I’d like to personally welcome each of you to the 2015 Utah Dental Association Convention. It’s an exciting time for
dentistry as our profession continues to grow, adapt, and progress. Our committee has endeavored to create a program that
keeps up with our ever changing industry. We hope that everyone will come away with a feeling of satisfaction, renewed
energy, and a commitment to provide quality work.
This year we have strived to provide quality educational opportunities for the entire dental team. We have added more
classes with greater variety for our hygienists, assistants, and office staff. This includes an increased number of hands on
workshops. And of course, we have a great lineup of speakers and topics for our dentists. We hope the entire dental team
will be able to return and incorporate something new and valuable Monday morning.
On behalf of the planning committee we appreciate your continued support to help make this one of the leading

Adhesion Dentistry

There are many compelling clinical
benefits for utilizing adhesion rather than
mechanical retention with its inherent tooth
destruction. Remaining dentin thickness correlates with the need
for endodontics. Endodontics increases risk for catastrophic tooth
fracture. Remaining dentin thickness is preserved by utilizing
adhesion. Clinical protocols for adhesion of composite, metal and
ceramics, HF etchable and not HF etchable ceramics are presented:
• avoid crowns to increase the chance of avoiding endodontics
• the ideal veneer prep is facial–incisal, avoiding envelope of
motion alteration
• the ideal onlay prep is flat
• mechanical retention leads to post-op sensitivity
• most one bottle bonds are too good to be true
Dr. Bertolotti received his DDS from University of California
San Francisco, after working as a PhD metallurgical and ceramic
engineer at Sandia National Laboratories. He is currently honorary
associate professor at Warwick University in England. Dr.
Bertolotti is perhaps best known for introducing total etch to North
America in 1984. He also introduced Panavia in 1985, tin plating
in 1989, self-etching primers in 1992 and HealOzone in 2004.
The sectional Contact Matrix system, MicroPrime, MicroEtcher
sandblasting and intraoral tin-plating are also his innovations. He
is a well-known international lecturer, having presented lectures in
over thirty countries. Oral Lesions of the Oral Cavity

This program is designed for everyone on
the dental team who wishes to review, update,
and further their knowledge of common oral
conditions and diseases, and their management protocols. Attendees
will also be instructed on the technologies available to help with the
early detection of oral cancer:
• intraoral and extraoral head and neck examination
• recognition and management protocols for common oral
lesions and diseases
• clinical manifestations of age-related changes and nutritional
deficiencies

• recognition and management of oral cancer and
precancerous lesions
Attendees may expect to return to their practices with the latest

• the recognition of premalignant and malignant oral

• technologies available to help with the early detection of
oral cancer
Dr. Budenz is professor in the department of biomedical
sciences and is vice chair of diagnostic sciences and services,
department of dental practice at UOP in San Francisco CA. He
received his graduate degree in anatomy from UCLA, and his
dental degree from UCSF. He has more than thirty years of general
practice experience in San Francisco, has served as a general
dentistry clinical floor instructor, and has taught general dentistry,
local anesthesia, and anatomy courses at UOP since 1984.

New solutions to edentulism are evolving
monthly as pressures to provide better, faster,
less expensive solutions for patients increase. As
evidenced from strong contemporary literature this rapid evolution
has outdated the science we were taught in dental school programs
regarding conventional dentures and implant prosthetics for
edentulous patients. This seminar will thoroughly review the most
contemporary techniques that provide better, faster, less expensive,
literature-supported solutions for edentulous patients in both
conventional and implant prosthetics.
Dr. Durham is a full-time practicing prosthodontist, the GPR
assistant residency director, and oversees the dental laboratory
at the Salt Lake City Veterans hospital. He maintains a private
practice in prosthodontics. He is an assistant professor at the U of
U school of dentistry, and course director over Edentulism. He is a
founder of dentucation.com, and the CEO of Lightrium. He received
his DMD from Case Western, and his prosthodontic diploma
from Louisiana State University school of dentistry. His research
interests and lecture topics include: prosthodontics, ceramics and
cements, conventional dentures, implant overdentures, implantsupported hybrid prostheses, full-mouth fixed dental and implant
rehabilitation, esthetics, treatment planning, and post and cores. The Diagnosis and Treatment of
Mucogingival Defects
Ryan Edmunds, DDS, MS

