TO OUR SUPPORTERS
AND ADVOCATES
We are most grateful for the trust and support you have given us over
this past year and eagerly look forward to continue building on the
legacy of the Foundation as we support our local health care system.
Throughout the 29-county region served by Mountain States Health
Alliance, we have volunteers who work tirelessly to support our
fundraising events. Foundation efforts would be for naught without
the ongoing generosity of our donors and partners. It is only through
your kindness and support that we can continue to serve the children,
families and individuals of this region. I hope you have a sense of
accomplishment in knowing that you are making a difference.
WHO WE ARE
OUR FOUNDATION SUPPORTS A TRULY LOCAL HEALTH CARE
SYSTEM. MUCH OF THE WORK WE DO GOES TO SUPPORT NEW
PROGRAMS AND HEALTH CARE SERVICES ALLOWING PATIENTS
TO RECEIVE LOVING CARE RIGHT IN THEIR OWN COMMUNITIES,
RATHER THAN HAVING TO TRAVEL FOR TREATMENT.
We are proud of the legacy of the Mountain
States Foundation. In our 34-year history, we
have served the people of our community by
providing funding for services and programs
that directly benefit our patients. We believe
in our charter of Changing Lives Through Giving.
In FY2014, we successfully funded pediatric
radiation oncology services at Niswonger
Children’s Hospital. We also provided funds
for radiation oncology’s TrueBeam linear
accelerator that provides cancer treatments
with pinpoint accuracy.
We are eager to continue building on our legacy
as we look to the future of the Foundation.
Whether we reach out to donors through
our signature events, such as the Niswonger
Children’s Hospital Golf Classic or the annual
Spirit Gala, or engage our community with the
Niswonger Children’s Hospital Radiothon, we
consistently strive to provide opportunities to
fund the health care services most needed
in our region. We know that excellent and
comprehensive service at our hospitals means
that the people of this community can receive
the care they need right here at home.
Reflecting back on 2014, the entire community embraced the Radiation
Oncology Capital Campaign. Everyone has been touched by cancer,
either through family or friends. By partnering with Mountain States
Health Alliance, we have had the opportunity to make a difference
through advanced technology and equipment, a more healing
environment and dedicated space for pediatric patients. We are
deeply grateful to you for your support, which will make a difference
for years to come.
As we move into this next year, we will continue to work tirelessly to
support local health care needs, with a special focus on Niswonger
Children’s Hospital. While we have already achieved so many worthy
goals, we know that the children of our region are still in need of
specialized services available at our local facility. We are excited to
have you join with us as we begin a new journey to serve the children
of our region, and to offer local children and families the very best in
health care, while allowing them to stay close to home.

While in hospice, many children look for something solid
to hold on to. One of our doctors discovered something
called a “Holding Cross” made out of smooth, unfinished
wood, and in a shape that fits perfectly in the palm of
the hand.

OF HOPE

As the patient grasps it the oils made from the hand
absorb into the wood, creating a unique stain from the
child’s own skin. These crosses were purchased for use
with our hospice services.

