist’s country of practice as indicated in the agreement (excluding the United Nations Convention of the
International Sale of Goods). The parties hereby submit to the jurisdiction of the courts of the dentist’s/orthodontist’s country of practice. 17) The contents of the Programme is considered Align Confidential
Information and must be protected as such. This Agreement and any of the terms hereunder may not be disclosed to any third party without Align’s written consent. All Align intellectual property is
owned by Align and Align is not providing any license rights therein. All materials produced or distributed by Align in connection with this Programme are owned by Align and no duplication, recording
of the Programme or any part thereof, or publication or distribution is allowed without Align’s written consent. Any violations will be prosecuted to the full extent of the law. 18) Void where prohibited.
