Welcome to Litchfield Park, Arizona and the
Arizona Pharmacy Association 8th Annual
Meeting at The Wigwam! This historic property
is the perfect venue to commemorate Arizona’s
Centennial and the history of pharmacy in
Arizona. The theme of this year’s conference is
Arizona Pharmacy Pioneers: Past, Present, and
Future. Over the next few days we will celebrate
the accomplishments of our profession and lay
the groundwork for the future contributions
that pharmacists and pharmacy technicians will
make as health care providers. Attending this
conference is a great way to prepare for the road
ahead. While participating in the informationfilled continuing education activities, enjoy
reconnecting with old friends and building some
new relationships.
In addition to the continuing education
activities, please take time to visit the Westernthemed Exhibit Hall where you will have the
opportunity to network and socialize with
your colleagues and learn about the latest
pharmaceutical products, technology, and
services. Come enjoy the food and fun, and maybe
even try your hand at the shootin’ gallery.
Please plan to join us for the Annual AzPA
Awards Banquet on Saturday evening. This
celebration marks the transition of our
Association leadership and recognizes and honors
our peers and colleagues. It is always a special
evening. Rumor has it there may be an oldfashioned duel!
Thank you to Jon Glover, Annual Meeting
Steering Committee Chair, and to the entire
steering committee. This year’s exceptional
programming is a testament to their
professionalism and dedication. Thank you,
too, to the AzPA Board of Directors, Academy
leaders, and Committee members for your hard
work and support of the Association and our
profession. Last, and certainly not least, I extend
my tremendous gratitude to the AzPA staff - Kelly,
Kathy, Janet, Cindy, Deb, and Lisa. These are the
individuals who get the job done!
This past year has seen many changes for our
organization, including a name change and a
new Chief Executive Officer, Kelly Ridgway. The
enthusiasm and commitment she brings to AzPA
has reenergized our Association.
As I conclude my year as AzPA President, my
wish is that when you return to your practice site
after attending this year’s meeting you will share
the benefit of your experience with your fellow
professionals and encourage them to become a
part of the Arizona Pharmacy Association, the
voice - YOUR VOICE - of pharmacy in Arizona.

Please wear your name badge
at all times. It identifies you as
a participant at the conference
and serves as your meal ticket.
NO SMOKING PLEASE!
As a health-conscious
association, AzPA has adopted
a no-smoking policy at all
meetings. If you choose to
smoke, we ask that you use
the designated areas. Please
remember that smoking is
prohibited in meeting rooms and
at indoor meal functions.

presenters and other attendees

to vibrate, silent, or off prior to
entering the meeting rooms.
CHILDREN ARE WELCOME!
Children are welcome to attend
the Exhibit Hall reception &
lunch. However, each child (ages
5-18) must be registered and
wearing a name badge to enter
either the Exhibit Hall reception
on Friday night or Saturday
lunch. The cost is $10 per child per
meal function.
CONVENTION ATTIRE
Please dress comfortably!
Convention attire, including
the banquet, is resort casual! A
“No Tie Policy” will be in effect.
A reminder: Meeting room
temperatures and personal
comfort vary widely. Even
though it’s summer in Arizona,
meeting rooms always seem
cold, so please bring a sweater.
REFUND POLICY
Refund and/or cancellation
requests must be made in
writing to the Arizona Pharmacy
Association office before
June 1, 2012. A $50 service charge
will apply to all cancellations.
Substitutions are welcome.
Refund checks will be mailed two
weeks after the conclusion of
the annual meeting. No refund
requests will be honored after
June 15, 2012. AzPA reserves
the right to cancel any activity
because of insufficient preregistration and as determined
by the CEO or CPE Administrator.
In this case, all individuals
pre-registered are notified and
issued a full refund.