LARRY ENGLAND
2014 TOM CHASE AWARD RECIPIENT
The Tom Chase Award recognizes those who strongly support the
community without necessarily taking a top leadership role. This
award is given to someone who exhibits four qualities: integrity,
informal leadership, working behind the scene(s) to get things done
and willingness to go the extra mile.
Larry England and his wife Debbie own four Cartridge World
locations in Northeast Tennessee, opening their first three locations
in 22 months. Their first store opened in their home town of
Johnson City, Tennessee, in January of 2005 and since opening has
been the top performing store in the state. After graduating high
school in 1972, Larry worked in his family’s furniture factory until the
business was sold to La-Z-Boy in the spring of 1995. In 2009, Larry
and Debbie received a Blue Ribbon Small Business Award from the
U.S. Chamber of Commerce in Washington D.C. In that same year
they were recipients of the Cartridge World President’s Award and
the Community Service Award, presented by then Cartridge World
President Steve Yeffa, at the 2009 National Convention in Nashville.
ABOUT THE
TOM CHASE AWARD
The Tom Chase Award is
given to someone who
exhibits four qualities:
integrity, informal
leadership, working
behind the scenes to
get things done and
willingness to go the extra
mile. In ancient times,
a Roman soldier could
legally force anyone on
the road to carry his pack
for one mile. One man
exhorted his followers to
go voluntarily an extra
mile. The Tom Chase
Award recipient is such
a person.
DENNIS VONDERFECHT
2014 SPIRIT AWARD RECIPIENT
“This year’s Spirit Award winner is a man without whom there would
be no Mountain States Foundation, because there would be no
Mountain States Health Alliance,” said Pat Holtsclaw, Mountain States
Foundation president. “When Dennis came to Northeast Tennessee in
1990 as CEO of Johnson City Medical Center, he came with a vision for
what our region’s health care services could become.
"Without his vision, strategy, and long-term commitment, we would
not have the outstanding health care resources that we enjoy in our
region today.”
In 1998, Dennis Vonderfecht’s strategic vision led Johnson City
Medical Center to purchase six hospitals from Columbia HCA, forming
Mountain States Health Alliance, with Johnson City Medical Center
as its flagship. As the newly-formed health system began to develop
its own identity and character, he helped define the organization’s
mission, vision and values, along with 10 guiding principles of patientcentered care that help to keep all team members engaged in their
mission of bringing loving care to health care. These ideals, coupled
with a rigorous strategic planning process, have allowed the health
care system to grow rapidly over the last 15 years while maintaining a
culture centered on the patient’s needs.
ABOUT THE
SPIRIT AWARD
Mountain States
Foundation recognizes
one individual annually
who makes a difference
in our region through
their commitment,
dedication and support
of community needs.
Twenty-one recipients
have been recognized
for demonstrating these
qualities since 1990.
Today Mountain States Health Alliance serves people in 29 counties
with a family of 14 hospitals and numerous outpatient clinics, along
with retail pharmacies, home health and hospice services and the
region’s only provider-owned health insurance company. Among his
most treasured achievements are the establishment of Wings Air
Rescue, the region’s first air ambulance service, and the creation of
Niswonger Children’s Hospital.
When he retired from Mountain States Health Alliance at the
end of 2013, Dennis had completed 24 years of service as CEO —
a remarkable tenure in an industry where CEOs stay in one place,
on average, less than five years.

DR. LESLI TAYLOR
Dr. Lesli Taylor was nominated for her award by Sarah Parker, whose son
Rowan was born with gastroschisis and spent the first 10 months of his
life at Niswonger Children’s Hospital. The Parker family was present at the
luncheon to see Dr. Taylor’s award presentation. Right after Rowan was
born, Dr. Taylor told the family she had a plan to save Rowan, despite the
serious complications, but it would take a long time.
HELPING HANDS
CLARINDA JEANES
We had a patient from Bluefield, Va., which is a 2-1/2-hourdrive from Woodridge Hospital. Her family did not have enough
money to drive to Woodridge to pick her up and bring her home.
When we began to plan for her discharge, the family tried for
several days to get together enough funds to be able to travel to
Woodridge in Johnson City.
Clarinda Jeanes serves as chairperson of the Mountain States Foundation
Board of Trustees and has helped lead multi-million-dollar fundraising
campaigns to benefit Niswonger Children’s Hospital and Mountain States
Cancer Services. Special events revenue has more than tripled during her
time as chairman of events.
SCOTT NISWONGER
Scott Niswonger and his wife Nikki donated $10 million toward the
construction of the 69-bed children’s hospital that now bears his name.
He continues to take an active interest in the hospital, which he’s called
“the best investment I ever made.” Niswonger also is very active with
the organization of the Niswonger Children’s Hospital Golf Classic, a
fundraiser that brings in celebrity athletes.

RADIOTHON
NISWONGER GOLF CLASSIC
FOR THE SECOND YEAR IN A ROW,
THE REGION’S RADIOTHON TO SUPPORT
NISWONGER CHILDREN’S HOSPITAL FINISHED
ON A HIGH NOTE, WITH FUNDRAISING
SURPASSING $140,000.
“We are proud to announce that the radiothon
raised more than its anticipated goal,” said Don
Raines, Niswonger Children’s Hospital Radiothon
committee chairperson. “We are so humbled by
the generosity that the residents of this region
have shown, and so encouraged by what those
donations can do for our children.”
Local radio stations 98.5 WTFM, 95.9 The Hog,
Classic Hits 102.7, FM 94 WMEV, and ESPN Radio
teamed up with Mountain States Foundation to
take pledges from individuals and businesses to
support the region’s only children’s hospital.
“This is the first event of its kind to support
Niswonger Children’s Hospital,” said Steven
Godbold, the hospital’s CEO. “Contrary to popular
belief, our region’s children’s hospital does not
receive any funds from the Children’s Miracle
Network, so we are immensely grateful to the
fine folks at Holston Valley Broadcasting’s radio
stations for their enthusiastic and generous
support.”
CORPORATE SPONSORS FOR THE EVENT:
Walgreens, Chick-fil-A, Food City, BedinaBox,
Delta Dental, UBS, Arby’s and SportClips Haircuts.
“We know the money our corporate sponsors and
listeners donate during the Niswonger Children’s
Hospital Radiothon makes a positive and
comforting difference for our region’s children and
their families,” said David Widener, executive vice
president and general manager for Holston Valley
Broadcasting, the radio stations’ parent company.

