What is Synthesized ECG ?
Synthesized 18-lead ECG uses the 12-lead ECG waveforms to mathematically derive the waveforms of the right chest leads (V3R, V4R, V5R)
and back leads (V7, V8, V9). The measurement procedure is the same as the standard 12-lead ECG but more information can be obtained.
18-lead synthesized ECG is expected to be useful in detecting right side and posterior infarction.
The most common ECG exam is the standard 12-lead ECG. It is simple to measure, has low burden on the body, and observing the heart
from these 12 directions provides a lot of information which has a wide range of clinical applications.
However, some areas, especially pathological change in the right ventricle and the posterior wall cannot be observed from the
12-lead ECG. In order to actually measure the right chest (V3R, V4R, V5R) and back (V7, V8, V9) areas, it is necessary to use different electrode positions than the standard 12-lead ECG. In particular,

In 1984, NIHON KOHDEN implemented a comprehensive companywide quality system, IQC – Integrated Quality

manufacturing process, from the planning stage through product development, design, parts procurement,
production, inspection, storage, sales, distribution and after-sales support. IQC also involves nearly every employee.
Our rigid test procedures are structured to exceed the provisions of the GMP, Good Manufacturing Practice, codes
enforced by the US Food and Drug Administration. NIHON KOHDEN also has 130 in-house quality control inspection
staff to ensure that standards are implemented and maintained in every department of the company.
IQC extends beyond production and into sales and support at each subsidiary. This staff is responsible for ongoing
contact with customers to determine their needs and making sure that these changing requirements are reflected
in new products. Service engineers are responsible for communicating any problem that may arise in the use of
NIHON KOHDEN products, and assuring the customers of NIHON KOHDEN’s commitment to their satisfaction.
In 1995, NIHON KOHDEN received ISO 9001 certification, the international standard for quality systems. In addition,
NIHON KOHDEN’s four manufacturing facilities in Japan are the first factories in the medical equipment industry to
be certified by the CSA, Canadian Standards Association, under their category certification system. Most products

It is important to prepare the skin before attaching the electrodes
by cleaning it and removing chest hair. After this please attach
the lead wire to the electrodes before placing them on the skin.
This avoids pain to the patient. Secure the lead wire to the skin
(stress loop) preventing moving of the lead wire and therefore
noise on the ECG signal.

Facts
The NIHON KOHDEN Fastclip ECG Electrodes allow a flexible application for routine work and emergency on every paediatric and adult
patient thanks to a simple adjustability of the clip pressure (clip size). A more comfortable handling is guaranteed due to different con-nectors for any
existing adapter type, a big contact area for reliable ECG measurements and easier cleaning after the measurement because of less asperity of the clip.
In addition to this IEC colored label sets are delivered with NIHON KOHDEN Fastclip ECG Electrodes.
By following the next steps, optimized derivations can be realized with NIHON KOHDEN Fastclip ECG Electrodes:
1.) Clean the according skin areas with alcohol in order to remove traces of dirt and oil on the skin.
2.) Apply a thin layer of CardioCream F010 on the cleaned skin areas and on the electrode part.
3.) Clip the electrode on the wrist or ankle. Adjust the electrode positon by sliding the electrode part according to the thickness of the limb.

NIHON KOHDEN VITRODE Disposable ECG Electrodes –
The result of over 30 years experience in disposable electrode manufacturing.
The ECG electrodes are the most important portions of the ECG/arrhythmia monitoring system. It is through these electrodes that the electrical
signals are conducted from the patient and into the monitoring system. If the electrodes are left in place for an extended period of time (over
48 hours), or they are reused once they are removed, they do not adhere to the patient’s skin and they cause “noisy’’ ECG signals because of
the poor conduction. In addition, the patient’s movement causes the electrodes to lose conducting capabilities intermittently, and this leads
to a “wandering’’ ECG waveform and muscle artifact. The standard monitoring practice is to change these electrodes at least every 48 hours.

The result of over 30 years experience in disposable electrode manufacturing.
The ECG electrodes are the most important portions of the ECG/arrhythmia monitoring system. It is through these electrodes that
the electrical signals are conducted from the patient and into the monitoring system. If the electrodes are left in place for an extended period of time (over 48 hours), or they are reused once they are removed, they do not adhere to the patient’s skin and they cause
“noisy’’ ECG signals because of the poor conduction. In addition, the patient’s movement causes the electrodes to lose conducting
capabilities intermittently, and this leads to a “wandering’’ ECG waveform and muscle artifact. The standard monitoring practice is
to change these electrodes at least every 48 hours.

Standard Electrodes Placements
It is important to prepare the skin before attaching
the electrodes by cleaning it and removing chest
hair. After this please attach the lead wire to the
electrodes before placing them on the skin. This
avoids pain to the patient. Secure the lead wire to
the skin (stress loop) preventing moving of the
lead wire and therefore noise on the ECG signal.

