The Mission of the CSDA Council on Continuing Education is to provide for
the continuing professional education and advancement of dentists and
allied personnel to enhance their skills and provide the highest quality of
care for the public for which they serve.

OBTAINING YOUR CE CREDITS
For your convenience, the CSDA now offers a separate morning CE verification code worth 3 credits at
all of the Package Program courses. This will allow you to still get partial credit if you cannot stay until
the end of the program. If you do stay for the full day, you will need both the AM and PM codes to obtain
the full six (6) hours of credit.
Name badges are scanned at our programs for CE tracking
purposes. If you are a CSDA or ADA member, your attendance gets
uploaded to the “CE Tracking System” area of the ADA website. It is
up to each individual to obtain/keep record of the corresponding
verification code(s) in order to prove your attendance to any
professional/governing organization that may request your
continuing education records. The CSDA recommends that you
always keep your CE records in hard copy as a back-up and paper
CE Forms are always made available on-site. CE Forms can be
found on the table at the entryway to the lecture room.
Please remember: It is your responsibility to make note of the verification
code(s) in order to prove your attendance to any professional/governing

POLICIES
The CSDA Continuing Education Council reserves the right to cancel any course, limit the enrollment, modify the announced
course content, and/or change the location, time, date or Speaker as may be deemed necessary or advisable. (Please stay tuned to
monthly brochures and/or CDSA.com for the most updated information).
In the event of unusual circumstances, a continuing education course will proceed as long as the speaker and one moderator are
present. If a fall program is cancelled, a December date may be used, if possible. If a winter program is cancelled, a May or June
date may be used.
All course registrations must be received by the CSDA Central Office by 4:00 p.m. on the Friday immediately preceding the course
date. After this point, walk-in fees apply. Proper payment in full must accompany all registrations.
CANCELLATION POLICY: An individual who signs up for a single course (rather than the full Package) may request a refund from
the Central Office up to the Friday before the scheduled course. A $25 cancellation fee will be applied to cancellations received
by the deadline. All cancellation requests must be in writing. No other refunds will be given including partial refunds for missed
courses from Package registrations.
Cancellation Clause: It is the responsibility of each individual registered to confirm a scheduled meeting if there is the possibility
of a cancellation due to inclement weather. Registered attendees should check for e-mail updates as well as information on
the homepage of CSDA.com. If the CSDA cancels or postpones a course due to inclement weather or some other unexpected
emergency, those registered for that course only are entitled to a full refund if he/she is unable to attend the rescheduled date.

This course satisfies the one-hour of required mandatory continuing education for CT dentists
for Prescribing Controlled Substances and Pain Management as well as Special Needs Patients.
The dentist is faced with an aging population with a myriad of medical problems and physician-prescribed medications. Medical
problems like diabetes mellitus, hypertension therapy, anticoagulant therapy, the drugs for a myriad of cardiovascular diseases,
pregnancy and the nursing mother, anxiety and depression directly affect dental treatment planning and prescriptions written by the
dentist. This course will provide the dentist with a framework for choosing antibiotics, analgesics, and local anesthetics within the
context of potential drug-drug interactions and disease-related complications of the medically compromised patient.

Computerized Dentistry for Private Practice:
Aesthetics, Implants and Occlusion (AM Session)
CAD/CAM technology has the advantage of reducing clinical chair-time and reducing laboratory costs. The accuracy of these techniques
has improved to the point where they are now comparable to conventional techniques. This presentation will review the current
technology which allows the fabrication of anterior incisors in one-visit protocol. In addition, these techniques can also be used to
fabricate implant restorations. Utilizing coded healing abutments or scan bodies without a fixture level impression can allow us to design
ideal abutments for implants. Digital acquisition impressions of implant and ceramic restorations will be demonstrated. Anterior and
posterior restorations using CAD/CAM technologies and impressions will be discussed.
At the conclusion of this lecture the participant will understand the advantages and disadvantages of:

Success Without the Stress (PM Session)
Since our dental school education, we were always told that “we could not open the bite”. Many clinicians avoided treating the “worn
dentition” while others had great success. This lecture will help de-mystify with practical methods of how to restore OVD position up
to 10 mm. The long-term success of these patients with these techniques has been documented and recorded both radiographically
and with photographs with up to 10-year results. Using sound prosthodontic principles and digital analysis, these OVD restorations can
become standard in our private practice. Anterior determinants and posterior determinants can show us the way through this “gray area”
of dentistry.

