Where would any of us be without it?
This fiscal year brought new challenges and opportunities for our agency. I thank our strong
clinical team, diligent support staff, volunteer board of directors and you – our loyal donors and
supporters. Without each of you playing a role, we could not have had as successful a year as we did.
I’m am truly grateful and touched by what I’ve seen and heard this year; here’s a sample:
Family Caregivers require more training… Thanks to you, a team of our expert
clinicians held community focus groups and tested new classes to help family caregivers tackle
complex situations at home, like dementia care, wound care and incontinence. With the data collected,
we created The Family Caregiver Education Series to help coach family caregivers in a variety of
specific issues and challenges. As you know, we are passionate about helping family caregivers and our
Family Caregiver Support Network has helped more than 1,200 family caregivers since it launched in
2003. We are excited to see how many more we can help with this new education series.

Our community is moving more… this year we saw a 14% increase in our exercise class
participation. People were trying new things like Tai Chi, Yoga and Zumba. Thanks to donor support,
we can keep our rates low so our classes are available for all fitness and income levels.
More people want to know their numbers… more than 2,500 individuals participated in
our free Ask the Nurse Clinics. Thanks to your support, thousands learned what their blood pressure
was, reviewed their medications and got advice from our community nurses.
Our continuum is vital… Our care affiliates, LIFETIME Care at Home and Strong House
Adult Day Center continue to offer our community choices and resources to help individuals remain
safely at home. LIFETIME Care at Home continues to seek out new and innovative technologies
and services to help families succeed. You can read more about their accomplishments online at

Our seniors are falling less… Thanks to our successful Fall Free Living program, funded
by the Connecticut Collaboration for Fall Prevention at Yale University School of Medicine through
the Connecticut State Department on Aging, more than 800 area seniors were screened, coached and
monitored to help them prevent falls! The program is so successful that it won the prestigious Triple
Aim Award from the CT Association for Healthcare at Home this year. We saw an 82% reduction in
falls, 78% reduction in ER visits and a 79% reduction in hospitalization.
We’ve also partnered with PRIDE, a new program developed by
Yale University School of Medicine (Department of Emergency Medicine
Section of Emergency Medical Services), other home healthcare agencies
and local emergency medical services, to provide in-home fall risk
assessments to patients who call 911 for lift assists.
What we do matters… the letters I receive about
our visiting staff and the care they provide are truly inspiring. In the
following pages are two such stories out of the thousands of patients we
help each year. We thank you for supporting us as we provide vital,
necessary care; helping our community get healthier – one visit at a time.

“I feel so safe knowing VNA Community
Healthcare is watching out for me”
A patient reflects on her telemonitor service
Lois Kiesewetter is a spunky, kind hearted, retired realtor who has an overwhelming love for her
family, which includes grandkids and great grandkids.
At 83, she struggles with pain and discomfort from Chronic
Obstructive Pulmonary Disease (COPD), Congestive Heart Failure
(CHF), and Diabetes. She requires constant oxygen and has
extreme difficulty leaving her home, only doing so for medical
appointments.
But Lois doesn’t let it get her down; “I’ve had a full life and
I was able to spend 50 years married to the love of my life.” Her
husband sadly passed away a few years ago. She now lives with
her son Carl and her delightfully peppy 4 year old Havanese dog,
Jewel.

how to relax my breathing and then my

nurse will notify her doctor about
changes in her condition and work with
him or her to change treatment plans.
In addition to checking Lois’ oxygen levels, the telemonitor checks
her weight, blood pressure, heart rate, and day-to-day well-being. The
goal is to prevent a trip to the hospital.

