1. Competition entry payments must be made by cheque or postal orders only and must be sent
with your entry form(s).
2. Payments to enter on the day of the competition will not be accepted even if entry forms have
been sent without payment.
3. Please write the name of the competiton and the dancers name(s) on the back of the cheque /
postal order.
4. One cheque or postal order per competition, one entry form per dancer must be filled out, more
than one dancers entry forms and fees can be paid with the same cheque or postal order but
only for the same competition.
5. All cheque and postal order payments must be made out to the S.O.H.D.A unless other wise
stated.
6. Dancers must be a paid up member of the SOHDA to dance the members dance, please enter
your membership number in the section provided.
