The mission of Ute Family Services is to strengthen families now and for the future through three core
service: crisis/respite/shelter; in-home and center based parenting classes, support groups, short-term
therapy for families including children. Services include child abuse prevention with families identified as
as-risk or identified to child welfare system, adult education, parent engagement in policy/advocacy,
youth development activities, counseling services, among many other services.

Anger Management Counseling will help you to identify the triggers, feelings and sources of your anger.
You will learn how to communicate and express your anger in a healthy and positive way. Individualized
counseling will consist of 12 sessions. Participants must attend all sessions to receive a certificate of
completion, to show proof of attendance.

The Family Group Decision Making offers services for families who need help and support in creating a
safe and healthy environment for their family unit. Families are empowered to make their own decisions
on how to raise their children effectively and culturally appropriate. Families are given the opportunity
to create their own support system by bringing in extended family, friends and community members
that they feel would be helpful in successfully achieving their goals. If you have more questions, please

UFS offers the Ute Tribe LIHEAP Program. Obtain an application from UFS and the following; attach IDs
(Tribal ID or Driver's License); copy of Utility Bill (Electric, Gas, Propane) with name/address of provider
and account number on bill; Social Security card (s)-must present cards of entire household (necessary
for anyone age of 6); Birth Certificate of Children 6 years and under; Proof of income of all household
members (last 4 check stubs, and/or copies of other assistance programs; Social Security, (SSI), Veterans
benefits, GA, Food Stamps (copy of check/bank statements and/or documentation of funds).
Parenting Classes: Love, Limits and Latitude

Classes are offered in 11 sessions per cycle. The curriculum used is called " Love, Limits and Latitude."
These classes will be held every Wednesday from 3:00 p.m. to 4:00 p.m. Classes begin on Wednesday,
January 8, 2014 and concludes on March 19, 2014. These classes are considered "open", meaning that
anyone can begin at anytime. These classes are free and will help participants to build a loving
relationship with their children so that they are easier to parent, know how to set appropriate limits and
understand the unique need of children. These classes are for anyone who is a parenting a child or

Counseling referrals are accepted for adolescent youth and families needs such as: couples counseling,
depression, anger management, grief, ungovernable behaviors, school achievement, attachment,
transition, etc. Youth must be at least 12 years old. For children younger than 12, counseling services is
offered for caretakers to improve behavior in their young children. In addition, an “open” Parenting

classes is offered every week where parents are able to join at any time to gain additional skills in
managing their children.

The purpose of the victim's advocate program is to help victims and their children access utilize local
resources available to them such as protective orders, emergency shelter, education, counseling, and
resource and crises intervention, which can prevent the cycle of family violence. Victims of any and all

Little Blossom Center is a satellite program of Prime Time 4 Kids, which is an early intervention program
for children ages 0-3, developmental delays and disabilities. They are supported by an experienced team
of trained early interventionists, therapists, and nurses that provide free developmental assessments.
Each eligible child is receives follow-up services, home visits, playgroups, specialized instruction, and
collaboration with other child service agencies. Prime Time 4 Kids is a family based service to assist
families with development services as well as provide strategies for children with developmental delay.

Ute Family Services offers financial assistance for basic needs of eligible American Indians who reside on
or near the reservations. Services include cash payments, basic needs, food, clothing, shelter, and
assistance for non-medical institutional or custodial care of adults and children who need protection.
Qualification is determined through an eligibility worker. Donation program is available for children in
the Foster Care Program and families in need.

The Urban Indian Center of Salt Lake City (Formerly Indian Walk-In Center) strives to "Preserve the
heritage, enhance the wellbeing and strengthen the future of the people." Services offered for eligible
American Indians in Northern Utah region include Healthcare access & referrals; immunizations,

week. They also provide emergency shelter for those who have been affected by domestic violence.

The Workforce Investment Act is a federally funded program under the Department of Labor's section
166 Indian and Native American Programs. The WIA program serves low income, unemployed or
underemployed American Indians residing in Uintah, Ouray reservation, or Uintah and Duchesne
counties. We provide temporary employment opportunities for Native Americans to obtain skills, and

WIC's mission is to safeguard the health of low-income women, infants, and children up to age 5, who

The IHS is the principal federal health care provider and health advocate for Indian people, and its goal is
to raise their health status to the highest possible level. Eligible individuals and families must show
membership of a federally recognized tribe. Services include ambulatory 40-hour facility staffed by five
physicians, one physician's assistant, three dentists, one optometrist, and three pharmacists. We
provide comprehensive public health services including general medical, surgical follow-up, pediatric,
prenatal care. Dental and optometry services are also available. Other clinical specialties are provided by
visiting consultants, which are scheduled periodically, and Behavioral Health service providers are also
available.