CHARITABLE
GIFT
PLANNING
WHY MAKE A
PLANNED GIFT?
THE TOOLS FOR PLANNED
GIVING INCLUDE:
• To make a larger gift than you
thought possible
• Gift annuities
• A bequest
• To increase your current income
through income-producing trusts
or annuities
• Charitable remainder trusts
• To plan for the financial needs of
a spouse or loved one
We are here to help. Call either of the
gift officers listed here, or phone the
Mountain States Foundation office at
423-302-3131. Planned giving is for
anyone – whether you are planning
a will, seeking tax information or
making a heartfelt gift. We are eager
to help you make the choice that is
right for you.
• To provide inheritances for your
heirs at a reduced tax cost
• To reduce your income tax or
avoid capital gains tax altogether
• To diversify your investment
portfolio
• To receive income from your
personal residence or farm
• To plan for the transfer of your
business
• To leave a charitable legacy for
future generations by supporting
your health care system through
Mountain States Foundation
Charitable gift planning is an
opportunity for charitable giving in
circumstances that may not otherwise
allow a donor to make a gift to charity.
This type of planning involves the
process of integrating personal,
financial and estate goals with lifetime
or testamentary charitable giving.
Charitable giving covers the full
spectrum of generosity by individuals
and institutions, and is based on a
proud tradition of giving in the United
States. It uses a number of financial
tools and techniques for giving, and
when appropriate, uses tax incentives
to encourage charitable giving.
Planned gifts include a variety of
tools to help achieve the goals of the
donor. They may be made with cash,
stock and bonds, and/or an IRA, but
many are made by donating assets
such as real estate, art pieces or
business interests – the possibilities
are endless.
• Charitable lead trusts
• Bargain sale
Making a planned charitable gift
to Mountain States Foundation
for use by the Mountain States
Health Alliance usually requires the
assistance of one of the Foundation’s
planned giving development
professionals, or a knowledgeable
advisor such as an estate planning
attorney, financial planner or CPA.

and personal care items. The program also has paid for other essentials
– such as electric bills – depending on the patients’ greatest needs. The
program is supported in part by the Hope on Wheels motorcycle ride,
which involves a 77-mile route from Johnson City, Elizabethton, Stoney
Creek and on to Bristol. In 2014, the 50 motorcyclists raised $7,900 to
continue supporting the Circle of Hope program.
GROWING HEALTHY
Growing Healthy serves the six school systems of upper East Tennessee.
The program is committed to promoting healthy lifestyle choices through
its efforts to help schools and communities effectively link health with
educational achievement and improve student’s overall well-being.
Growing Healthy focused its efforts on helping educators develop quality
planning/instruction lessons that integrated health concepts within all
curriculum areas in order to improve student health and achievement.
Trainings were conducted for second and third grade. Teachers attended
several three-day in-service trainings where they received instruction on
how to incorporate a resource-rich health curriculum that uses a variety
of peripheral materials to engage and captivate their students in their
classroom. Teachers participating received training on how to enhance
and incorporate the health curriculum with the new state common core
standards and the increasing demands on state testing scores.
HELPING HANDS
The Helping Hands Program was started in 2008 after discovering that
many of our caregivers were taking money out of their pockets to help
patients with a cab ride home, a meal in the cafeteria for a family member
or prescription assistance. Since that time, the program has given over
$115,000 to help patients in need of transportation assistance with taxi
or gas vouchers, non-coverred medications and meals. The program
currently operates in all 14 of our hopsitals and is generously supported
by our team members.