A Pulse Oximeter is a non-invasive device used to percutaneously measure oxygen saturation
in the blood by use of a clip-on finger probe. Pulse oximeter works with the integration of a
probe placed onto patient’s fingertip or toe, obtains signals from the probe and the main
device calculates oxygen saturation in the body.
Oxygen saturation measured by a Pulse Oximeter is SpO2. “P” stands for “pulse”, and the
measurement principle is based on beat-by-beat changes in blood volume. In general, arterial oxygen saturation measured by spectroscopic analysis is called “SaO2”, and oxygen saturation measured by pulse oximeter is called “SpO2”.
The method for determination of arterial oxygen saturation, based on the photoelectric plethysmography, was conceptualised and implemented in 1972 by the Japanese bio-engineer
Takuo Aoyagi (NIHON KOHDEN Corporation). This idea came about while using an ear densitometer for recording dilution curves for dyes (pigments). The first commercial oximeter
“OLV-5100” was presented by NIHON KOHDEN back in 1975.
Dr. Aoyagi’s innovative technology, pulse oximetry, has rapidly spread worldwide and become essential in critical care. In
2002, Dr. Aoyagi was awarded the Medal of Honor in Japan, in recognition of his great contribution to clinical medicine.

Handling of the probe affects measurement accuracy and patient safety
For the proper placement of the probe on the finger, it is advisable that the light emitter and detector are placed just opposite to each
other. When using a finger clip probe, it is necessary to ensure that the probe covers the finger properly, otherwise some portion of the light
never passes through the body tissues and may reach the light-receiving part directly, leading to incorrect measurement results.
The more distant the light emitter and detector gets, the weaker the transmitted light intensity becomes. Additionally, a thinner body part
tends to have small amount of blood, which makes it difficult to extract the arterial blood signal component. The most suitable thickness
of the probe site is about 10 mm.
Too loose probes may interfere with alignment of LEDs and detector, come off easily by body movement, cause larger motion artifacts or
be effected by venous pulse.
Too tight probes may cause venous pulsation, cause skin injuries such as burn and necrosis or unstable measurement because of a lower
pulsatile signal.
Reasons to measure ETCO₂ (the level of carbon dioxide in the air exhaled from the body –
normal value is about 38 mm Hg): Capnography helps to detect life-threatening problems in:
• Airway
• Breathing
• Circulation and
• Metabolism
Capnography is a fast and reliable help to determinate respiratory difficulties, wrong connections of airway
adapters, leaks in the ventilator circuit due to its placement in the respiratory circuit. An early detection by
capnography might help to avoid inadequate oxygen supply of the body.

NIBP Measurement (Non-Invasive Blood Pressure)
Blood pressure (BP) is the pressure exerted by circulating blood against the artery walls. During each heartbeat, the blood
pressure varies between a maximum (systolic) and a minimum (diastolic) pressure. The variation indicates the condition
of the pumping function of the heart as an important vital sign.
A non-invasive indirect method to measure the blood pressure is the oscillometric blood pressure measurement. This method is using an inflated blood pressure cuff which is applied to the patient’s arm or thigh. The blood pressure is detected
by partial occlusion of the artery by the cuff. The so called Korotkoff sound is generated by the distension of the arterial
wall with each cardiac impulse. Systolic and diastolic blood pressures are determined by an algorithmic interpretation of
the device based on the mean arterial pressure and changing amplitude of the Korotkoff sounds.
For a proper non-invasive measurement it is important to use the correct cuff size due to the fact that a cuff too small will
lead to faulty high pressure, while a large cuff can lead to wrong low pressure readings.

Facts
The trade name YAWARA CUFF comes from the Japanese word, YAWARAKAI, which
means soft in touch.
YAWARA CUFF 2 is available in 6 sizes. The exterior size-scale quickly helps in
selecting the proper cuff size, from infant to large adult or thigh.
The velcro part of the cuff is divided into two high quality velcro types. This helps
to fix the cuff on the arm with the first velcro part whereas you can completely
adjust the cuff with the second velcro part.
YAWARA CUFF 2 uses a soft and smooth cloth together with a buffer sheet to reduce
the risk of subcutaneous bleeding. The fabric is hypoallergic and allows the skin to
breathe. Furthermore, the material of the cloth can be easily wiped clean.
A wider opening of the cuff cover allows very easy removal of the bladder for
cleaning.
The watertight connector cap keeps out liquid during cleaning the bladder. There
is a nylon fibre inside the air tube for kink-free tube. There is a nylon fibre inside the
air tube preventing kinking and incidents associated with tube kinking. YAWARA

Reducing internal bleeding
If wrinkles are formed as below, part of the measuring site may be
trapped in the valley portions of the wrinkles, which could cause internal

Importance of AED in the Chain of Survival
Automated External Defibrillator or AED is perhaps the most important
link in the chain to improve the SCA victim‘s survival chances.
An AED is a device that is used to diagnose and treat potentially life threatening abnormal heart rhythms by providing the means to apply controlled electrical shocks (known as defibrillation shocks) to the heart in
an attempt to stop these abnormal rhythms, thereby allowing the heart
to establish again an effective rhythm.
It will also provide guidance by simple and clear vocal instructions to the
rescuer during the rescue sequence.
Studies have shown that the time to first defibrillation shock is the most
crucial factor in determining survival rates for SCA*. When an AED is
available, it should always be applied to the SCA victim at the earliest.
