Fees: There is no registration fee for this conference, but advance registration is
required as space is limited. The Virginia Center on Aging, under the auspices of
the Commonwealth of Virginia, has allocated generous grant funds to Southwest
Virginia AHEC (Area Health Education Center) to administer this activity.
The Quillen College of Medicine, East Tennessee State University, is accredited by
the Accreditation Council for Continuing Medical Education to provide continuing
medical education for physicians.
The Quillen College of Medicine, East Tennessee State University, designates this
live activity for a maximum of 6.5 AMA PRA Category 1 Credit(s)TM. Physicians
should only claim credit commensurate with the extent of their participation in
the activity.
Joint Sponsorship: This activity has been planned and implemented in accordance with the
Essential Areas and Policies of the Accreditation Council for Continuing Medical Education
(ACCME) through the joint sponsorship of Quillen College of Medicine at East Tennessee State
University and the Southwest Virginia Graduate Medical Education Consortium (GMEC) and
Southwest Virginia Area Health Education Center (AHEC). Quillen College of Medicine is
accredited by the ACCME to provide continuing medical education for physicians.
The University of Tennessee College of Pharmacy is accredited by the Accreditation
Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy
education. Successful completion of this knowledge-based program will provide a
statement for six & ½ (6.5) live contact hours credit (0.65 CEUs) and will be mailed
within 3 weeks following the program. Successful completion includes attending
the session, signing the attendance sheet, and completion of the program evaluation form. CE
credit will also be submitted to the NABP CE Monitor.

Grievance Policy: A participant, sponsor, faculty member or other individual wanting to file a
grievance with respect to any aspect of a program sponsored or co-sponsored by the UTCOP may
contact the Associate Dean for Continuing Education in writing. The grievance will be reviewed
and a response will be returned within 45 days of receiving the written statement. If not satisfied,
an appeal to the Dean of the College of Pharmacy can be made for a second level of review.
Application for CME credit has been filed with the American Academy of Family
Physicians. Determination of credit is pending.
