Before filling out any information read these instructions carefully.
Use a black pen only. Do not use pencil, gel pen, or marker.
Print clearly using both upper and lower-case letters.
Keep all letters inside the boxes provided.
Fill in ovals completely. Stay within the lines.
Avoid making stray marks.
Section 1 — Print the following items using both upper and lower-case letters.
