ADOPTION QUESTIONNAIRE Adopt Now or Wait
ADOPTION QUESTIONNAIRE
Please complete this form and bring it to the adoption desk when you have found a pet in whom you’re interested.
• By answering these questions, our adoption counselors will guide us both toward making the best decisions possible.
• Thank you for considering the Pet Alliance of Greater Orlando as a resource for you and your future pet.
The goal of the Pet Alliance of Greater Orlando is to find loving, permanent homes for the animals sheltered with us.
• We believe a pet adoption should be a LIFETIME commitment – a match made with great care.
• We know that temporary housing in a shelter is not ideal for pets. But we also know it’s much harder on a pet to leave
our shelter only to be returned because the adoption decision didn’t take into consideration all the important factors.
PLEASE NOTE: Many apartment complexes, home rentals, mobile home parks, condo associations, and insurance companies
place restrictions on the number, size, and breed of pets that can live with you.
• In addition, these same organizations may require a pet deposit.
• We strongly suggest you check with these organizations BEFORE continuing with your adoption application.
• The Pet Alliance of Greater Orlando also reserves the right to check with them before finalizing the adoption.
