The iDesign System is an ophthalmic diagnostic device that measures the refractive
error and wavefront aberrations of the human eye using a High-definition Hartmann-Shack wavefront sensor (five times higher resolution than recent WaveScan
system). The iDesign System provides the surgeon with many complex details

Today’s patients expect almost perfect vision after
cataract surgery. They want to see near without
glasses, and they desire comfortable vision without any complications such as halos and glare.
Because the Symfony IOL (Abbott Medical
Optics) is pupil independent, quality of vision
does not change as the pupil expands in dark
conditions. For those who drive at night or
enjoy hobbies typically done in poor light, pupil independence
is important.
Although multifocal IOLs continue to undergo modifications
to enhance distance and near vision, unwanted side effects are
still problematic. The Symfony IOL does not cause any disturbance in distance vision and can compensate for chromatic aberrations between the shorter blue and longer red wavelengths.
This is because the lens does not have multiple focal points, as
other multifocal IOLs do. When light approaches an echellette
grating surface, as on the Symfony’s optic, it is diffracted into specific directions that vary according to step height and area. The
design of the Symfony, therefore, provides an extended range of
vision to the patient without producing glare and halos.
In my limited experience with the IOL, which includes four eyes
to date, distance BCVA always remains unchanged or improved
after surgery. At the final follow-up visits, distance visual acuity is
20/20 or better in the majority of eyes. Additionally, there have
been no significant complaints of visual disturbances.
Unlike multifocal IOLs that create a fixed number of focal points,
the Symphony has provided my patients with continuous sharp
vision from distance to near. The lens has provided improved contrast sensitivity and sharper distance vision compared with other
pupil-dependent multifocal IOLs that I have used. I plan to analyze
a larger series of patients with longer follow-up.

Importance of High Quality
Instruments in Operation Room
A surgeon performing a surgical procedure should be able to assume that the
instruments used are safe and reliable – particularly if they are new. So, as
a matter of fact an instrument which is handy and is made of high quality
materials would be inevitable in operation rooms during a surgery.
Stainless steel instruments may appear to be of uniform quality
when they are new. However, there are various grades of quality,
ranging from high quality and premium to operating room and
floor grade. Some instruments appearing to be stainless steel are
of such poor quality that they are sold as single-use instruments.
Quality is determined by the manufacturer as instruments
represent a substantial portion of the budget of a surgical suite,
it is important to be knowledgeable about buying and selecting
products with the desired quality. Many factors affect quality.
Two major factors are a balanced carbon-chrome ratio and the
process of passivation. A balanced carbon-chrome is important for instrument strength and long life. Instruments that are
classified as premium have the correct balance. The passivation
process is important to create a proactive coat on the outer layer
of an instrument to prevent corrosion and extend its life. Electropolishing is sometimes substituted for passivation. The result
is a less expensive instrument but one that will not last as long.
When purchasing stainless steel instrument, it is best to deal
with a reputable manufacturer who will explain the variation in
quality of the product available.
Instruments manufactured with materials other than stainless
steel present an additional set of factors to consider before purchasing. These include their ability to be disassembled, cleaned
and reassembled; their life expectancy; and their compatibility
with the existing resources within the sterile processing department (e.g. cleaning chemistries, disinfecting agents and sterilization modalities) available in the center.