This course will cover the diagnosis,
prognosis, and treatment options for
mucogingival defects and gingival recession around teeth and
implants. We will discuss what is normal and abnormal, when
treatment is indicated and which treatment is best for each
circumstance. There are tried and true techniques, but also
minimally invasive new techniques with acellular dermal matrix

and collagen products that can produce very effective results that
are also esthetically pleasing. Reviewing diagnosis and prognosis
will allow you to predict results so that you can communicate to
your patients what can realistically be expected. You will leave
with a firm understanding of the free gingival graft, the connective
tissue graft, and the soft tissue allograft: when is each an appropriate
treatment and what outcomes can be expected. Dr. Edmunds is a full-time private practicing periodontist
in Provo. His practice focuses on treating periodontal disease,
regenerative procedures around teeth, implants and bone and soft
tissue augmentation in preparation for implant placement. His
practice largely focuses on tissue regeneration, soft tissue grafting,
implant placement particularly in the maxillary anterior where
immediate placement and temporization can be regularly performed,
and the treatment of ailing implants. He is a regular provider of
continuing education on periodontal and implant topics.
Cutting Edge Endodontics:
From Diagnosis to Obturation
David Landwehr, DDS, MS

Endodontic success is dependent on a
variety of related factors. A proper diagnosis,
profound anesthesia, and straight-line access into the chamber
are required prior to enlarging the glide path. This seminar will
provide a comprehensive review of endodontic diagnosis and the
limitations of diagnostic testing. Special attention will be given to
various anesthesia techniques and choices to increase anesthesia
predictability. This lecture will also include a comprehensive review
of glide path management to provide several options to treat the
most complex root canal anatomies.
Dr. Landwehr earned his DDS at the University of Minnesota
and an MS degree and certificate in oral and maxillofacial pathology
from Ohio State University. Dr. Landwehr went on to complete
specialty training in endodontics at the University of Michigan
in 1999, and has since maintained a private practice limited to
endodontics in Madison, Wisconsin. Dr. Landwehr is chief of
endodontics for the Meriter Hospital general practice residency
program in Madison. He has presented case studies in endodontics
and oral pathology both nationally and internationally, published
several articles in peer-reviewed journals, and served as an evidence
reviewer for the ADA.

Discuss and learn all there is to know about
periodontology. We will cover topics on periodontal disease, home
care for the periodontal patient, conservative and surgical therapy
for periodontal disease, treating furcations, gingival tissue grafts,
extraction and bone grafts, implant therapy and when to refer. We
will make periodontics fun.
Dr. Manwaring is a board certified periodontist who has been
practicing in Salt Lake City and Park City since 2004. Jeremy
received his DMD from Tufts University school of dental medicine.
He also completed a three year residency program in periodontology
and received a master’s degree from Tufts University. After working
in Boston a few years he was thrilled to move his family back to Utah.
He lectures frequently to the hygienists on periodontal updates.