FY14 SUPPORT SERVICES
FY14 SUPPORT SERVICES
HOSPICE PROGRAM
PARISH NURSING PROGRAM
Mountain States Hospice serves about 700 patients and families per year.
These services are offered in both Tennessee and Virginia and can be
provided in a wide variety of settings, including the patient’s home, nursing
homes and assisted living homes. Inpatient hospice also is offered in most
Mountain States Health Alliance acute care settings. If a patient does not
qualify for hospice under Medicare guidelines, Mountain States also offers
a palliative care program under the home health benefit. Hospice services
include skilled nursing visits, social workers who assist with end-of-life issues,
chaplains and volunteer services. In addition to these services, the Hospice
Program will pay for most supplies and any medications related to the
patient’s diagnosis.
With over 6,000 phone calls, 800+ hospital visits and almost 1,000
home visits, our parish nurses touch thousands of lives each year. The
Parish Nursing Program also provides for counseling services, blood
pressure clinics, office visits and services at nursing homes and assisted
living facilities. To fully get a grasp of parish nursing, see the letter
below about one of our nurses. It is used with permission of the nurse.
“I want to let you know how much I appreciate the assistance (the
parish nurse) has provided to Mother and me. She has accompanied
Mother to doctor visits and helped her to understand the terminology
and procedures. She has provided me with counseling about skilled
nursing facilities for Mother’s hospital aftercare, and, in my absence,
she assisted Mother in transition from the hospital to nursing facilities
on more than one occasion. (Her) unselfishness in sharing her time and
talent in the medical field has increased our ability to make informed
decisions. This facet of the church’s ministry is a godsend to those who
have the misfortune of needing medical assistance. Living so far away,
I could not have managed so well without her. (She) is truly living out
her parish ministry.”
Through monies received from the Foundation, Mountain States Hospice is
able to provide financial assistance on an individual basis to our patients as
well as quality bereavement materials to our families.
Hospice provides continuing contact and support to family and loved ones
for at least a year following their loved one’s death. This year we have
implemented a process to hand-deliver the initial bereavement packet to
provide more support to the families. Other mailings include brochures,
newsletters, resource letters and sympathy cards at the one-year anniversary
of their loved one’s death.

Grief support groups are conducted as well as individual grief counseling.
A variety of books and other printed material is offered to our survivors.
CIRCLE OF HOPE
A child with leukemia lived with her single mother,
whose phone service was cut off. This affected the
daughter’s treatment, as it was important for caregivers
to be able to reach the mother at any time to discuss
changes her daughter’s health.
A patient receiving care at Mountain States Cancer Care
at Johnston Memorial Hospital had received an eviction
notice. She had always taken pride in paying her bills, but
since going on short-term disability, she was having a hard
time making her payments.
STORIES
OF HOPE
AND
HEALING
A representative from Circle of Hope worked with the
patient’s landlord, and was able to use Circle of Hope
funds to enable the patient to stay in her home. Now, she
has completed treatment and is scheduled to return to
work shortly. She is extremely grateful for the difference
Circle of Hope made in her treatment, and in her life.

CIRCLE OF HOPE
STORIES
OF HOPE
AND
HEALING
We had a patient living an hour away in Flag Pond, Tenn.
He was struggling to be able to afford the gas to come
for his daily radiation treatments. It got to the point where
his family was running out of food and couldn’t afford
their prescriptions. Through the Circle of Hope program
we were able to help him with gas cards, along with food
donated by our team members. This allowed him to be
able to finish his radiation treatment.

MOUNTAIN STATES FOUNDATION | Changing Lives through Giving | WWW.MSHAFOUNDATION.ORG
We had a patient who informed us that she would have
to stop all treatment for her breast cancer. She lost her
insurance and was two months behind on her rent. With
children at home, she had no other option but to go back
to work full time in order to avoid being evicted, leaving her
no time to get the chemotherapy she needed. Through the
Circle of Hope program we were able to pay her past due
rent so she could complete her treatment.

The toy room in the surgery center at
Niswonger Children’s Hospital is chockfull of toys ready to help cheer up young
patients, thanks to funds raised last year
by Niswonger Children’s Hospital license
tag sales. But the hospital needs help if
the program is to continue another year.
Every registration we receive helps to
ensure that this program will continue
to assist local children. Thank you for
showing your support for Niswonger
Children’s Hospital.
Purchase your license plate at your local
county courthouse or by calling Mountain
States Foundation at 423-302-3131.
