The Faxton St. Luke’s Healthcare (FSLH) Foundation, in
partnership with WKTV NewsChannel 2, Lite 98.7FM,
The Observer-Dispatch and Lewis Custom Homes is
fulfilling dreams with the Miracle Home Makeover to
benefit the children of the Mohawk Valley.
A maximum of 4,300 tickets are available beginning
April 11, 2016 and may be purchased at $100 each
through the FSLH Foundation at 315-624-5600. The
drawing for the Miracle Home Makeover will be held
on Wednesday, October 12, 2016, at 6 p.m. at the St.
Luke’s Campus, 1656 Champlin Avenue, New Hartford,
and will be audited by an independent auditing firm.

Zachary received the care he needed because
of your support. Zachary was receiving care
through the Hearing and Speech Department at
Faxton St. Luke’s Healthcare. Due to insurance
restrictions, he became unable to continue
the care he needed to thrive. The Children’s
Miracle Network Hospitals at FSLH was able to
help by providing the necessary funding to supplement where
the insurance fell short. Thanks to CMN, Zachary was able to
continue his therapy. His hard work and determination helped
him graduate from the program in December.
Evan is getting so big thanks to your
kindness. Evan was born unexpectedly at 36
weeks and became one of our tiny miracles.
Since Evan was born prematurely, his lungs
were underdeveloped, which meant he had
to spend time in the area’s only Level II Special
Care Nursery at FSLH until he was strong enough
to go home. He was cared for by neonatologists — physicians
trained to provide specialized care to sick or premature babies
— who are on staff 24 hours a day, seven days a week at FSLH.
Thanks to your generosity, CMN is able to support the Level
II Nursery so that Evan could stay close to home during this
critical time, with his family by his side.
Nicholas experienced your generosity
first hand. A fun summer night turned tragic
when the car Nic was riding in was in a roll-over
accident. He was stabilized at the scene by
the first responders and was transferred to
the Emergency Department (ED) at FSLH. Once
in the ED, it was determined that he had broken
his neck in two places during the crash. The team worked
collaboratively to carefully prepare Nic for a safe transport
to a hospital with a higher level of care for his type of injury.
The time sensitive and critical care that Nic received from the
first responders at the scene and from the team in the ED was
crucial in him making a full recovery from his injuries. CMN
helps to support the first responders and ED so that individuals
in critical situations receive the best care possible.
Winner Eligibility
and Responsibilities
The Faxton St. Luke’s Healthcare Foundation staff, Foundation Board of Directors and its members, Mohawk Valley
Health System Board of Directors and senior management
staff, First Source Federal Credit Union and GPO Federal
Credit Union senior management staffs, and owners of
Lewis Custom Homes, Inc. and immediate families of
the owners of Lewis Custom Homes, Inc. (grandparents,
parents, spouses and children) are not eligible to win. The
Miracle Home Makeover prize to be awarded to the winner
is the makeover project valued at $100,000 in products and
services from the Miracle Home Makeover participants, net
of taxes, or the cash equivalent, net of taxes. The Makeover
project specifications, time frame, location and scope of
work are to be approved by Faxton St. Luke’s Healthcare
Foundation and Lewis Custom Homes, Inc. The winner of
the Miracle Home Makeover is responsible for all applicable
taxes, specifically including income tax. Tickets cannot
be transferred. Entrants must be at least 18 years of age.
The winner of the Miracle Home Makeover must start the
makeover project within six months from the date of the
drawing and complete the makeover project within one
year of the date of drawing.