Insurance 101: What You Need to Know to Reduce Your
Accounts Receivable

Get an insider’s perspective on how insurance payments work and
understand rules and mandates so you can get paid faster.
Ever wondered why insurers don’t all require the same
information or process claims the same way? In this session, you’ll
learn all about the different types of insurance coverage and which are
governed by the State Insurance Department so you can be prepared
for different billing requirements, including coordination of benefits
(COB) billing. We’ll help you understand how insurers determine
how much each insurer is responsible to pay in a COB situation.
You’ll also gain an understanding from the State of Utah Insurance
Department Fraud Division on what constitutes insurance fraud,
mandatory reporting laws, immunity and HIPAA issues. We’ll also
talk about:
• federally mandated insurance coverage and how the
mandates affect you as a provider
• how to prepare for new mandates such as ICD-10
• appropriate use of the National Provider Identifier and the
importance of setting up new providers in your practice early
• the electronic claims cycle and what your clearinghouse
should be doing for you
• this session will feature Q and A with a payer panel – the best
way to learn tips and tricks to help you reduce the lag time
between billing and payment
The UHIN Dental Work Group is a collaboration of dental
payers, providers and billing companies whose goal is to decrease
administrative waste and improve the billing and reimbursement
process for dentists through electronic transactions. The Work
Group is facilitated by UHIN, a non-profit clearinghouse whose
mission is to reduce healthcare administrative costs and increase
care quality. UHIN members include dentists, Utah dental insurers
and the Utah Department of Health including Medicaid.

Jenna Kim Jones
Jenna Kim Jones was born in Seoul, Korea where she lived
for 8 years until moving to Provo, Utah. At 18 she left
Provo to attend New York University where she majored in
producing and writing for television. While attending NYU
she interned at Follow Productions, the Martha Show with
Martha Stewart, the Late Show with David Letterman, The
Daily Show with Jon Stewart, and CBS Primetime Casting.
Jenna began performing stand-up comedy while working
at The Daily Show with Jon Stewart. After working at
The Daily Show for almost four years she met her future
husband. They were married and moved to Los Angeles so
Jenna could pursue her comedy career.
In 2012 Huffington Post Comedy named Jenna one of 18
Funny Women You Should Be Following On Twitter. Jenna
has appeared on The Daily Show with Jon Stewart and has
performed stand-up alongside some of the best comedians
in the business including Jim Gaffigan, John Oliver,
Wyatt Cenac and Aasif Mandvi. When not entertaining
crowds with her stand-up comedy routines Jenna can be
found working on her popular weekly podcast entitled
#SorryNotSorry. Many will recognize her as the on-camera
narrator of the recently released documentary Meet The
Mormons. Plan to laugh and be entertained!

endodontics in Madison, Wisconsin. Dr. Landwehr is chief of

Years ago it was possible to run a profitable
practice by simply choosing an office, hanging out a shingle, hiring
staff, and purchasing supplies. No longer. Group practice dentistry
can charge less for procedures due to a focus on cost containment
and speed of service. Dental insurance reimbursement is lower.
Dentists are wedged between these two entities as they attempt to
remain profitable while maintaining the professionalism and quality
of care they desire.
Since dental plan reimbursement can be up to 95 percent of
a dentist’s take home pay, it is crucial to know whether plans are
profitable or not. Most dentists have a contract with at least one
dental plan that shows a negative net profit. Some dentists literally
work for free and pay for the privilege. At the same time, most
dentists also realize positive net profits from many plans.
How can you tell which dental plans are profitable for your
specific practice? Armed with this information you can add, drop, or
cap the dental plans that work for you:
• third party dental insurance penetration in Utah is very high
• Utah is in the bottom tier of dental insurance reimbursement
• it is difficult and time consuming to know which dental plans
are profitable for each practice
• large group practices account for approximately 10 percent
of dentistry in the U.S. and are projected to reach 11
percent by 2015
Chris graduated from the U of U with a BA and received his MBA
from Westminster College with an Entrepreneur Certificate. He has
presented seminars at recent CQIs, UDA conventions and has published

for provisional and indirect restorative

are no longer regarded as temporary
restorations but rather as provisional restorations with distinct
functions and purposes. Provisional restorations have become
a vital diagnostic and assessment tool to evaluate function,
color, shape, contour, occlusion, periodontal response, implant
healing, and overall esthetics. An accurate fit and margination
is essential to insure and maintain pulpal health. With increased
demands being placed on provisional restorations, new materials
and techniques are being developed and some existing protocols