“I can’t imagine not having my daily check-ins (with the telemonitor)… I like to know that I’m
doing ok, and if I don’t feel right, this machine can tell my nurse exactly what is wrong…. I am so
grateful to have it and would recommend it to anyone like me,” states Lois.
We thank you, our donors, for enabling us to help Lois and
559 other patients like her. Telemonitoring is the future of home
healthcare and it is not an inexpensive service. The cost in fiscal
year 2015 was more than $200,000. We could not provide this
service without your support and generosity.
“We couldn’t do it without you”
A dedicated grandmother reflects on the team of nurses
who help her grandson live a normal life at home.
Anthony Caruso was born in 1997 with cerebral palsy. His
grandmother, Ruby recalls that the doctors told them, “he would be
a vegetable… he would never eat or drink, never sit up, never speak
or communicate and would have to live in a facility…”
With that Anthony’s vibrant, gentle blue eyes lit up and
he began to sing and dance in his chair, smiling the biggest smile
you’ve ever seen. Ruby sighed and looked adoringly at him as a
warm, proud smile swept across her face, “I ask you, does he look
like a vegetable?”
“We were devastated with what his first doctor told us, but I
was determined to fight for Anthony.” And fight she did. Finally,
after the first agonizing 18 months of his life, Anthony had ongoing
hospital trips, suffering from horrible reflux that destroyed his
esophagus and up to 200 seizures a day, Ruby found her “dream doctor”. “Dr. Testa gave us (Ruby and
her husband Ralph) hope, changed Anthony’s medications and he became seizure free and we could
finally go home with help from VNA (Community Healthcare). Our lives changed for the better and it
was the first time Anthony smiled.”
When VNA Community Healthcare first started to care for Anthony, our nurses worked 24 hours
a day. He now attends ACES High School and 4 different nurses work with him daily for a total of
88 hours a week. “They have given us a life... life would be unimaginable without them. They have
become part of our family.”
His nurses administer his medications through a tube in his stomach (G-tube); monitor his
health, as he is prone to sinus infections and seizures; do exercises 2-3 times daily with him; transfer all
120 pounds of him into his stander, a device that enables him to stand for at least an hour a day; attend
to all his personal care and bathing; and help him redirect his agitation when he gets frustrated or upset.
Anthony cannot walk and requires the use of a specially equipped wheelchair and total
assistance from his nurse and family. He has good days and bad days. On the good days, you can find
him with his family watching a concert on the Branford green or going for a ride.

“During the summer we get in the car and drive… Anthony sings and spots
American flags as we go… he always spots the flags,” Ruby says with a
smile. With that Anthony, started singing the Star Spangled Banner. He is
unable to say the words, but you know that he knows them and tries hard to
carry the tune. “Since he was a baby I sang to him and we listened to music…
anything from Tina Turner to Barney. He loves it all.” He has a portable
DVD player that enables him to play music at any time and that soothes him.
Patients like Anthony deserve to live a quality life and VNA
Community Healthcare helps them achieve that goal. But services such as
these are not fully compensated by insurance or government programs. Your
contributions help us sustain this program.

At Strong House Adult Day Center, we stimulate and celebrate
seniors with memory or physical care needs who may need a little extra
help during the day. By keeping them engaged and catering to each of
their unique abilities, we preserve each client’s dignity and help them
maintain their highest level of function while enriching their lives.
Our veteran staff of 12 and volunteers provides a safe, yet warm
and comfortable home-like setting for the 20 clients that visit each day.
Health & wellness services and medication assistance are provided by
an on-site RN. We do a variety of activities and take frequent field trips.
Every day we do exercises, provide stimulating recreational programs
customized for all levels, and offer interactive musical entertainment.
We take pride in making the lives of our clients and their family
caregivers a little easier. All charitable gifts we receive go towards
enhancing the programs they so love.

A doctor says, "Why I give”
“I know of no other agency in our area that supports patients and families as you do,” says Dr.
Steven Wolfson. “Keep doing what you are doing," he added, “you fill a real void in our community."
Dr. Wolfson is a recently retired cardiologist who has a long history with us, both as physician
and volunteer. Starting in the early 1970’s Dr. Wolfson referred his patients to VNA Community
Healthcare when they left the hospital, but he also learned the benefit of referring them directly from
his office to avoid hospitalization.
“My patients benefited greatly from the compassionate supervision and observation provided
by (VNA Community Healthcare) nurses. The result? Fewer hospital
admissions.”
Over the last 5 years, Dr. Wolfson looked to VNA Community
Healthcare for another purpose: community patient education and
engagement. He joined our Health Neighborhood committee and volunteered
to be one of the “actors” in our educational video on Health Literacy which
describes the importance of bringing someone with you to a medical visit.
“VNA Community Healthcare started the Health Neighborhood
for no gain. The purpose was to help educate the community and improve
communication among healthcare professionals. This public spirited work is
another reason for me to admire VNA Community Healthcare.”
He added, “You do whatever you possibly can to improve the health
of those in our community and you never look for profit … you see a need and you seek out ways to
fill that need.”