Upon successful completion of
each evaluation form, your CE
Statement of Credit for that
activity will be sent to you via
email. Login is required to
complete the evaluation forms
and receive your CE Statements
of Credit. You will also need your
NABP eProfile ID number.
In order to avoid additional fees
for issuance of CE Statements
of Credit for this conference, all
evaluation forms MUST be
completed by August 1, 2012.
After this date, online access
to the evaluation forms will be
closed and requests for issuance
of CE Statements of Credit from
the AzPA Annual Meeting will be
subject to a $10 per certificate fee.
No exceptions.
CE SESSION CODES
All participants receiving ACPE
credit must have attended all
sessions indicated, completed
a session evaluation form for
each session attended, and are
responsible for tracking the CE
Session Codes for the sessions.
This code will be announced
throughout each session. The
session room monitors will also
have this number. CE Session
Codes are not printed in the
program and will not be given
out anywhere else except in the
sessions attended. You must have
this code to receive CE Credit!

Continuing Pharmacy Education Information
CPE Requirements: Qualified attendees can earn up to 12.0 hours (1.20 CEUs) of continuing
pharmacy education (CPE) credit. ACPE Universal Activity Numbers of CEUs are listed in
the on-site program and on the online evaluation forms. Learning objectives for all CPE
sessions are listed within this on-site program. Program validation codes will be announced
at the beginning of each session. You must track these validation codes on the Session Code
Tracking Form found on page 25 of this program and then complete the online evaluation
forms (see instructions on page 25) to receive your Continuing Education Statements of
Credit.
Disclosures: Financial support for individual education sessions are listed within the on-site
program. Faculty disclosures are provided on every program evaluation form.
Grievance Policy: All grievances regarding courses, personnel, or practices at any
continuing pharmaceutical education program should be brought to the attention of the
CPE Administrator. Every attempt will be made to satisfy the complainant. In addition,
measures shall be taken to correct deficiencies. Those program participants who wish to
appeal their participation at a program should address such an appeal in writing to the CPE
Administrator Kathy Harty, Arizona Pharmacy Association, within 10 working days of the
program. The Program Administrator will convene an appeal process committee consisting
of the members of the CE Committee within 30 days after the filing of the appeal. The
written results of the appeal will be shared with the participant within two weeks following
the committee meeting.
The Arizona Pharmacy Alliance is accredited by the Accreditation Council for
Pharmacy Education as a provider of continuing pharmacy education.

What is CPE Monitor?
A national collaborative effort between the National Association of Boards of Pharmacy®
(NABP®), the Accreditation Council for Pharmacy Education (ACPE), and accredited continuing
pharmacy education (CPE) providers, the CPE Monitor service will allow you to easily track
your ACPE-accredited CPE units electronically.
What are the benefits of CPE Monitor?
• Streamlines the reporting and compliance verification process.
• Eliminates paper forms and the need to submit paper copies of CPE statements of
credit for ACPE-accredited activities in most jurisdictions.
• As part of your NABP e-Profile, the CPE Monitor service will store a comprehensive
list of your CPE activities completed and will allow you to verify compliance with CPE
requirements.
Pharmacists and technicians are required to provide their NABP e-Profile ID, plus their birth
date (MMDD) to AzPA in order to receive credit for completed CPE. As a secure, central system,
CPE Monitor will assist licensees by maintaining & tracking all ACPE-accredited CPE units.
AzPA is required to transmit CPE data to CPE Monitor after December 31, 2012. The AzPA 8th
Annual Meeting programs will not be submitted on this system. All programs commencing
after January 1, 2013 must be uploaded to ACPE which will then be transferred to the NABP CPE
Monitor platform. Until 2012, please save your statements of credit for all AzPA CE activities
you have attended, including the AzPA 8th Annual Meeting.

Pharmacists and pharmacy technicians wishing to receive an ACPE Certificate of
Completion for Continuing Pharmacy Education for the activities presented at the
AzPA Annual Meeting MUST have attended all sessions indicated, complete an online
evaluation form for each activity, and are responsible for tracking the CE Session Codes
for the sessions. This code will be announced throughout each session. The session
room monitors will also have this number. CE Session Codes are not printed in the
program and will not be given out anywhere else except in the sessions attended.