Weight Loss that Work. She created and produced numerous
successful workout programs, including: StepTonic, SculptTonic, and
DanceTonic. Melanie loves to motivate people to improve their health

a dental hygienist, only to have them tell you “I
floss!” When surveyed, consumers agree the most
frustrating part of the dental hygiene visit is this question. Research has
confirmed that only 2 to 10 percent floss regularly and effectively. With
this dismal statistic the seminar will review the history and research
of string floss for interdental health. Are there other equally effective
or more effective methods? Is it time to toss the floss? This interactive
session will empower you to consider other options that will increase
patient health and satisfaction:
• integrate the latest research on interdental health tools and
technologies
• identify thirty ways to use floss…outside of the mouth
• implement a new process of care that will empower you to
‘toss the floss’
Kristy is a graduate of the University of Maryland, is the director of
Educational Designs, is celebrating her thirtieth year in dental hygiene

by going back to school. She is enrolled in the master dental hygiene
program at UCSF, where she is also an assistant clinical professor. She
is an international speaker, has served on editorial boards for state and
national publications for the past 20 years, and authored a chapter in the
latest edition of Mosby’s dental hygiene textbook on oral malodor.

Learn the basics about dental radiology and
related techniques. Attending this session and
passing a written quiz qualify participants to
receive a required Utah safety certificate. Attendees must register for
the entire convention and pay an additional $20 fee for this course.
Wendi is a certified dental assistant with the dental assisting
national board. She has worked in the dental field for twenty-four
years and is currently the dental department head at Bridgerland
ATC in Logan.

DISTINGUISHED SERVICE AWARDS
Mark Blaisdell, DDS
Mark grew up in Caldwell, Idaho and attended BYU where he graduated in zoology in 1977. He
received his DDS from UOP in San Francisco in 1981. Mark and his young family moved to Bountiful
where he started his dental practice. After eight years he moved into the current building he built with
several colleagues.
He has served as a leader of the South Davis Dental Society, and of the UDA, serving as president
in 2005. He then served as a delegate to the ADA for four years. He currently is a member of the
ADA Council of Dental Benefits Programs and is the chair of the Dental Benefits Information Service
Subcommittee. Mark helped find the initial funding of the student-at-large scholarships given to ten
Utah students a year that awarded over 5 million dollars in awards, and also led the Utah legislative
action by UDA to exempt for all Utah dentists significant amounts on sales taxes on prosthetics.
Mark has participated in numerous dental humanitarian excursions to Mexico, Peru, Guatemala
and Honduras, and is a member of the ICD, Pierre Fauchard and the ACD. Mark and his wife are the
parents of three children, and have 4 grandsons.
Gary Crawford, DDS
Gary claims to have dentistry in his DNA. He is a descendent of Alexander Neibaur who was the first
dentist in Utah. Dr. Crawford was born in Moab, and raised in Salt Lake City where he attended Skyline high
school. He attended Snow College, the University of Utah, and dental school at the University of Oklahoma.
He did a general practice residency at St. Anthony Hospital in Oklahoma City and then made the decision to
concentrate exclusively on surgery in Chicago as a resident in oral and maxillofacial surgery at Cook County
University of Illinois, and Michael Reese hospitals. He then returned to Utah and joined Niles Herrod in
private practice in Provo.
From the beginning of his career Gary has concentrated on giving back to the profession, his community,
and humanity. He has served in many positions in dentistry including president of the Utah Dental Association,
delegate to the ADA, and president of the Utah Association of Oral and Maxillofacial Surgeons. He has served
on many committees at Utah Valley regional medical center, serves on the facial trauma service, and has
lectured to physician colleagues on the management of oral and facial trauma.
Gary has been involved in the charitable care of people in need through donated care, community health
connect, and as president of the Utah oral health coalition. He has traveled many times to Central America to
provide surgical care, bring tremendously needed instruments and facilities, and provide education to people of
those countries.
He married the love of his life Kathy, who has been at his side and helping with all these endeavors since
they met in high school. They have two sons and two daughters-in-law, and to this point one grandchild.