A follow-up hands-on workshop with Dr. Kaminer will be available to doctors who take this full-day lecture during CSDA’s 2017 Annual
Meeting at Mohegan Sun. There will be two offerings to choose from on Thursday, May 11 – one in the morning and one in the
afternoon. Each workshop will be limited to 35 participants.
A Streamlined Approach to Preparation, Temporization,
and Cementation of the Esthetic Patient
As a follow up to our lecture, we will review the key highlights of treating the esthetic patient. During this hands-on workshop, we will
build upon the concepts previously discussed and implement those concepts in a simulated esthetic case. We will perform preparations
on study models, temporize the case, seat a few restorations, and have a chance to work with some digital impression scanners. The
participant should expect to leave with an understanding of how to break down the esthetic case and feel comfortable with the
aforementioned exercises as he/she translates this into a live esthetic case. The participant will also gain knowledge of the differences in
digital impression scanners helping to make a decision if this technology is right for his/ her practice.

ABOUT THE SPEAKERS
September 14th
Dr. Gary DeWood earned his D.D.S. from Case Western Reserve University in Cleveland in 1980.
He completed the University of Florida Facial Pain Center Curriculum from 1992 through 1995, and in
2004 he earned a Master of Science degree at The University of Toledo College of Medicine. He left
full-time private practice in 2003 to serve as Clinical Director at The Pankey Institute, a position he held
until 2008 when he joined Frank Spear in Seattle as president of The Seattle Institute. With the merger
of The Scottsdale Center and The Seattle Institute to form Spear Education, he relocated to Arizona
and today serves as Executive Vice-President and director of Curriculum for Spear. He maintains
a limited private restorative practice on the Spear campus, teaches in the Spear workshops, Spear
Online, and in the Spear Study Clubs.

November 9th
Dr. Tieraona Low Dog’s exploration of natural medicine and its role in modern health care
began more than 35 years ago as she studied midwifery, herbal medicine, massage therapy and
martial arts before earning her medical degree from the University of New Mexico School of Medicine.
She currently serves as the Fellowship Director for the Academy of Integrative Health and Medicine.
This two-year fellowship is the first integrative interdisciplinary program in the world committed to
training teams of medical doctors, dentists, pharmacists, nursing professionals, physician’s assistants,
dietitians, acupuncturists, naturopathic and chiropractic physicians in integrative health and medicine.
Dr. Low Dog is a founding member of the American Board of Physician Specialties American Board of
Integrative Medicine and the Academy of Women’s Health.

Dr. Fazio is a graduate of Harvard College and the Harvard School of Dental Medicine with
additional fellowships in Periodontology and Oral Medicine. For 35 years he has been an Associate
Clinical Professor of Surgery at Yale Medical School and an Attending at the Yale New Haven Hospital,
New Haven Connecticut. His current book, co-authored with Leslie Fang M.D., is the 2015 Edition

of “The Ultimate Cheat Sheets – The Practical Guide for Dentists”. It has been named by Gordon
Christensen’s CRA Newsletter: “Best Educational Product” for three consecutive years. He serves on the
Editorial Board for the Journals: Inside Dentistry and Compendium of Continuing Education in Dentistry.
Dentistry Today named Dr. Fazio one of the “Top 100 Clinicians in Continuing Education in Dentistry” for
nine consecutive years.
February 8th
Dr. Dean Vafiadis received his dental degree and Prosthodontic specialty training at New York
University College of Dentistry. He is currently the Director of the Full-Mouth Rehabilitation CE course at
NYU. He is an Associate Professor of Prosthodontics at NYU College of Dentistry. Dr. Vafiadis published
and lectured on various topics such as aesthetics, implant designs, computer restorations, ceramic
materials and occlusion. He is the founder of the New York Smile Institute. It is an educational center, full
service laboratory and learning facility as well as a private practice location for a multi-specialty practice
for Implant and Aesthetic Dentistry. Currently he is lecturing for the Continuing Education program at
NYUCD for Dental Districts in US, Europe and Asia. He maintains his private practice Valentino Building
near Rockefeller Center. Dr. Vafiadis’ primary professional mission is to educate, publish and invent new
technologies that will help change the dental profession and patient’s lives.