The Urban Indian Center of Salt Lake City (Formerly Indian Walk-In Center) strives to "Preserve the
heritage, enhance the wellbeing and strengthen the future of the people." Services offered for eligible
American Indians in Northern Utah region include Healthcare access & referrals; immunizations,
medical, dental, HIV & STI testing & counseling, CHIP/Medicaid application assistance. Diabetes support;

cholesterol, ht & wt, fitness assessment & instruction. Behavioral Health Outpatient clinic; counseling
for youth & adults (Individual, couples and family), and Adult substance abuse treatment. Youth
Programs; physical, Commercial Tobacco & Substance prevention and Chronic disease prevention
programs; Spiritual; identity, formation, craft classes, storytelling, and language arts, Emotional Support;
homework Lab, College preparation; Leadership development, summer program, and youth
conferences. Additional programs include Special Projects which consists of prevention programs, family
health and education, cultural outreach, mentorship, family and community preparedness; and other
referrals for health screenings, resource information, clothing vouchers, volunteer work, community

They are supported by an experienced team of trained early interventionists, therapists, and nurses that

Referrals come from schools, church leaders, therapists, juvenile courts, state agencies, insurance
companies and community organizations. Families First is an in-home intervention designed to give
families the tools they need to solve their problems, and is a low-cost alternative to residential
treatment for troubled youth. UYU helps parents with support, regain control of the family, improve
relations between parent and child, helps the entire family, and not just the child. The intervention lasts
six weeks. Families First specialists are available for crisis help to the family for up to one year after the
service.

Kids is a family based service to assist families with development services as well as provide strategies

Ashley Valley Wilderness Inc. is a treatment program operated by Mountain Homes Youth Ranch. The
Ashley Valley Wilderness Program is an outdoor-based therapy and drug rehabilitation program for
young adults between the ages of 18-25 years old. It is operated from a more than 15,000 acre working
ranch in Vernal, Utah. Ashley Valley Wilderness offers treatment services for drug and alcohol-related
problems. It conducts physical activities to teach outdoor wilderness skills. The program includes
academic curriculum also includes science, social studies, physical education and language arts. The
program additionally offers therapy and counseling services.

The housing partnership is a cooperative between Duchesne County Justice Complex (DCJC), Uintah

Self -Help Housing is a program to assist low and very low income families or individuals to achieve the
dream of home ownership. Participants work together to build each other's homes. Participants are

It is painful to watch someone you love struggle with an addiction. If the loved one is an adult who is
refusing treatment, there is often a sense of helplessness which only enhances the pain. Please call us
and talk with one of our professionals who will provide concrete guidance tailored toward your specific
situation and make suggestions for ways to convince your loved one to come to our clinic where we
cannot only determine what type of treatment is necessary, but persuade him or her to follow through

with the referral. Often an individual who initially is unwilling to go to treatment will agree to meet with
us because we are not a treatment program. Although there is a onetime fee, if we can meet face to
face with you or loved one. the guidance we provide will be ongoing to help you navigate the system of

human services to the residents of Daggett, Duchesne and Uintah Counties. The overall agency goal is to
help those with mental health, emotional disorders or substance abuse problems to overcome their
challenges and become healthy, functioning members of society. The agency is supported through a
combination of public and private sources. Some services are eligible for private insurance and Medicaid
reimbursement. A sliding scale is available to the uninsured.

The volunteers of America Center for Women and Children is a 30 bed, social model residential
detoxification center for homeless women and children. The program provides a safe and supportive
living environment for women to withdraw from Alcohol and/or drugs. Women who enter the program
are taking their first steps on the road to successful transition from substance abuse, addiction and
homelessness. Therefore, the program seeks to provide women with the skills necessary to be
successful in treatment and begin the process of moving out of homelessness. There is no cost to Salt
Lake area residents. There is a $65.00 per day cost for outside residents.

The Volunteers of America Adult Detoxification Center is a 56 bed residential program where homeless
and low-income men and women can safely detoxify from Alcohol and/or drugs. This program is the
primary provider for detoxification services in Salt Lake City County. Once individuals have completed
physical detoxification, clients are assessed and referred for additional substance abuse services. While
in the program clients attend 12-step recovery groups and participate in case management services in
order to increase readiness for substance abuse treatment and a life in recovery. There is no cost to Salt
Lake area residents. there is a $65.00 per day cost for non-residents.