Darrell Cain is the founding partner of Cain, Watters & Associates, a Dallas-area CPA/RIA firm providing
tax and accounting services, ongoing financial monitoring, investments solutions, practice valuations and
transition services, and estate planning. Mr. Cain received his BBA and MPA from Baylor University in Waco,
Texas. He is a member of the Texas Society of Certified Public Accountants and an Investment Advisor
Representative (IAR). Additionally, he is a Certified Valuation Analyst. For more than 30 years, he has spoken
to dental organizations, study clubs, associations, and investment symposiums across the country, as a
recognized leader in the field. Mr. Cain’s deep understanding of the critical issues that dentists face has
made him a sought after voice providing audiences with the answers and tools they need to move toward
practice and personal financial success.

Dr. Ron Kaminer is a 1990 graduate from SUNY at Buffalo School of Dental Medicine. He maintains
two practices, one in Hewlett, NY and one in Oceanside, NY. Dr. Kaminer is an international expert in the
field of dental lasers and has lectured on lasers and minimally invasive dentistry both nationally and
internationally. He is the Director of the Masters of Laser training program in New York and is a clinical
instructor with the International College of Laser Education. He evaluates new dental materials for the
Catapult Group and sits on the advisory boards/is a clinical consultant for numerous dental manufacturers.
He has authored numerous articles on dental lasers and minimally invasive dentistry, is a frequent
contributor to Mentor magazine and is an associate editor for Dental Product Shopper. He is a member of

What You Should Know About
Allied Membership in the CSDA
What is Allied membership?
The CSDA has created a special membership category for
hygienists, assistants, administrative staff, lab owners and
technicians so they can join our association.
What are the special discounts available to Allied members?
Allied members can save up to $50 when attending CSDA events:

A $10 discount off the registration fee for individual CSDA Package
Program Series courses.

A free registration for the CSDA Annual Meeting (a $40 savings) if they
sign up prior to April 1st or $20 off their registration after the early bird
deadline.
What are the other benefits of Allied membership?
Allied members receive a complimentary subscription to The
Communicator, our quarterly news magazine. The CSDA
also offers more timely updates through our bi-weekly enewsletter.
How much are Allied membership dues?
At $35 the annual dues for Allied membership is incredibly inexpensive.

An alternative to the traditional classroom, online CE allows you the convenience
of completing some of your continuing education requirements anytime, anywhere!
The CSDA, in partnership with DrBicuspid.com, is pleased to offer several online
versions of some of the allowable mandatory CE topics for dentist license renewal
in Connecticut. There are currently nine CSDA courses to choose from and you are
encouraged to check out DrB’s entire library as a means of earning some of your
CE credit.

CE Requirements for CT Dentists:
The State of Connecticut mandates that licensed
dentists must earn a minimum of 25 hours of CE
credit every two years commencing on the first
date of license renewal. Of those 25 total hours,
one hour must be on the topic of “prescribing
controlled substances and pain management”
(effective 10/1/15). Four additional hours of
training must be in any of these ten topical areas:
infectious disease (including immune deficiency
syndrome and human immunodeficiency virus),
access to care, risk management, care of special
needs patients, domestic violence (including
sexual abuse), record keeping, cultural
competence, current topics in infection control
and patient safety, medical emergencies in the
dental office (including current training in CPR),
and common medications prescribed (what you
need to know before you treat). The remaining 20
hours can be any combination of elective credits
offered by approved providers in areas related to
the licensee’s practice and reflect the professional
needs of the licensee in order to meet the health
care needs of the licensee’s practice.
Internet-based, on-line and other distance
learning opportunities are acceptable.

2017 CSDA ANNUAL MEETING
Don’t forget, the 2016-2017 Package Program includes registration for the CSDA 152nd Charter Oak Dental
Meeting, May 10 - 12, 2017 at Mohegan Sun Resort Casino. You must register by April 1, 2017 in order to take
advantage of the waived registration fee. After that date, fees will apply.
COURSE HANDOUTS
You will receive an e-mail from the CSDA approximately two weeks before the scheduled course confirming
your registration with the corresponding handout attached. It will be sent a second time the Monday before and
will also be accessible within CSDA’s CE mobile app. Not all courses will have handouts. If you wish to have these
materials to reference during the course, please be sure to download and print them in advance. These materials
